Premature extravasation. A bleeding site identified during the dynamic phase of Tc-99m red blood cell bleeding scintigraphy.

Tc-99m RBC scintigraphy is favored by many investigators because it provides the ability to image the abdomen over a prolonged period of time, thereby allowing identification of delayed bleeding sites that are frequently encountered due to the intermittent nature of gastrointestinal bleeding. The authors describe a case of bleeding scintigraphy with labeled red blood cells in which the bleeding site was identifiable only on the dynamic blood-flow and first static images. On later images, the labeled blood cells had spread throughout the colon, rendering localization of the actual bleeding site impossible. Two previous red blood cell scintigraphies and a subsequent contrast angiogram did not reveal sites of active bleeding. As illustrated by this unusual case, factors governing timing and visualization of abnormal bleeding sites are discussed, as is a differential diagnosis of abnormal foci of activity seen on the dynamic phase of bleeding scintigraphy.

GM1 ganglioside treatment of spontaneously hypertensive stroke prone rats.

Many reports indicate that GM1 ganglioside is effective in reducing CNS ischemic injury in animal models. These models employ invasive surgery to induce ischemic damage in otherwise healthy animals. The purpose of this study was to determine if the beneficial effects of GM1 could be generalized to Spontaneously Hypertensive Rats-Stroke Prone (SHRSP). The SHRSP strain develops a pathology similar to those observed in patients with stroke. The SHRSP have "risk" factors that include hypertension, fibrinoid necrosis, and sensitivity to diet. Female SHRSP were randomly assigned to GM1- or saline-treatment conditions. Rats were fed a stroke-inducing diet. Daily body weights, weekly blood pressure, time of stroke onset, and age at death were recorded. Spontaneous activity and performance on a tail-hang test were assessed thrice weekly. The results indicate that GM1 treatment did not delay the time of stroke onset or death. GM1 did reduce hyperactivity in the initial stages of the ischemic pathology, but did not prevent the marked decline in behavioral activity observed at later time points. There were no differences in weight loss, performance on the tail-hang test, or number of CNS injury-related symptoms observed. These findings suggest that GM1 was not as effective in decreasing mortality, weight loss, or behavioral deficits in SHRSP as previously reported using other animal models of ischemia. Distinguishing between those animal models in which GM1 is more and less effective may be useful in determining under which clinical situations GM1 is likely to be most suitable.

Dopamine increase in the prefrontal cortex correlates with reversal of haloperidol-induced catalepsy in rats.

The mechanism by which forced swimming reverses, haloperidol-induced catalepsy was examined by measuring dopamine (DA) turnover in the nucleus accumbens-ventromedial caudate (NAC-C) and the prefrontal cortex (PFC) in rats. DA and its metabolites 3,4-dihydroxiphenylacetic acid (DOPAC) and homovanillic acid (HVA) were assessed by microdialysis and high pressure liquid chromatography with electrochemical detection (HPLC-ED) after systemic administration of a cataleptic dose of haloperidol (5 mg/kg) or saline. Haloperidol-induced catalepsy was temporarily suppressed by forced swimming. Haloperidol-treated rats showed an increase of DA, DOPAC, and HVA overflow in the PFC and the NAC-C. This increase was greater in the PFC of rats that were forced to swim. Rats that were not treated with haloperidol but were forced to swim (control group) showed an increase of DA, DOPAC, and HVA in the PFC but not in the NAC-C. Zero micrograms, 5 micrograms, 10 micrograms, and 20 micrograms of DA was bilaterally injected in the PFC of cataleptic rats to evaluate the hypothesis that DA in the PFC reverses catalepsy. Haloperidol-induced catalepsy was diminished by bilateral microinjections of 10 micrograms and 20 micrograms but not by 5 micrograms of DA in the PFC. The higher the dose of DA, the longer the decrease of catalepsy. These results suggest that an increase of DA turnover in the PFC might mediate temporal suppression of haloperidol-induced catalepsy. The mechanism by which the mesocortical DA system reduces catalepsy is discussed.

Effect of two tidal volumes on oxygenation and respiratory system mechanics during the early stage of adult respiratory distress syndrome.

PURPOSE: To study the effect of two tidal volumes on gas exchange, lung mechanics, and hemodynamics in 12 patients with acute respiratory distress syndrome (ARDS) within the first 72 hours of mechanical ventilation. METHODS: Tidal volume (VT) was increased by 40% from the initial value at fixed positive end-expiratory pressure (PEEP) and matched minute ventilation by adjusting the respiratory rate (RR) of the ventilator. Initial VT and RR were 592 +/- 42 mL and 19 +/- 1 min-1, respectively. High VT amounted to 825 +/- 54 mL with a RR of 12 +/- 1 min-1. RESULTS: We found that at high VT (1) the index of oxygenation increased from 0.22 +/- 0.03 to 0.32 +/- 0.04 (P < .001) with a parallel decrease in the right to left venous admixture from 0.26 +/- 0.02 to 0.23 +/- 0.02 (P < .001), and in the ratio of physiological dead space to tidal volume (VDS/VT) from 0.53 +/- 0.05 to 0.46 +/- 0.04 (P < .01), without impairment to hemodynamics and (2) respiratory system compliance improved significantly from 34.8 +/- 2.8 mL/cm H2O to 37.2 +/- 2.9 mL/cm H2O (P < .05). In 4 patients, we performed pressure-volume curves on PEEP with the ventilator finding an upward concavity reflecting progressive alveolar recruitment with increasing inflation volume in 3. CONCLUSIONS: High-tidal ventilation in the early stage of ARDS improved gas exchange, suggesting recruitment during the inspiratory phase. However, the benefit of better oxygenation should be weighed against the potential risk of barotrauma induced at high VT.

Cutaneous expansion using enforced position in the treatment of large skin defects.

This paper presents a new method of reconstruction of a large skin defect, e.g., following excision of a giant pigmented nevus. By taking advantage of the flexibility of the joints and the extensibility of skin in the children, the large defects were closed in one stage by an enforced position that allows an easy closure of the wound. The scar was protected against stretching for 6 weeks to obtain sufficient strength. Later, free movements across the joints were permitted to allow gradual tension to the skin surrounding the scar. The final result is a permanent gain of the tissue replacing the defect.

Virulence of patient and water isolates of Legionella pneumophila in guinea pigs and mouse L929 cells varies with bacterial genotype.

Thirteen isolates of Legionella pneumophila serogroup 1 (five from patients, eight from water) were screened for their virulence in guinea pigs after intraperitoneal injection and for their infectivity in L929 mouse cells. Since these isolates included three monoclonal antibody subtypes and four genotypes, the relative influence of these parameters on the pathogenicity of naturally occurring L. pneumophila could be assessed. There was no correlation between infectivity in the L929 assay and virulence for guinea pigs. The source of the isolate, patient or environmental, as well as the isolate's monoclonal antibody subtype did not correlate with virulence. At the p < 0.05 level, isolates with genotype IIb (20-MDa plasmid and EcoRI fragmentation pattern b) were significantly more virulent (mean log LD50 6.84) than genotype VIb (100-MDa plasmid, pattern b), IIId (72- and 96-MDa plasmids, pattern d) or Oc (no detectable plasmid, pattern c) isolates. Genotype IIId isolates were the least virulent (mean log LD50 9.49). Plasmid-containing isolates were more infective than plasmidless ones in L929 cells (p = 0.0001). We conclude that our strain types of L. pneumophila exhibit a gradation in virulence for guinea pigs and that infectivity in L929 cells does not correlate with virulence for guinea pigs.

Mechanism of acidification along cortical distal tubule of the rat.

The cellular mechanism of luminal acidification (bicarbonate reabsorption) was studied in cortical distal tubules of rat kidney. The stopped-flow microperfusion technique was applied to early distal (ED) and late distal (LD) segments, perfused with bicarbonate Ringer solution to which specific inhibitors were added, to measure bicarbonate reabsorption [HCO3 flux (JHCO3)]. pH and transepithelial potential difference (Vt) were recorded by double-barreled H+ exchange resin/reference (1 M KCl) electrodes. Amiloride increased stationary pH and reduced Vt in both early and late segments. Hexamethylene-amiloride (HMA), a specific Na(+)-H+ exchange blocker, reduced JHCO3 in both segments (ED by 43.6 and LD by 40.3%) without affecting Vt. Benzamil, an Na(+)-channel blocker, reduced Vt by 75.9 in ED and 74.9% in LD but had no significant effect on acidification in both segments. The specific inhibitor of H(+)-ATPase, bafilomycin A1, inhibited LD JHCO3 at a concentration of 2 x 10(-7) M by 49%, but ED was inhibited by 24% only at 2 x 10(-6) M. Sch-28080, an inhibitor of gastric H(+)-K(+)-ATPase, reduced JHCO3 by 35% in LD of K(+)-depleted rats but not in control rats and had no effect on ED. These data indicate that, in ED, bicarbonate reabsorption is mediated mostly by Na(+)-H+ exchange. In LD, there is evidence for contribution of Na(+)-H+ exchange, vacuolar H(+)-ATPase, and H(+)-K(+)-ATPase (in K(+)-depleted rats) to bicarbonate reabsorption.

Relationship between expired capnogram and respiratory system resistance in critically ill patients during total ventilatory support.

To examine the relationship of expired capnograms and respiratory system resistance (Rrs) in intubated critically ill patients, we consecutively studied 41 mechanically ventilated patients to (1) analyze the association between expired CO2 slope and auto-positive end-expiratory pressure (auto-PEEP), between Rrs and auto-PEEP, between Rrs and expired CO2 slope, and between Rrs and arterial minus end-tidal PCO2 gradient (PaCO2-PETCO2 gradient) and (2) to investigate the capacity of the expired CO2 slope and PaCO2-PETCO2 gradient to predict Rrs during mechanical ventilation. Regression analysis found a close correlation between Rrs and expired CO2 slope (r = 0.86; p < 0.001), between Rrs and auto-PEEP (r = 0.75; p < 0.001), and between auto-PEEP and expired CO2 slope (r = 0.74; p < 0.001). Weak correlation was found between Rrs and PaCO2-PETCO2 gradient (r = 0.48; p < 0.01). Prediction interval limits at 95 percent confidence level for Rrs are approximately +/- 7.39 cm H2O/L/s from the predicted value obtained by the regression equation, where Rrs = 11.42 + 2.28 expired CO2 slope. These observations suggest that CO2 elimination in critically ill patients is strongly modulated by lung, airway, endotracheal tube, and ventilator equipment resistances. Although continuous capnogram waveform monitoring at the bedside might be useful to assess Rrs, very accurate predictions could be done only in determinate patients.

The role of the distal nephron in the regulation of acid-base equilibrium by the kidney.

The present paper reviews mechanisms by which the kidney controls systemic acid-base balance, with emphasis on the role of the distal nephron, and particularly of the cortical distal tubule. These mechanisms are essentially based on H-ion transport along the whole nephron. In proximal tubule cells, approximately 80% of H-ion secretion is mediated by Na+/H+ exchange, and 20% by H(+)-ATPase. In the distal nephron, acid-base transport mechanisms are located mainly in intercalated cells. H-ion secretion is effected by vacuolar H(+)-ATPase in alpha-intercalated cells and, in K-depleted animals, also by the gastric type H/K ATPase. In animals in alkalosis, beta-intercalated cells secrete bicarbonate by an apical Cl-/HCO3- exchanger, while a basolateral H-ATPase transfers H-ions into the interstitium. In cortical distal tubule, these mechanisms have been shown to be present in the intercalated cells of the connecting segment and of the initial collecting duct (the late distal tubule of micropuncture experiments). In the convoluted distal tubule (early distal tubule), most H-ion secretion occurs by means of the Na+/H+ exchanger. These data show that the distal nephron, including the cortical distal tubule, is a nephron segment responsible for a sizeable portion of bicarbonate reabsorption and titratable acid generation, as well as for bicarbonate secretion under appropriate metabolic conditions, being therefore the site of fine regulation of renal mechanisms that maintain acid-base homeostasis.

Inflation static pressure-volume curves of the total respiratory system determined without any instrumentation other than the mechanical ventilator.

OBJECTIVE: To assess the accuracy of static pressure-volume (PV) curves of the total respiratory system performed with the data directly obtained from the Servo 900 C vs. the data obtained from an external calibrated device. DESIGN: Performance of the PV curve by the method of Levy with simultaneous recording of data obtained from both systems. SETTING: The general ICU of Hospital de Sabadell. PATIENTS: Ten sedated and paralyzed patients ventilated in the control mode for acute respiratory failure were evaluated. INTERVENTIONS AND MEASUREMENTS: Inflation static PV curves were performed by the method of Levy. We simultaneously measured airway pressure and volume by means of calibrated pressure transducer and pneumotachograph and by the internal devices built in the ventilator Siemens Servo 900 C. Statistics were concordance analysis between the two methods and covariance analysis between derived curves. RESULTS: concordance analysis between both methods showed a 95% confidence interval of (+4%, -5%) in volume and (+2.2 cmH2O, -1.7 cmH2O) in pressure. Derived PV curves analyzed by MANOVA showed no significant differences whichever the method used within subject (p = 0.579). CONCLUSION: Inflation static pressure volume curves of the total respiratory system can be accurately performed with the data directly obtained from the Servo Ventilator 900 C without the need of any other external device.

Pressure support ventilation via face mask in acute respiratory failure in hypercapnic COPD patients.

OBJECTIVE: To test whether non-invasive ventilation via facial mask could reduce the need for tracheal intubation when mechanical ventilation must be initiated in COPD patients. DESIGN: Open prospective interventional study. SETTING: General Intensive Care Service of a County Hospital. PATIENTS: We have studied 12 COPD patients during 14 episodes of acute exacerbation of chronic respiratory failure who failed to improve with intensive medical therapy and showed impairments in severe respiratory acidosis and/or hypercapnic encephalopathy leading their attending physicians to order mechanical ventilation. INTERVENTIONS: In these circumstances, a trial of pressure-support (PS) ventilation (Servo Ventilator 900C) via facial mask Vital Signs Inc.) was performed. The level of pressure support was adjusted to obtain a tidal volume > 400 ml. If the patient deteriorated, tracheal intubation and standard mechanical ventilation were performed. MEASUREMENTS AND RESULTS: Measurements are presented as means +/- SEM. A pressure-support level of 14 +/- 3 cmH2O was used during a period of 8 +/- 4 h. Low levels of external PEEP were used in 4 patients, while it generated excessive leaks in the others. Significant differences (p < 0.05 ANOVA for repeated measures) in data obtained on admission, when patients deteriorated and after pressure support was administered were only observed in PaCO2 (68 +/- 3 versus 92 +/- 3 versus 67 +/- 3 mmHg), arterial pH (7.27 +/- 0.03 versus 7.19 +/- 0.02 versus 7.31 +/- 0.01). SaO2 (60 +/- 4 versus 86 +/- 3 versus 92 +/- 1%) and respiratory rate (35 +/- 2 versus 32 +/- 2 versus 23 +/- 1 breaths.min-1). Three patients needed intubation and one of them died in the ICU. CONCLUSION: Non-invasive ventilation (pressure-support) via face mask may reduce the need for tracheal intubation in the severe hypercapnic failure of COPD patients.

Plasmodium vivax VK247 and VK210 circumsporozoite proteins in Anopheles mosquitoes from Andoas, Peru.

Anopheles mosquitoes captured in Andoas, Peru, a Plasmodium vivax-endemic area in the Peruvian Amazon region, contained both VK210 and VK247 P. vivax circumsporozoite (CS) proteins. Approximately 0.9% of the 4,403 mosquitoes tested by enzyme-linked immunosorbent assay were positive; 28% and 72% of the positive mosquitoes contained VK210 and VK247 CS proteins, respectively. These findings correlate strongly with a recent report of the presence of antibodies in residents of this area that recognize the VK210 and VK247 repeats, clearly indicating that both P. vivax CS protein polymorphs are common in the region.

Microlaparoscopy: a preliminary report.

A series of 30 microlaparoscopies performed under local anaesthesia and sedation are presented. The visualization of the pelvic organs was acceptable and the patients reported mild discomfort only. Microlaparoscopy may potentially replace macrolaparoscopy in selected cases.

Endobronchial lipomas.

We report 2 cases of endobronchial lipomas diagnosed by biopsy through a fiberoptic bronchoscope. Its endoscopic differences with bronchial adenomas are analyzed. Computerized tomography contribution to differential diagnosis with other pulmonary tumors is reported. Early diagnosis is advised and the endoscopic removal, before pulmonary complications are observed, is thought preferable to surgery.

Effect of bafilomycin on proximal bicarbonate absorption in the rat.

To evaluate the relative importance of the V-type H(+)-ATPase in proximal bicarbonate reabsorption in vivo, proximal tubules of male and female Wistar rats (180 to 260 g) were perfused with bicarbonate-Ringer solution with and without the addition of 2 microM bafilomycin A1. Bafilomycin significantly increased stationary pH from 6.75 +/- 0.05 (N = 39) to 6.86 +/- 0.03 (N = 82), the stationary concentration of bicarbonate from 5.24 +/- 0.62 to 6.33 +/- 0.46 mM and the half-time of acidification from 3.72 +/- 0.22 to 4.65 +/- 0.25 s, and significantly decreased net bicarbonate reabsorption from 3.17 +/- 0.21 to 2.55 +/- 0.15 nmol s-1 cm-2, that is, by 20%. Since bafilomycin is considered to be a specific inhibitor for V-type H(+)-ATPase, these data establish 1) the existence of this type of transport in the rat proximal tubule and 2) that approximately a fifth of the total proximal bicarbonate reabsorption is due to this mechanism of transport.

Fat embolism syndrome and pulmonary microvascular cytology.

Pulmonary microvascular cytology consists of analysis of capillary blood sampled while a Swan-Ganz catheter is in the wedge position. This technique has proved to be useful in the diagnosis of lymphangitic spread of carcinoma in the lungs and there are case reports of their use in amniotic fluid embolism. Its usefulness in diagnosing fat embolism syndrome has been shown only rarely. We report a new case in which pulmonary microvascular cytologic study allowed a definite diagnosis of fat embolism syndrome. We suggest obtaining routinely samples of capillary blood when a pulmonary catheter is in place and fat embolism is suspected on a clinical basis.

Object chaining and thought disorder in schizophrenic speech.

The phenomenon of object chaining was investigated to determine its relationship to thought disorder in schizophrenia. Samples from thought-disordered schizophrenics (N = 12) and controls (10 non-thought-disordered schizophrenics and 10 normals) were analysed. Using the object subject ratio (OSR) to measure object chaining, we found higher OSRs in the speech of thought-disordered subjects than in that of subjects free of thought disorder. Object chaining correlated with low predictability of speech. We conclude that object chaining is associated with reduced speech comprehensibility and probably contributes to the judgement that thought disorder is present.
