Immersion ultrasound of testicular pathology.

A water-coupled free-immersion ultrasound scanner, the Octoson, was used to study 70 patients with scrotal masses. Scrotal swelling can be identified accurately as intrascrotal fluid collections of solid lesions, and an intra- or extra-testicular origin can be determined precisely. In most cases, testicular tumors can be distinguished from inflammatory conditions. Immersion testicular ultrasound can disclose the site, delineate the extent, and characterize the nature of intra- and extra-testicular lesions.

Effects of a beta adrenergic drug and a secretolytic agent on regional mucociliary clearance in patients with COLD.

The effect of clenbuterol, a beta adrenergic drug, and ambroxol, a secretolytic agent, on regional mucociliary clearance and pulmonary radioaerosol distribution was investigated in two groups of 15 patients with COLD in a double-blind cross-over trial with placebo. Clearance rates of inhaled 99mTc-labelled human serum albumin minimicrospheres (HAMM) were determined over upper, middle, lower, central, and peripheral anterior lung regions. Additionally, an index was employed for quantitative analysis of initial pulmonary aerosol distribution in order to detect changes in the site of particle deposition caused by the drugs. Regional measurement of tracheobronchial particle clearance showed clenbuterol to have a significant (P less than 0.05) stimulating effect in 4 of 5 tested pulmonary zones resulting in significantly accelerated whole lung clearance. Ambroxol was effective in only 1 of 5 tested lung areas and did not prove to enhance whole lung clearance significantly. The secretolytic agent was associated with significant (P less than 0.05) improvement of lung aerosol distribution in obstructive emphysematous patients, whereas no significant change in lung deposition of the inhaled particles was encountered in the patients with chronic obstructive bronchitis after either drug regimen.

[The action of oxyfedrine on haemodynamics, inotropism and blood perfusion of the partially ischaemic myocardium after digoxin premedication. Studies on anaesthetized dogs (author's transl)].

The effect exerted on the partially ischaemic heart when administering 0.9 mg/kg L-3-(beta-hydroxy-alpha-methylphenethylamino)-3'-methoxypropiophenone (oxyfedrine, ildamen) approx. 10 min after i.v. application of 0.05 mg/kg digoxin was tested on 20 dogs previously anaesthetized with propiomazine-pentobarbital. The following parameters were studied and subsequently compound with the results of former trials of ours' without digoxin premedication: aortic pressure (ASP, ADP), left-ventricular pressure (LVSP, LVEDP), heart rate (HR), cardiac output (HMV), stroke volume (SV), dp/dtmax, dp/dtmax/IP, t-dp/dtmax, blood flow in the normal and partially ischaemic myocardium, the latter being measured with heat conductance probes and labelled microspheres. ASP and ADP show the same reduction--as compared with the control value--both after the administration of oxyfedrine with and without digoxin premedication. After digoxin premedication oxyfedrine led to a somewhat less marked reduction of LVSP; on premedication with digoxin LVEDP was slightly increased whereas it was reduced after additional administration of oxyfedrine as was also the case without digoxin pretreatment. The increase in HR after oxyfedrine is almost the same as without digoxin pretreatment. Also the increase in HMV and the SV lowering are not influenced by digoxin. By administration of oxyfedrine dp/dtmax is always increased by the same amount, starting from the already increased value after digoxin premedication, which is probably an additive effect. The same applies to the quotient dp/dtmax/IP. After oxyfedrine the time t-dp/dtmax is lowered by the same amount, irrespective of a digoxin premedication. Oxyfedrine does not produce a further increase in the heat conductance values after previous application of digoxin; when measuring the blood flow with labelled microspheres the same result was found, which means that by previous administration of digoxin the circulatory effect of oxyfedrine is obviously inhibited. Summing up one can say that by combining the active principles digoxin and oxyfedrine the function parameters of the heart can be influenced only positively.

Immersion ultrasonography of scrotal and testicular pathology.

A water-coupled free-immersion ultrasound scanner, the Octoson, was used to study 70 patients with scrotal disease. The main advantage of the immersion technique is the perfect overview of the whole male genital organ. Scrotal masses could be differentiated to a high percentage into intrascrotal fluid collections and intra- or extra-testicular solid lesions. In some cases, testicular tumors could be distinguished from inflammatory conditions on the basis of the ultrasound examination alone. According to out present experience, immersion ultrasound of the scrotum proved to be an effective method in disclosing the site, delineating the extent and characterizing the nature of extra- and intra-testicular disease.

Contrast media-induced side effects on excitation and conduction of electrical activity in the heart on intracardiac application. Investigations in anaesthetized dogs.

In several series of experiments with intracardiac application in anaesthetized dogs, the following contrast media were tested for their adverse effects on excitation and conduction of electrical activity in the heart. Diatrizoate, lysine-diatrizoate, lysine-sodium-diatrizoate, ioxaglate, iopamidol, iodamide, ioxitalamate, ioglicinate and metrizamide. Blood pressure and, using the His' bundle-ECG technique, the parameters P0-P1 (heart rate), A1-H1 period (conduction time in the AV-node), and H1-V1 period (conduction time in the TAWARA branches) were measured. A statistical correlation exists between osmolarity and blood pressure (higher osmolarity causes a greater decrease in blood pressure), as well as between osmolality and decrease in heart rate (higher osmolarity causes a greater decrease in heart rate). The delay in conduction time in the AV-node correlates only with viscosity and sodium content (increased viscosity with increased sodium content delays the time of conduction). A tendency towards delay in conduction in the TAWARA branches could not be correlated to any one of the physico-chemical parameters studied.

[Computed tomorgraphy and ultrasound; comparative study in diseases of the upper abdomen and retroperitoneal space (author's transl)].

In a comparative study with 242 patients (90 renal, 152 pancreatic and abdominal diseases) ultrasound and computed tomography were compared. With an overall accuracy quota of 92.56% for CT and 85.53 for US, we get the following quotas for the organs: kidney (95.5% Ct, 91.1% US), pancreas and abdominal disease (90.78% CT, 82.23% US). CT has the same specificity (CT: 100, US: 97.7) but a greater sensitivity II (making the final diagnosis) than US (CT: 88.15, US: 78.28). The grading scale of diagnostic efficiency and therapy planning shows a higher value for CT (3.12) than for US (2.89). US is often handicapped by sonic shadows behind calcifications and air, CT by motion artifacts and by the lack of far between the organs. Both methods show the same result for differentiation of the enlarged head of the pancreas.

[A new immersion ultrasound technique for the investigation of the scrotum (author's transl)].

The testes of about 50 patients were examined by ultrasound, using a water immersion technique. Immersion sonography provides an excellent and anatomically detailed demonstration of the male genitalia. Intrascrotal fluid collections, extra- and intratesticular solid lesions and inflammatory processes can be accurately differentiated. The method is suitable for showing the size, shape and extent of a scrotal lesion. In many cases the echo pattern can provide information concerning the pathological changes of these organs.

[Quantitative radioaerosol lung imaging in healthy subjects and in patients with chronic obstructive lung disease (author's transl)].

A quantitative method for study of the penetration, deposition pattern and clearance of inhaled particles (O,5-micron-99Tc-labeled human serum millimicrospheres) in the lung is described. 10 healthy subjects and 15 patients with chronic obstructive lung disease (COLD) were studied. For both groups significant differences in the mean values of two indices describing peripheral particle penetration and particle distribution homogeneity were observed. Mucociliary efficiency has been assessed by evaluating aerosol clearance rates from the whole right lung and from upper, middle, lower, perihilar and peripheral zones of the right lung field. Whereas the total lung clearance of both groups showed no marked difference, there was a statistical significant clearance delay in the patient's group upper and peripheral lung area.
