Extension of gastric lymphoma into the esophagus and duodenum.

In 81 patients with gastric lymphoma, spread of tumor beyond the stomach was analyzed based on radiographs and medical records. Tumor extension into the esophagus was present in 10% of patients, and spread across the pylorus into the duodenal bulb was seen in 33%. When the gastric fundus alone was involved, transcardial extension was seen in four of 10 patients, and when only the antrum was involved, transcardial extension was seen in four of 10 patients, and when only the antrum was involved, transpyloric extension was present in six of seven patients. One-third of the patients with gastric lymphoma secondary to disease in adjacent structures had contiguous involvement of the esophagus or duodenum. This study shows that transcardial extension cannot be used to differentiate gastric lymphoma from adenocarcinoma, and its confirms the frequent occurrence of transpyloric tumor extension in patients with gastric lymphoma.

A double-blind study of radiologic accuracy in diverticulitis, diverticulosis, and carcinoma of the sigmoid colon.

A review of the barium enema studies in 73 patients with diverticular disease of the colon and with and without associated neoplasm leads to the conclusion that the correct radiologic recognition of an associated neoplasm in a colon with diverticulitis or diverticulosis can be made only about half the time. The limitations of the radiographic method and radiologic findings in distinguishing diverticulitis from carcinoma should be more clearly recognized by the clinician.

The radiographic appearance of complications following Nissen fundoplication.

Seventy patients had an upper gastrointestinal examination following Nissen fundoplication for reflux esophagitis associated with hiatal hernia. Thirty-nine were asymptomatic and the fundoplication appeared normal. Of the 31 patients with symptoms (dysphagia, pain, or vomiting), 15 had spontaneous relief and demonstrated a normal postsurgical radiographic appearance of the stomach. The other 16 had both persistent symptoms and radiographic abnormalities, including 5 stenoses, 3 recurrent hernias, and 8 pouch deformities of the fundus. The roentgenographic features and etiology of these surgical failures are discussed and the importance of the radiographic examination in discerning successful surgical repair from failure despite similar postsurgical symptoms is stressed.

The state of radiographic technique in the examination of the colon: a survey.

The results of an extensive, multiple choice questionnaire sent to 113 leading medical centers in the world (100 centers responded) were analyzed and compared to those of a similar survey from 1968: single-contrast and double-contrast barium enema procedures are still the standard methods of radiographic assessment of the colon; the double-contrast, barium enema examination is now used more frequently, especially in conjunction with the single-contrast barium enema; preparation of the colon is now more meticulous; and drugs are added more frequently to the barium enema. Equipment has become more sophisticated through increased use of image intensifiers, television, and remote controls. More use is made of videotapes. Water-soluble contrast media are administered more frequently. The use of special radiographic procedures to evaluate the colon has decreased.

Comparison of barium enema nad colonoscopy in the detection of small colonic polyps.

A total of 210 patients was examined by barium enema and colonoscopy. The authors found 219 polyps in 112 patients. Single-contrast barium enema missed 33 (45.2%) and double-contrast barium enema missed 17 (11.7%) polyps. Colonoscopy failed to detect 6 polyps (2.7%) and another 21 (10%) were not seen because the area could not be reached by the colonoscope. Malignancy was present in 5 small polyps. The authors conclude that the double-contrast barium enema offers a definite advantage in the right colon. In general, the two methods are complementary.

Observation on the metabolism of iopanoyl (Telepaque) glucuronide in dogs treated with antibiotics.

The purpose of our investigation was to determine whether iopanoyl glucuronide, the major metabolite of iopanoic acid (Telepaque), undergoes hydrolysis by bacterial beta-glucuronidase in dogs. The conjugated compound was identified and quantitated by elemental analysis, fluorescent excitation analysis, thin-layer chromatography, and high pressure liquid chromatography. The experiments were performed before and after combined antibiotic treatment with neomycin and vancomycin. It was first determined that reabsorption and excretion of sodium iopanoate was only minimally diminished during antibiotic treatment. Known amounts of iopanoyl glucuronide were infused into the small bowel of 4 awake dogs with chronic bile fistula, and bile was collected for 5--8 hours. The excretion of the recirculated conjugated compound was 4--5 times lower during antibiotic treatment. Incubation of ileal fluid with bile containing iopanoyl glucuronide suggested that beta-glucuronidase hydrolyzes the conjugated compound. Hydrolysis was markedly decreased after pretreatment with antibiotics. These findings suggest that the beta-glucuronidase produced by bacteria may be a major mechanism in enterohepatic recirculation of iopanoyl glucuronide. Mechanisms and possible implications are discussed.

Intracolonic pressures during barium-enema studies using the single- and double- contrast techniques.

Six dogs were examined by means of single- and double-contrast barium enemas, and colonic pressures and diameters were recorded at five intervals while the volume of the contrast medium was increased. Examinations were performed with and without administration of glucagon. Changes in colonic pressure and diameter were similar for the two techniques in 77% of the studies with and in 65% of the studies without administration of glucagon. Neither technique produced significantly different pressure or diameter changes with glucagon as compared to without it. Limitations in the experimental technique must be considered before extending results to human subjects.

The influence of fasting on opacification of the gallbladder during oral cholecystography using two consecutive doses in a canine model.

Oral cholecystography was performed in eight adult mongrel dogs. Contrast agents (Telepaque and/or Bilopaque) were administered in six different combinations, given as two consecutive doses with and without food. A combination of Bilopaque alone on both days and a combination of Telepaque with food on the first day and Bilopaque alone on the second day each proved to be the best method in 37.5% of the dogs. Telepaque with food on both consecutive days was the best method in 25% of the dogs. Among the eight combinations, two consecutive doses of Bilopaque alone or a combination of Telepaque with food on the first day and Bilopaque alone on the second day resulted in the most dense opacification of the gallbladder. Clinical implications of these results are discussed.
