Myodermal flap closure of below the knee amputation.

Primary healing was achieved in 20 of 21 below the knee amputations which were closed with a gastrocnemius myodermal flap. A version of this versatile flap is available for closure of any below knee amputation. Preservation of the perforating muscular arteries in these myodermal flaps will maximize circulation to the skin of the lower extremity.

Breast reconstruction after mastectomy.

Recreation of normal breast anatomy and restoration of female body image after mastectomy for cancer are the goals of the reconstructive surgeon. A blending of ideas has begun between the general surgeon performing the mastectomy and the plastic surgeon reconstructing the breast. Breast reconstruction in selection patients has become an accepted part of rehabilitation. It is the purpose of this paper to address the problems of breast cancer and patient selection for reconstruction and to outline the surgical techniques.

Pilar tumors.

We present 5 cases of pilar tumor, and we review the controversy over whether this tumor has a malignant potential. We feel the correct treatment is excision deep to galea, with a one to two cm margin of normal scalp. Presented with a pathology report of "pilar tumor" on a "shelled out" or ruptured sebaceous cyst, we feel the correct treatment is complete excision of the previous scar, going deep to the galea and taking a one to two cm margin of surrounding normal scalp.
