Sepsis of the cheek over a subcutaneous forehead flap.

In two cases, a temporal flap was folded medially and passed through a subcutaneous tunnel superficial to the parotid gland. In both cases sepsis developed at the point at which the flap entered the mouth. In one case a spontaneous perforation occurred and the other required drainage. If one elects to fold the flap medially and pass it superficial to the zygomatic arch, the route described by Lewis and Remensnyder, i.e., deep to the masseter, is preferable.

Management of the neurogenic bowel using durolax solution.

A simple and effective bowel care regime is described which has been in use in the Spinal Injuries Unit at Prince Henry Hospital, Sydney, for more than 10 years. Three times a week, Durolax solution (bisacodyl 10 mg in 5 ml of propylene glycol), usually 5 mL undiluted or diluted with 5 mL of tap water, is introduced into the rectum using a catheter and syringe. A faecal softener is given 24 hours before the Durolax solution. Evacuation usually results within 30 minutes, tending to occur sooner with continued use of the regime. The procedure is well tolerated and there have been no serious problems.

A new pressure measuring system for cushins and beds--with a review of the literature.

An interface pressure measuring device for the assessment of subject-cushion interface pressures at ten points has been described. Preliminary trials using 12 healthy subjects on four cushions and three spinal injury patients on six cushings, has been carried out. It was also found that the water and foam cushion distributed the pressure over a large area than the foam cushions.

Description of strains of Peptostreptococcus anaerobius isolated from subcutaneous abscesses in cats.

Strains of Peptostreptococcus, Streptococcus and of a Gram-positive coccus, which was initially isolated as an anaerobe but grew subsequently as a facultative organism, were isolated from subcutaneous abscesses in cats. The cat strains of Peptostreptococcus gave metabolic fermentation products in combinations described for P. anaerobius. The Streptococcus strains conformed to the group S. intermedius. The facultative organism described had the metabolic products of P. anaerobius but the distinctly different biochemical characteristics of S. intermedius and fits neither of the genera strictly.

Isolation and characterisation of bacteria from abscesses in the subcutis of cats.

Thirty-six closed abcesses in the subcutis of cats were examined. Of 168 bacterial strains isolated, 121(72%) were anaerobes and 47 (28%) were facultative anaerobes. Twenty-six abscesses contained mixtures of facultative anaerobes and anaerobes, six contained anaerobes only and four contained facultative anaerobes only. Bacteriodes was the genus most commonly isolated (28.6% of all isolates) followed by Fusobacterium (19.0%) and Pasteurella (multocida) (13.1%). Peptostreptococcus anaerobius was the most commonly isolated anaerobic species (13.2% of anaerobic isolates and 9.5% of all isolates)and Past, multocida was the most commonly isolated facultative anaerobe (46.8%; 13.1%of all isolates).

Evaluation of the relationship of sodium in drinking water and toxemia of pregnancy.

This study is a retrospective case comparison analysis of the relationship of water-borne Na+ and toxemia of pregnancy as a part of a continuous investigation at our facility on the role of various water constituents in human chronic disease. Five hundred thirty-seven toxemic pregnant women delivered at Jefferson Davis Hospital, Houston, Texas, during 1976 were matched by age, race, obstetrical history, and month of delivery to 537 non-toxemic controls. Sodium concentration in each patient's drinking water was derived, based on her address, from earlier prepared maps of sodium distribution in Houston's water supplies. Paired t-tests were performed to compare the Na+ levels, which varied from 19 to 250 mg/l in water supplies of cases and controls. Odds ratio analysis was used to test for the presence of a gradient in occurrence of toxemia in parallel with the gradient of Na+. No significant impact of water-borne Na+ in the indicated range was observed. Further studies incorporating still higher ranges are contemplated.

Hyperbaric oxygen and acute spinal cord injuries in humans.

Clinical assessment of a regime of hyperbaric oxygen within 12 hours of acute spinal injury in humans suggests that further study of this method of treatment is indicated. For statistical proof of the efficacy of this form of treatment study of a large number of patients is necessary and an Australia-wide study is suggested. A recommendation is made for early referral to the spinal unit.

Perinatal foal mortality associated with a herpesvirus.

An outbreak of perinatal foal mortality associated with a herpesvirus is described. Twenty two foals either were still-born, or died soon after birth, or were weak and soon developed severe respiratory signs, or were normal at birth and developed respiratory symptoms 18 to 24 hours later. Elevated temperatures, heart and respiratory rates were constant features. The animals were severely leucopaenic, and showed an absolute neutropaenia. At autopsy the lungs were enlarged, and showed varying degrees of aeration and moderate to severe oedema and congestion. Histopathology showed an acute focal necrotising bronchiolitis with the presence of intranuclear eosinophilic inclusion bodies. Herpesvirus was recovered from 9 foals in cell culture and identified by electron microscopy.

Hormonal induction of mammary tumor viruses and its implications for carcinogenesis.

The purpose of this study was to evaluate the possibility that hormones and mouse mammary tumor virus (MuMTV) are cocarcinogenic for mammary epithelium. Results of our investigations in vivo and in vitro suggest: (a) Hormones promote mammary carcinogenesis in BALB/c females whether MuMTV is germinally (BALB/c) or horizontally (BALB/cfC3H) transmitted; the rate of carcinogenesis in BALB/cfC3H females is substantially faster than it is in BALB/c, but the final mammary carcinoma incidence is approximately the same. The rate-limiting step in malignant transformation in BALB/c, which infectious MuMTV overcomes, is in premalignant transformation from normal. (b) One characteristic of horizontal MuMTV transmission in BALB/c that is not observed in germinal transmission is integration of new MuMTV sequences in mammary cell DNA. Integration is mammary cell specific and constant at 2 to 3 copies/cell from tumor to tumor. (c) MuMTV expression is changed in mammary epithelial cells during hormonal carcinogenesis. The nature of the change is qualitatively similar in both BALB/c and BALB/cfC3H. Expression of envelope glycopeptides (glycoprotein with a molecular weight of 52,000) is induced, which correlates with amplification of MuMTV RNA sequence content. Quantitative differences exist in induced levels in BALB/c and BALB/cfC3H. (d) MuMTV RNA and a glycoprotein with a molecular weight of 52,000 were not inducible with dexamethasone in normal mammary epithelial cells in culture. These structural components were induced in both premalignant (BALB/cfC3H) and malignant (BALB/c and BALB/cfC3H) cells. MuMTV RNA was induced by dexamethasone in normal cells pretreated with 5-iodo-2'-deoxyuridine. (e) Both premalignant and malignant cells have altered (vis-à-vis normal) surfaces, discernible by differences in reactivity with concanavalin A in hemadsorption assays. Indirect evidence suggests that the alteration includes membrane incorporation of MuMTV-related determinants of a glycoprotein with a molecular weight of 52,000. (f) Malignant cells exhibit enhanced sensitivity to insulin for reinitiation of DNA synthesis and mitosis in contact-inhibited homotypical monolayers. These findings have been organized into a hormonal cocarcinogenesis hypothesis in which expression of germinally transmitted MuMTV genes is the proximal cause of neoplastic transformation.

Practolol and deafness.

Practolol ('Eraldin') is a beta-blocking agent whose prolonged use may result in the appearance of untoward side-effects, notably ocular symptoms, skin rashes and deafness. We report ten cases of deafness due, or thought to be due, to practolol; and in all except three of the cases (8,9 and 10), the deafness became manifest many months after the appearance of lesions in the skin and eyes. Progression of any hearing loss caused by this drug seems to be arrested by its withdrawal. Otologists are urged to report any suspicious cases to the Committee on Safety of Medicines, Finsbury Square House, 33/37A Finsbury Square, London EC2A IPP.

Lethal and mutagenic effects of nitrosoguanidine on synchronized Chlamydomonas.

The lethal and mutagenic effects of 5 mug/ml N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) were maximal during the nuclear S-period of synchronously grown Chlamydomonas reinhardtii. This was revealed by a 50% drop in survival and a 50- to 100-fold increase in the recovery of slow-growth mutants (up to 40% of the survivors) which were first recognized as small colonies on agar medium. Partial characterization of these isolates revealed about 50% to be stable on subculture, and several were demonstrated to be either acetate-dependent, dark-lethal (light-dependent), or acetate-sensitive mutants. There was no significant increase of lethality or of slow-growth mutants correlated with treatment during the chloroplast DNA replication phase of the cell-cycle. The results of genetic analysis with 13 mutants induced during the nuclear S-period were consistent with their nuclear origin. These analyses were hampered by the high proportion of lethality among the progeny of most crosses. It is concluded that the enhanced mutant induction among nuclear S-phase cells may indicate preferential mutagenesis of replication fork DNA and induction of multiple-closely-linked mutations, as in some bacteria. Consequently, for C. reinhardtii, caution should be exercised in drawing relationships between abnormal behavioral and biochemical phenotypes in MNNG-induced mutants.
