A case-control study of hantavirus pulmonary syndrome during an outbreak in the southwestern United States.

In May 1993, an outbreak of hantavirus pulmonary syndrome (HPS) occurred in the southwestern United States. A case-control study determined risk factors for HPS. Seventeen case-patients were compared with 3 groups of controls: members of case-patient households (household controls), members of neighboring households (near controls), and members of randomly selected households > or = 24 km away (far controls). Investigators trapped more small rodents at case households than at near (P = .03) or far control households (P = .02). After the number of small rodents was controlled for, case-patients were more likely than household controls to hand plow (odds ratio [OR], 12.3; 95% confidence interval [CI], 1.1-143.0) or to clean feed storage areas (OR, 33.4; 95% CI, 1.7-666.0). Case-patients were more likely than near controls to plant (OR, 6.2; 95% CI, 1.1-34.0) and more likely than far controls to clean animal sheds (OR, 11.9; 95% CI, 1.4-103.0). Peridomestic cleaning, agricultural activities, and an increased number of small rodents at the household were associated with HPS.

Separation and purification of recombinant proteins from Escherichia coli with aqueous two-phase systems.

The partition of the protein thaumatin in the presence of Escherichia coli contaminant proteins has been studied. Extraction of thaumatin was followed by back-extraction of the product into a new phosphate phase and also back extraction combined with recycle of the top polyethylene glycol (PEG) phase. When partitioned in the absence or presence of insoluble cell debris (whole cell homogenate) little effect on the partitioning of thaumatin or the soluble E. coli protein was observed. A back extraction step that allowed for dilution of the NaCl was successful in extracting thaumatin back into a heavy phosphate phase. The PEG top phase was recycled to the first extraction stage. The stability ratio (tie-line length) had an effect on the average partition coefficient (Kapp) of E. coli soluble proteins in PEG-phosphate systems but in PEG sulphate systems Kapp was independent of this ratio. An increase in NaCl resulted in an increase in Kapp but this was always below 1. A mathematical model that describes the continuous steady-state operation of extraction and back extraction has been developed; it is based on steady state mass balances of the main components and phase equilibrium data and was successfully used to simulate the extraction and back extraction processes.

An evaluation of patient-controlled analgesia in adults undergoing cardiac surgery.

OBJECTIVE: To evaluate the effectiveness of patient-controlled analgesia (PCA) in postoperative cardiac surgery patients. DESIGN: Prospective evaluation. SAMPLE: Ten patients who had undergone coronary artery vein grafting. SETTING: A cardiac intensive care unit in a private clinic in London, UK. MEASUREMENTS: Pain scores, level of sedation and amount of analgesia and sedation required were recorded, together with each patient's perception of PCA. RESULTS: Patients had very little recall of their time in the ICU and none was aware of pain during this period. Patients liked managing their own pain. CONCLUSION: PCA is an effective means of controlling postoperative pain in cardiac surgery patients.

The clinical presentation of command hallucinations in a forensic population.

In a forensic population, patients with command hallucinations (N = 25) were compared to two groups of psychotic patients: those with noncommand hallucinations (N = 24) and those without hallucinations (N = 16). The three groups did not differ in overall impairment as measured by the Global Assessment Scale and the Social Behavior Rating Schedule. However, the group with command hallucinations differed in the content of their hallucinations (more aggression, dependency, and self-punishment themes), and nearly one-half did not report or denied their command hallucinations to the assessment team. Many patients (N = 11, 44%) reported that they frequently responded to hallucinatory commands with unquestioning obedience.

Self-association of hyaluronate segments in aqueous NaCl solution.

The potential for self-association by hyaluronate (HA) chains in 0.15 M NaCl was investigated, using low molecular weight HA segments as a model system. HA segments were derived from the polymer by controlled enzymatic digestion, and purified by gel filtration chromatography. Seven samples of narrow molecular weight distribution were analyzed by sensitivity-enhanced polyacrylamide gel electrophoresis, and found to have the following weight-average numbers of repeating disaccharide units: A, 90; B, 51; C, 38; D, 31; E, 23; F, 18; G, 13. The segment preparations were studied in 0.15 M NaCl by capillary viscometry, low angle laser light scattering, and circular dichroism spectroscopy. The data indicate concentration-dependent intermolecular association of short segments, and a capability for intramolecular association (hairpin formation) by larger HA segments.

Forensic psychiatrists' and psychologists' understanding of insanity: misguided expertise?

The ability of mental health experts to consult on insanity cases is highly dependent on their accurate understanding of the current Canadian standard. The study surveyed experienced forensic psychiatrists and psychologists and found the majority (88%) had erroneous beliefs regarding the insanity standard. Results of a discriminant analysis suggest that experience with insanity evaluations and courtroom testimony may improve experts' understanding. Implications of these results are discussed with respect to professional training and public policy.

Cationic dye binding by hyaluronate fragments: dependence on hyaluronate chain length.

Sodium hyaluronate, digested with bovine testicular hyaluronidase, yielded a mixture of oligosaccharides with identical repeating disaccharide structures and differing molecular weights. The oligosaccharides were separated into a ladder-like series of bands by electrophoresis on a 10% polyacrylamide gel matrix. Coelectrophoresis of purified oligosaccharides has established that adjacent bands differ in chain length by one disaccharide unit. This procedure formed the basis for a rapid screening method in which the binding of cationic dyes by hyaluronate oligosaccharides may be assayed. As a function of chain length, the oligosaccharides showed a marked change in dye binding. Species containing less than seven repeating disaccharide units are not detected by any dye tested, even at very high sample loads. Larger oligosaccharides show an increase in dye binding. The chain length at which constant maximal dye binding is reached depends on the dye structure and solvent conditions, varying from approximately 12 to 30 disaccharide units. The hyaluronate fragments of sufficient chain length to duplicate polymer behavior should be useful models for the study of hyaluronate structure and interactions in solution.

A prospective study of Raynaud phenomenon and early connective tissue disease. A five-year report.

To define features of patients with Raynaud phenomenon that predict the evolution of connective tissue disorders, a prospective study was initiated. Patients with history or physical evidence of Raynaud phenomenon without causal or associated disorders underwent initial multisystem evaluation. Scleroderma (systemic sclerosis) of less than five years' duration was included for comparison. Patients were classified as having Raynaud phenomenon "only," undifferentiated connective tissue syndrome or scleroderma. Nailfold capillary microscopy was performed, and patterns were scored blindly from coded photographs. Of 91 patients with Raynaud phenomenon entered (Raynaud phenomenon only, n = 49; undifferentiated connective tissue syndrome, n = 22; scleroderma, n = 20), abnormal "scleroderma pattern" capillaries were noted in seven of 49, 19 of 22 and 19 of 20, respectively (p less than 0.005). Of 39 patients with Raynaud phenomenon only followed (mean duration, 23.7 months) undifferentiated connective tissue syndrome developed in three. Of 17 patients with undifferentiated connective tissue syndrome followed (mean duration, 27.6 months), six patients had transitions (four scleroderma; one scleroderma-systemic lupus erythematosus overlap; one SLE). Nailfold capillary abnormalities best identified transitional patients in both groups (eight of nine) and were more sensitive than ANA (six of nine), presence of digital ulcers (four of nine) or decreased esophageal motility (two of nine). This prospective study documents a useful role for capillary examinations in evaluating Raynaud phenomenon. Abnormal capillaries indicate an increased risk for connective tissue disease; normal capillaries favor idiopathic Raynaud phenomenon.

Forensic psychiatric assessment in selected Canadian cities.

Forensic psychiatrists in six Canadian municipalities completed a single-page form following each court-ordered assessment conducted during the month of July 1978. A total of 248 cases accumulated during the study period. During the following 18 months researchers collected court dispositions on the sample. With data from all six cities pooled, 85% of patients were found fit to stand trail. Almost all, 96%, were accorded a diagnosis with 39% classified as psychotic. Overall the examining psychiatrists considered 36% to be dangerous to others in the future. When psychiatrists recommended a custodial setting, in 73% of cases the person was incarcerated. Recommendations for hospitalization were observed in 59% of cases. Although the data would seem to suggest that judges take psychiatrists' recommendations into account, the investigators were nonetheless struck by the general lack of communication between clinic and court. They offer a number a number of suggestions for improvement.

Criminal justice system behavior.

Much has been written recently about the underlying motivation of criminals and about the behavior of the criminal justice system itself. Typically, such studies have shown negative correlations between crime rate, probability of apprehension and probability of conviction. However, in addressing the behavior of the system, the present study examines measures of workload for the criminal justice system rather than performance indicators such as those above. Preliminary evidence is examined over several provinces of Canada and is used to investigate the hypothesis that there is adaptation by different subsystems in the criminal justice system to changing workloads in other parts. The initial findings show that present upstream, i.e., toward the beginning of the system, workload affects future behavior of the system downstream, i.e., toward the end, rather than the reverse, especially for the more serious crime types.
