Buffered hydrochloric acid: a modern method of treating metabolic alkalosis.

Twenty-one patients with metabolic alkalosis were treated successfully with intravenous hydrochloric acid (HCl) buffered in an amino acid solution (TPN). No complications of HCl were seen. TPN was used to meet energy needs and provide a buffering effect through the interaction of HCl and amino acids. Buffered HCl therapy should be considered as the initial treatment in patients with metabolic alkalosis associated with congestive heart failure, renal failure, hepatic failure, cerebral edema, or refractory metabolic alkalosis.

Jejunal loop interposition: a remedy for failed jejunoileal bypass.

A jejunal loop interposition procedure has been developed for selected post-jejunoileal bypass patients who require repeated hospitalizations for electrolyte imbalance and malnutrition. Twelve patients have undergone interposition of an additional 6 inches of jejunum into the functional segment of bowel with excellent results. Deficiencies in serum albumin, calcium, magnesium and potassium were rapidly corrected following this operation. Desired weight was maintained, and rapid, dramatic improvement in general state of well-being was observed. No intraoperative complications were encountered and the procedure is easily performed.

Ultrasound as the initial dignostic study in patients with suspected gallstones.

The accuracy, sensitivity, and specificity of ultrasound versus oral cholecystography (OCG) in the preoperative diagnosis of gallstones was studied in 144 patients undergoing cholecystectomy. The 89 per cent accuracy of ultrasound was similar to the 92 per cent achieved with OCG. However, ultrasound was found to have a lower false-positive rate and a greater sensitivity with far fewer indeterminate studies than OCG. A high rate of nonvisualization diminished the usefulness of OCG as a first-line test. Because of a greater sensitivity less tendency for false-positive results, and a moninvasive nature, ultrasound should be the initial diagnostic study of choice in patients with suspected gallstones.

Thyroid cancer: diagnosis and management based on a review of 142 cases.

Review of 142 patients with thyroid cancer diagnosed and treated in a community hospital from 1958 through 1972 suggests that total thyroidectomy and radical neck dissection are unwarranted in the treatment of papillary carcinoma. Radioactive iodine scanning and needle biopsy are also shown to be unreliable indexes of malignancy. An operation approach to the management of specific thyroid cancers is presented.

The surgical clerkship in the community hospital.

Since 1972 Michigan State University College of Human Medicine has participated with community hospitals and practicing physicians in a number of cities to provide surgical clerkship experiences for its students. The problems of satisfying the educational philosophies and objectives of the university, of fulfilling the needs of the practicing physicians and community hospitals, and of supplying a viable educational experience for students are presented and discussed. The new role of a full-time community-based university surgeon to facilitate this program is described. The organization of a surgery clerkship, which includes weekly objectives, nongraded weekly examinations with group feedback sessions, a surgical laboratory, and sessions dealing with the psychosocial aspects of patient care, is presented. Multiple evaluation methods (upon which the credibility of the program depends) are explained.
