Salivary gland tumors in children.

Tumors of salivary gland origin are relatively uncommon in children. Over a 23-yr period, there were 12 such tumors affecting children under 16 yr of age out of a total 937 cases reported by a central cancer registry. The commonest lesion was the pleomorphic adenoma involving the parotid gland. Wide focal excision seemed to be an adequate procedure as long as the tumor was completely removed. Inadequate excision resulted in multiple local recurrences with the tumor taking on more malignant characteristics. Early radical excision, with preservation of the facial nerve, is advocated.

Management of airway complications of burns in children.

Children who have been exposed to smoke in a confined space or who have soot or burns, however minimal, on the face should be admitted to hospital. Respiratory distress may be delayed, but if it is progressive the patient should be curarised, intubated, and mechanically ventilated. Unless ventilation continues for 48 hours, followed by 24 hours' spontaneous respiration against a positive airway pressure, stridor and pulmonary oedema may recur. An endotracheal tube small enough to allow a leak between it and the oedematous mucosa must be passed to prevent laryngeal damage and subsequent subglottic stenosis. High humidity of inspired gases keeps secretions fluid and the endotracheal tube patent. A high oxygen concentration compensates for deficient oxygen uptake and transport caused by pulmonary lesions and the presence of poisonous compounds interfering with oxygen transport. Dexamethasone to minimise cerebral oedema and antibiotics to reduce the incidence of chest infections should be given.

The advantages of a ventriculostomy reservoir with the Holter valve system.

The use of a Rickham ventriculostomy reservoir incorporated in the Holter valve system is advocated. In a retrospective study of 173 button tape, the clinical assessment of the valve was proved correct in 51% of occasions and in a prospective review of 100 taps the clinical findings were correct in 53%. The complication rate of tapping the reservoir is negligible.

Signifance of faecoliths in the diagnosis of acute appendicitis.

Three cases are presented which demonstrate the significance of faecoliths in the diagnosis of acute appendicitis in infants. A wide spectrum of symptoms is noteworthy. The usefulness of a plain X-ray of the abdomen in infants with acute abdomen is stressed.
