In connection with a case revealed by digestive tract hemorrhage per rectum, the authors emphasize the infrequency of Meckel's diverticula leiomyomas and stress the value of selective arteriography of the superior mesenteric artery as a means of pre-operative identification of these tumors.
The authors describe the extra- and intra-hepatic echographic images of the portal system in the foetus and the adult on the basis of data from literature and their personal experience with angiographic comparisons and investigation of anatomical specimens. The knowledge gained by echography provides the means for a sure approach to systematisation of the liver and thus accurately identifies the site of focal processes (tumour, abscess, etc.) furnishes very useful information in a large number of pathological cases (foetal hypotrophy, portal hypertension, etc.), facilitates diagnostic and therapeutic instrumental exploration and will soon make it possible to carry out haemodynamic studies of the portal system through the addition of pulsed Doppler to the present echographs.

The obstetrical interest of the pelvimetry is increasing. The irradiation must be limited and the maximum informations must be provided. The authors try to demonstrate that the use of two views (Fernström and Lateral) and of diagram gives as much prognostic informations as an examination including a Throms' incidence.

A pedagogical network has been developed at University Hospital of Rennes from 1996. The challenge is to give medical information and informatics tools to all medical students in the clinical wards of the University Hospital. At first, nine wards were connected to the medical school server which is linked to the Internet. Client software electronic mail and WWW Netscape on Macintosh computers. Sever software is set up on Unix SUN providing a local homepage with selected pedagogical resources. These documents are stored in a DBMS database ORACLE and queries can be provided by specialty, authors or disease. The students can access a set of interactive teaching programs or electronic textbooks and can explore the Internet through the library information system and search engines. The teachers can send URL and indexation of pedagogical documents and can produce clinical cases: the database updating will be done by the users. This experience of using Web tools generated enthusiasm when we first introduced it to students. The evaluation shows that if the students can use this training early on, they will adapt the resources of the Internet to their own needs.
The popliteal artery entrapment syndrome (PAES) is a frequent disease of especially young patients, often following a muscular exertion. To avoid the complications, the diagnosis must rapidly be done. According to this report of a 12 year old child, we think that magnetic resonance imaging (MRI) is very accurate to diagnose the abnormalities of the popliteal artery course, the muscular hypertrophies and can relieve of more invasive procedures such as the arteriography associated with known morbidity in young patients.
The authors report 6 cases of portal hypertension with gastrorenal shunt. This shunt did not arise from the left gastric vein, but from the splenic vein. Portal hypertension was related to alcoholic cirrhosis in 3 cases, to extensive portal thrombosis in 2 cases, and to nodular regenerative hyperplasia of the liver in one case. A gastrointestinal hemorrhage revealed portal hypertension and the liver disease in the 3 cases of alcoholic cirrhosis and complicated the course of the disease in the other cases. Hemorrhage was either massive and life-threatening or often recurred. It was related to a rupture of fundic varices in all cases. The fundic varices were not associated with esophageal varices in the 3 cases of cirrhosis. The degree of portal hypertension was above 20 mm Hg, as assessed by the portohepatic gradient (one case), or the pressure gradient between a tributary portal system vein and the inferior vena cava during laparotomy (5 cases). Definitive control of hemorrhage could not be achieved by endoscopic variceal sclerotherapy (2 cases) or percutaneous transhepatic embolization (one case). Portacaval shunt or splenectomy was performed in 5 cases. These findings suggest that spontaneous splenogastrorenal shunt is a clinical and hemodynamic entity which requires specific treatment when associated with gastric variceal bleeding.

From the study of 80 patients, the authors show the interest of abdominal thermography to explore a portal hypertension. Linear stripes, stellar pictures, periumbilical hot spot are specific pictures. These thermographic modifications come before cutaneous venous signs in half of the population. Their interest for prognosis remains to be specified.
After four years' experience of the use of a computer-based system for establishing appointments for radiologic examinations, a more elaborate organization is envisaged and its broad outlines described. The developed ideas should assist preparation of one part of the specifications of a computer-based system for a department of medical imaging.
Errors encountered during evaluation of hepatobiliary ultrasonography explorations due to various artefacts are described. These are mainly due to the problem of the lateral resolution of ultrasonographic recordings leading to false negatives and positives during diagnosis of the pathological process, lithiasis and cholecystitis. These are also the cause of errors during ultrasound guiding of needle punctures and thus the precision of the explorations. The major problem of echogenetic resolution is raised as this is also a cause of numerous artefacts, though it is much more difficult to evaluate their importance. Suggestions are made concerning the use of equipment for conducting explorations with the minimum of error, and the techniques to be employed.
