Phenotypic heterogeneity associated with defective apolipoprotein B-100 and occurrence of the familial hypercholesterolemia phenotype in the absence of an LDL-receptor defect within a Canadian kindred.

Of 163 individuals with a diagnosis of heterozygous familial hypercholesterolemia (FH), only one subject was found to be positive for familial defective apo B-100 (FDB). The eight-member kindred ascertained through this subject who presented with both a clinical phenotype of FH and the FDB apo B-100 (Arg3500----Gln) mutation was studied. Plasma lipid and lipoprotein profiles, apo E phenotypes, apo B gene markers at the 3' hypervariable region and LDL-receptor haplotypes (ApaLI, PvuII, NcoI), were determined, together with LDL-receptor activity on freshly isolated blood lymphocytes. The FDB mutation, present in four relatives, was associated with three different phenotypes: FH and severe hypercholesterolemia, moderate hypercholesterolemia and normolipidemia. The FH phenotype occurred in the absence of any functional LDL-receptor defect. In homozygotes for the absence of the PvuII cutting site who had the apo B mutation, LDL-cholesterol levels were low in the presence of the apo E3/2 phenotype and high in the presence of the apo E4/4 phenotype. None of the major known environmental influences accounted for the wide range of variation in LDL-cholesterol among the affected members. Further observations in the spouse and offspring of the normolipidemic FDB subject confirmed the association of apo E4, the FDB mutation and the PvuII(-/-) genotype with high cholesterol levels. It is concluded that the phenotypic expression of the FDB mutation may vary widely as a function of the genetic environment within a family.(ABSTRACT TRUNCATED AT 250 WORDS)

Probucol protects lipoprotein (a) against oxidative modification.

Probucol, which decreases cholesterol levels and has antioxidant properties, was administered orally to patients with familial combined hyperlipidemia and high plasma lipoprotein(a) [Lp(a)] levels. The drug had no effect on Lp(a) concentrations after 4 weeks, but was found to be distributed in both Lp(a) and low-density lipoprotein (LDL). Before treatment, in each case LDL and Lp(a) isolated from the same individual were readily oxidized by copper, resulting in increased electrophoretic mobility and enhanced uptake and degradation by macrophages of both lipoproteins. After probucol treatment, both lipoproteins acquired resistance to in vitro oxidation by copper. Furthermore, probucol prevented their enhanced uptake and degradation by the macrophages. It is surmised that oxidized Lp(a) may carry an atherogenic potential that could be opposed by probucol administration.

HMG-CoA reductase inhibitors: a look back and a look ahead.

OBJECTIVES: This review is a follow-up to the Vancouver Symposium on hydroxymethyl glutaryl coenzyme A (HMG-CoA) reductase inhibitors held in May 1990 (Can J Cardiol 1992;8 [Supplement A]: March 1992). The aim is to provide an update on the issues discussed at the time, focusing primarily on aspects of practical importance for the clinician and new developments. DATA SOURCES: The available literature on HMG-CoA reductase inhibitors from May 1990 to May 1991 was systematically reviewed. DATA SELECTION: The review focuses especially on human studies which provide a better understanding of mechanisms of action, pharmacokinetics, efficacy, safety and tolerance, as well as structure-function relationships. CONCLUSIONS: A look back indicates that HMG-CoA reductase inhibitors, as a class, appear to be safe and effective for long term use and constitute a major drug addition for the control of hypercholesterolemia. Combination therapy may enhance effectiveness but close monitoring is needed when used in conjunction with fibrates and nicotinic acid because of potential muscle toxicity. A look ahead uncovers new properties that may be of potential benefit in the treatment of atherosclerosis, hypertension, cholelithiasis, mild renal disease and possibly thrombogenesis and cancer. In view of the fact that the magnitude of low density lipoprotein cholesterol lowering is commensurate with the degree of atherosclerosis regression, HMG-CoA reductase inhibitors provide a unique tool for the control of aggressive atherosclerotic vascular disease and for secondary prevention of coronary artery disease.

Effect of the increase of steroid binding plasma levels after passive immunization against testosterone on the control of luteinizing hormone (LH) secretion in ovariectomized underfed dairy heifers.

The ability of passive immunization against testosterone to increase sex steroid binding levels in plasma and thus to overcome the negative feedback of oestradiol-17 beta (E2) on LH secretion in underfed heifers was investigated. Dairy heifers were ovariectomized and divided in 3 groups: high energy diet (H group, n = 4), low energy diet (L group, n = 3) and low energy diet + E2 implants (LE2 group, n = 4). Twenty-four h before injection of bovine immunoglobulins, the mean concentrations of LH were not different between H and L groups. LH baseline was lower (0.8 vs 1.1 ng/ml, P less than 0.03) and the median number of LH pulses was higher (10 vs 5, P less than 0.03) in H than in L group. E2 markedly decreased (P less than 0.01) the mean and basal concentrations of LH (0.27 ng/ml), and number of LH pulses (0) in the LE2 group (P less than 0.05). After injection of anti-testosterone immunoglobulins in the L group, mean and basal LH concentrations tended to decrease. The median number of LH pulses in the L group rose 8 days after immunization (5 vs 7 on day -1 and day +8, P less than or equal to 0.05). Amplitude of pulses tended to decrease after injection (P = 0.08). In the LE2 group, the mean concentration and baseline of LH were not affected by passive immunization against testosterone, while pulses of LH appeared at day +1 and rose (P = 0.07) at day +8 after immunization with 3.5 pulses.(ABSTRACT TRUNCATED AT 250 WORDS)

Cancer patterns in the Inuit population of Canada 1970-1984.

1. Rates for lung cancer in Canadian Inuit are high and increasing for both men and women; in fact they are the highest reported rates for lung cancer among any Inuit population. 2. Cervical cancer in Canadian Inuit are high but rates appear to be stable, unlike the rapidly increasing trend reported in both Alaska and Greenland. 3. Rates for traditional Inuit cancers such as those of the nasopharynx and salivary gland do not appear to be declining in Canadian Inuit. 4. Rates for colorectal cancer in Canadian Inuit are similar to those expected for the Canadian population as a whole and do not appear to be increasing. 5. Finally, cancers traditionally reported to be rare in Inuit are still rare in the Canadian Inuit population; these include breast, prostate, bladder, and endometrial cancer. Cancer is a disease which can be controlled through prevention, early detection or treatment. The following future directions arising from this work reflect this paradigm. First, the Inuit cancer registry developed for this analysis will be used as a base for further research into environmental and genetic factors influencing cancer rates in the Inuit. Second, given that at least 40% of cancers in Canadian Inuit are cancers of the lung and cervix, health promotion programs should be developed to encourage tobacco-free environments to halt the epidemic of lung cancer, and also to make available Pap smear programs for early detection of cervical cancer. Third, health care services and programs, including diagnosis and treatment, should be culturally accessible to the Inuit population (6).(ABSTRACT TRUNCATED AT 250 WORDS)

[Therapy of retinal detachment with minimal intervention (author's transl)].

Two personal observations of a cure of retinal detachment with minimal intervention are presented: photocoagulation of a retinal tear and prolonged forced immobilization. As shown by E. Custodis, the closure of the retinal tear remains essential whereas the drainage of subretinal fluid is accessory.

[Significance of spontaneous reduction of long term auricular fibrillation. Apropos of a case followed by subtotal auricular paralysis].

The authors report the case of a female patient with mitro-aortic valve disease, whose atrial fibrillation had been recorded over a period of 13 years. There was then a conversion to sinus rhythm, followed by the appearance of a junctional rhythm with retrograde atrial conduction. Intracavitory investigation revealed that the electrical activity of the right atrium could be recorded only from a limited area, and that atrial stimulation was effective only over that area. This subtotal atrial paralysis suggests that the spontaneous conversion from atrial fibrillation was a sign of deterioration in the condition rather than of a "paradoxical improvement".

[Genetics and idiopathic retinal detachment].

Study of all patients treated for retinal deteachment at the University Eye Clinic Lausanne from 1960-1971. All secondary detachments as well as 128 purely traumatic, 120 aphakic detachments and juvenile retinoschisis are not included in these statistics. Therefore there remain 802 patients with idiopathic detachments. A family occurrence is recorded in 7,6% of these cases. Bilateral detachment is observed in 25%. If one counts the controlateral degenerescence of the retina, bilaterality ascends to 42%. The disease occurs at an earlier age in cases with a family background. All these factors allow to appreciate the importance of heredity together with even more important peristatic factors. In 24% of cases, high myopia is associated with detachment. In this instance the heredity of detachment identifies itself with the heridity of myopia. This would be due to an autosomal monofactorial gene with variable penetration. Nevertheless in most cases, the detachment occurs independently of myopia. For the few hereditary cases (3,7%) the detachment or its predisposition would be due to a different gene, but also to a monofactorial autosomal dominant gene with variable and lower penetration, reaching even recessivity, acting together with peristatic often preponderant factors. Several pedigrees of high myopia as well as isolated retinal detachment are presented. Statistics beginning in 1960 do not allow any conclusion on Favre' or Wagner's diseases.

[Factors of successful breast feeding in New Brunswick: information and compatible working conditions].

Although breastmilk is the best food source for the first six months of life, breastfeeding rates at three and six months of age in New Brunswick are quite low. To determine action priorities to increase the duration of breastfeeding, we tried to identify its most important determining factors. Data were obtained from the 431 breastfeeding mothers in a representative sample of 777 infants born after a normal pregnancy. The mother's knowledge about breastfeeding is the most valuable factor that explains breastfeeding duration, followed by the compatibility of the mother's employment with breastfeeding. Interventions directed at improving either of these two factors should be encouraged particularly among mothers of lower socioeconomic status who breastfeed for shorter periods of time.

[Headaches and cervicarthrosis : arthrosis, vascular or psychic origin? (author's transl)].

Cervicarthrosis is an extremely frequent disease yet often appears as asymptomatic. It can however give birth to varied clinical manifestations, for which it can be held responsible together with other factors : inflammatory as in cervico-brachial neuralgia, vascular as in cervicarthrosis medullopathy, psychological as in posterior cervicalgia and headaches belonging to the classical Barré-Liéou syndrom. We must stress the therapeutic interest of tiapride, which acts on the algic element as well as on the element of psychological tension in the last case.
