Does the purity of intraportal islet isografts affect their endocrine function?

The effect of collagenase digested pancreatic tissue upon the endocrine function of intraportal islet isografts has been studied in rats. The ability of transplanted islets to restore normoglycemia in recipients was not affected by tissue contamination, but the insulin response to glucose challenge 12 weeks after transplantation was negatively correlated with the amylase content of this additional tissue transplant. All animals receiving a single dispersed pancreas died within 24 hr. These findings are discussed in relation to transplantation of the dispersed pancreas in human.

Effect of intrathecal morphine on the adrenocortical and hyperglycaemic responses to upper abdominal surgery.

Changes in plasma cortisol concentration and serum glucose concentration were measured in a group of 10 patients given intrathecal morphine 0.8 mg before cholecystectomy and the results compared with those from a control group of 10 patients receiving papaveretum i.v. during the operative procedure. Intrathecal morphine had no effect upon the hyperglycaemic response to surgery, but attenuated the increase in serum cortisol concentration.

A misdiagnosis of carotid pseudoaneurysm.

We report a case of blunt injury of the neck associated with the use of an imported plastic rock-guard designed for use in motorcycle scrambling. The injury may have important implications for the future use of this particular guard.

Problems in diagnosis of popliteal aneurysms.

Clinical features and errors in diagnosis have been assessed in a retrospective study of 62 popliteal aneurysms in 40 patients, 22 of whom had bilateral aneurysms. Only 29% of patients complained of pain or swelling behind the knee, while 31% of aneurysms had produced distal ischaemia presenting as intermittent claudication, 9 aneurysms had thrombosed producing ischaemic rest pain in 6 legs, and 4 aneurysms had ruptured. Although 94% of aneurysms were suspected or confidently diagnosed by palpation alone, only 43% of patients had had the correct diagnosis made at the time of initial referral. Treatment was delayed in 12 patients, 8 of whom subsequently required amputation. Popliteal aneurysm should be suspected in patients with a prominent popliteal pulse who present with intermittent claudication, and in patients with acute ischaemia of the leg who may have a thrombosed aneurysm requiring surgical exploration.

When do patients given intrathecal morphine need postoperative systemic opiates?

A prospective, randomised study has compared the requirements for intramuscular papaveretum after cholecystectomy in patients given either 0.8 mg intrathecal morphine preoperatively or intravenous papaveretum peroperatively. Patients given intrathecal morphine required significantly less papaveretum during the first 48 hours after operation, but no significant difference in analgesic requirements was observed by 72 hours due to a continuing demand for papaveretum by these patients.

Venous access using the Hickman catheter.

Fifty silastic (Hickman) right atrial catheters were implanted in 43 patients receiving cytotoxic chemotherapy or parenteral nutrition. The mean catheter survival was 72 days (range 2-316 days), but 17 catheters were removed before treatment was complete. Thirteen catheters were removed for septic complications. It is concluded that the Hickman catheter will provide venous access for prolonged periods, but its benefits must be weighed against the risks of septic complications which may occur frequently in the absence of uniform care and maintenance of the catheter.

Demonstration of carcinoembryonic antigen in bone marrow from patients with carcinoma.

Primary and secondary tumour and bone marrow trephine biopsies from 20 patients with carcinomas were stained for carcinoembryonic antigen by the three stage immunoperoxidase method. Six marrow biopsies contained tumour deposits, five of which were positive for carcinoembryonic antigen. A further five marrow biopsies contained single carcinoembryonic antigen positive cells of uncertain origin. Carcinoembryonic antigen staining may be a useful adjunct to conventional histology in the diagnosis of marrow metastases.
