Gianturco self-expanding stents: clinical experience in the vena cava and large veins.

Twenty-five patients with stenosis of the vena cava (21) and other large veins (4) have been treated with self-expanding Gianturco metallic stents. Eighteen patients had superior vena cava syndrome. In 17, the stricture was due to malignant superior vena cava compression recurrent after maximum tolerance radiotherapy and/or chemotherapy. In 16 of these patients there was early symptomatic relief. In 1 patient with a benign posttraumatic superior vena cava stricture, the stenosis was not relieved, and occlusion occurred after 1 month. Stenoses associated with dialysis shunts were relieved in 2 other patients. Two malignant and one benign inferior vena cava stenoses were relieved either until death, or in the benign case, for 30 months. One malignant subclavian vein obstruction occluded after 24 h due to stent misplacement and another with extrinsic mediastinal compression remained patent until death, extensive thrombus having been lysed prior to stent placement. The results of this short series suggest that the Gianturco self-expanding stent in the vena cava and large veins is easy and safe to place, and in most cases produces almost immediate palliation of the distressing effects of venous obstruction, often in a preterminal and inoperable patient.

Role of the Gianturco expandable metal stent in the management of tracheobronchial obstruction.

This paper reports our experience in treating localized airway obstruction with expandable metal stents. Nine patients were treated for malignant obstruction and 6 for benign obstruction. The main indications for treatment were imminent asphyxia, breathlessness and/or repeated chest infections. All but 2 patients had received other treatments before referral. Seven patients with malignant obstruction had extrinsically compressed airways and all derived a sustained symptomatic improvement. Two patients with recurrent tracheal obstruction caused by intraluminal tumor gained lasting relief with the use of a covered expandable metal stent. All 6 patients with benign strictures were improved, although 2 developed recurrent obstruction caused by granulation tissue growing within and beyond the stent. It is concluded that the expandable metal stent provides an effective and noninvasive method of relieving large airway obstruction. As the long-term tissue tolerance to this type of stent is not known, caution is advised in the management of benign strictures. In patients with malignant obstruction, however, the expandable metal stent would appear to have considerable potential as a palliative treatment.

A comparison of standing steadiness measurements from two devices: covariates and normal values.

Quantifying standing steadiness may be a useful method of detecting neurotoxicity in epidemiological studies. Unfortunately, use of quantitative standing steadiness outcomes in epidemiologic studies has been limited by lack of standardization of methods, insufficient availability of normative data, and inadequately characterized effects of covariates. Additionally, the current gold standard method, the force platform (FP), has been expensive and unwieldy for use in field studies. A relatively inexpensive and portable head position monitor (HPM) has been introduced as an alternate method for measuring standing steadiness. In this study 211 subjects were tested with one or both devices using a common testing protocol. The correlations between measurements obtained with the FP and the HPM were high and similar to those obtained during repeated measurements with each device separately. The effects of potential covariates on outcome measures were investigated. There was significant age x sex interaction in the FP standing steadiness measurements in this population with decreased steadiness among older men but not older women. Information for estimating normal values for the outcome measures is provided. This study suggests that measures of standing steadiness obtained with the HPM are similar, but not identical, to measures obtained with a conventional FP and that the HPM may be useful in field studies of occupational exposure to neurotoxicants.

Ochratoxin A in table wine and grape-juice: occurrence and risk assessment.

The concentrations of ochratoxin A (OA) and C (ethyl ester of OA, OC) have been measured in table wine from the Swiss retail market. The overall median concentrations of OA in white, rose and red table wine were (number of samples; range) < 3 pg/ml (24; < 3-178 pg/ml), 19 pg/ml (15; < 3-123 pg/ml) and 13 pg/ml (79; < 3-388 pg/ml)), respectively. For dessert wines (Malaga, Marsala) a median of 337 pg/ml (5; 44-451 pg/ml) resulted. Concentrations of OC were about 10% of those of OA. OA has been detected in red grape-juices for the first time with median concentrations amounting to 235 pg/ml (8; < 3-311 pg/ml). White grape-juices (3) as well as other fruit juices (2 apple, 6 orange, 2 others) did not contain OA (< 5 pg/ml). The analytical method used was not appropriate for grapefruit-juices because of an interfering substance. The results indicate that OA is formed prior to the alcoholic fermentation. It also appears that the more southerly in Europe is the origin of the samples the higher is the frequency of occurrence and the concentration of OA in red wines and red grape-juices. Although the estimated mean daily OA intake of 0-7 ng/kg body mass (men) can easily be more than doubled by moderate wine consumption, an additional health risk by OA is not to be expected. On the other hand the consumption of red grape-juices among infants and children may result in intakes high enough to justify a reduction of the OA concentrations in these juices to less than 100 pg/ml.

Use of 64copper measurements to diagnose canine copper toxicosis.

Inherited canine copper toxicosis is a serious problem in Bedlington terriers and West Highland White terriers, and may also be a problem in other less-studied breeds. Affected dogs become ill at midlife with progressive and ultimately fatal liver disease. Treatments for removal of copper and prevention of copper accumulation are available, but are most effective if begun before the dog becomes ill. Until recently diagnosis has not been available until the dog is 1 year of age, and then only by an invasive liver biopsy with determination of liver copper concentration. The authors studied the use of 64copper for early diagnosis of canine copper toxicosis. Two procedures were evaluated. The first involved measuring the concentration of 64copper in blood 24 hours after oral administration of the radioisotope. At this time, 64copper was associated primarily with ceruloplasmin secreted into the blood by the liver. This procedure is useful in the diagnosis of the human counterpart, Wilson's disease. However, the authors found it to be nondiscriminatory between affected and unaffected dogs. In contrast, the second procedure, which involved measuring 64copper excreted in stool during 48 hours after an intravenous dose of radioisotope, yielded results that differentiated most affected and unaffected dogs.

Knee injury patterns among men and women in collegiate basketball and soccer. NCAA data and review of literature.

Women's participation in intercollegiate athletics has increased dramatically in recent years. Greater participation has increased awareness of health and medical issues specific to the female athlete. Some reports have noted a higher susceptibility to knee injury, specifically injuries to the anterior cruciate ligament, in female athletes as compared with their male counterparts. We performed a 5-year evaluation of anterior cruciate ligament injuries in collegiate men's and women's soccer and basketball programs using the National College Athletic Association Injury Surveillance System. Results showed significantly higher anterior cruciate ligament injury rates in both female sports compared with the male sports. Noncontact mechanisms were the primary cause of anterior cruciate ligament injury in both female sports. Possible causative factors for this increase in anterior cruciate ligament injuries among women may be extrinsic (body movement, muscular strength, shoe-surface interface, and skill level) or intrinsic (joint laxity, limb alignment, notch dimensions, and ligament size).

[Sequela of hand burns in children: 32 cases followed and treated in the Plastic Surgery and Reconstruction department of the University Hospital Center of Treichville, Abidjan].

An investigation on the effects of hand burns has been undertaken about 32 children admitted in the plastic surgery outpatients Department. Accident occurs among the lower class population and heat is the major factor pointed out. The most frequent effects are hand palmar contracture. The surgical treatment completed with the functional re-education has given very good results in 79% cases. However the cheloid contractures are the most difficult to cure. Indeed the authors insist or the prevention and the creation of a center of burned patients and a plastic surgery Service to take care of patients whose life prognostic is optimistic.

[Microsurgery in revascularizations and replantations of the upper extremity in emergency cases. Department of Plastic and Reconstructive Surgery. University Hospital Center of Treichville-Abidjan].

The authors report their experience in vascular and nervous microsurgery. They remark that there is a long time between the emergency reception of the wounded person and the appeal to plastic surgery. They deplore also the bad collaboration between the different department of emergency surgery. Finally they emphasize that the scarcity of cases is due to insufficient information of different departments of emergency surgery in microsurgery era.

[Intra-keloid excision and deferred corticoid infiltration in the treatment of keloids. University Hospital Center of Treichville-Abidjan].

On a series of 38 keloid surgery patients the authors noted a strong female predominance and the frequency of ear lobule keloid. After some rather unsuccessful excision-suture of the keloid they recommend the intra- keloid excision to be followed by a late corticoid infiltration but deferred until the 15th day. The results obtained were encouraging but some researches have to be made with regard to the etiology of this affection which might probably resolve the therapeutic problem.

[Semilunar dislocations. A report of 8 cases. (Orthopedic service-University Hospital Center of Treichville-Abidjan)].

The authors have underlined in a study made on 8 observations that the lunate dislocation is scar. All their patients were men under 40 years old. Industrial injury was the most dominant cause. Using WITVOET and ALLIEU's classification, they noticed a predominance of perilunar and posterior dislocations (7/8 cases). Perilunar anterior dislocations and isolated scaphoid carpal dislocation have not been encountered. The treatment which has been orthopaedic or surgical has given moderate results: Good results = 4; Average result = 1; Bad results = 2; Non seen to again = 1 The authors think that a precocious diagnosis and an orthopaedic treatment in the forms detected early and surgical treatment in ancient forms are the only guarantee of a good functional result avoiding dread diseases such as carpal instability, radiocarpal arthrosis.

[Surgical treatment of hepatosplenic bilharziosis in the Ivory Coast. Apropos of 34 cases].

Portal hypertension due to hepato-splenic bilharziasis is common in Abidjan, where we operated on 34 cases in 15 years. The indications for surgical intervention include gastrointestinal haemorrhage due to rupture of esophageal varices, a very bulky enlarged spleen and, more, rarely, recurrent ascites. Simple splenectomy (9 cases) suppresses discomfort and pain, corrects the hypersplenism and brings about a moderate reduction of portal pressure. Spleno-renal anastomosis (19 cases) remains the operation of choice. The long-term results, difficult as these are to assess under ill-regulated African conditions, appear satisfactory on the whole, due to less serious damage to the hepatocyte than in the cirrhoses (62% Child A and 29% Child B). We therefore continue to advocate splenectomy followed if possible by a spleno-renal shunt in bilharzial portal hypertension.
