A cause de l'effet favorable de la grossesse sur l'activité de l'arthrite rhumatoïde, on s'est demandé si une pseudo-grossesse, produite par un progestatif de synthèse n'entraînerait pas une rémission du moins partielle de cette maladie.Noréthynodrel associée à mestranol (Enovid), 30 mg./jour, a été administrée à 44 femmes pendant quatre à 24 mois. A cause d'effets secondaires indésirables, 11 patientes furent soustraites de l'investigation. Les résultats s'appuient sur 33 cas. Une rémission apparente complète s'est manifestée chez sept patientes aux stades précoces de la maladie; chez 15, une amélioration objective des signes inflammatoires a été observée; chez quatre, une amélioration subjective seule a été notée; chez sept, il n'y a eu aucune amélioration. Dix-sept femmes sur 36 ont présenté une altération d'un ou plusieurs tests hépatiques. Trois présentèrent un ictère cholestatique. Les 17-OH plasmatiques se sont élevés à trois ou quatre fois la normale.De cette étude il ressort que noréthynodrel associée à mestranol peut produire une atténuation des signes inflammatoires de l'arthrite rhumatoïde. L'effet est palliatif, mitigé et non curatif et ne résulte pas nécessairement de l'état de pseudo-grossesse en soi.
One hundred and twenty-seven synovectomies were performed for rheumatoid arthritis of the knee at the Hôtel-Dieu Hospital, Montreal, over the past 15 years. The authors have reviewed 56 synovectomies in 49 patients to determine whether the operation might have any prophylactic value.They conclude that synovectomy of the knee can delay or prevent gross joint destruction if the operation is performed during the synovial phase of the disease. If it is performed too late, the operation can at best only salvage an already damaged knee.The surgical technique, the operative indications and the postoperative regimen are described.

Two networks of medical care have grown up in Japan: 1 network based in hospitals and 1 based in private clinics. Most physicians in clinics practice general medicine, although only a small proportion of them are trained in primary care. Postgraduate medical education is oriented toward the training of specialists. Owing to the impending retirement of the cohort of clinic-based physicians who have dominated medical politics in Japan, the system is expected to change substantially in the next decade. Among the reformers is a small group of physicians who are attempting to introduce family practice.
Total health care utilization in the Seattle-King County jail over a 2-month period in spring 1979 was studied. The annualized visit rate for men was 48 per prisoner per year, 2.6 times the rate for men studied in a long-term prison and almost 20 times the rate for men studied in the general population. The annualized rate for female prisoners was three times that of male prisoners. For all prisoners, the most common problems seen were skin conditions (9.7 percent), musculoskeletal (8.3 percent), and psychiatric (8.2 percent). An examination of practitioners' patterns in providing care demonstrated the primary role of registered nurses, who saw 70 percent of the patients. During the study period, 125 transfers were made to a hospital; 48.8 percent were for medical problems, 41.6 percent for trauma, and 9.6 percent for psychosocial problems.
