Congenital stenosis of lumbar spinal canal: comparison of results of surgical treatment for this and other causes of lumbar syndrome.

The operative results in 37 consecutive patients suffering from developmental stenosis of the lumbar spinal canal, compared with those in spondylosis and disc herniations, are discussed. The diagnostic certainty of stenosis, suspected on the bases of clinical and radiographic data, is reached only at the operating table. To judge the usefulness of the operation we have considered not only the patients' verdicts, but also the improvements in neurological signs and the appearance of new deficits. Satisfactory results are around 80%, slightly less good with stenosis than with the other causes of lumbar syndrome. Radiographic study with contrast medium is mandatory. Dimer-X has been used with very clear radiographic findings and very few complications. Operating technique is also described: a wide laminectomy with facetectomy is advised. The great importance of early physiotherapy is emphasized.

Contribution to the surgical treatment of acute supratentorial subdural haematomas.

Acute traumatic subdural haematoma suspected of extending over an entire hemisphere must be treated with all speed. To obviate unnecessary skin incisions and subsequent craniectomies and craniotomies during the operation, we think it advantageous to plan a series of burrholes at set points in order to; a) begin to clear the haematoma; b) assess its extent and possible origin; c) decide whether a hemicranial or smaller craniotomy is called for.

An intradiploic meningioma of the orbital roof: case report.

The case of a young man with a 4-year history of increasingly severe exophthalmos and no other symptoms is reported. The radiological investigations showed a space-occupying lesion at the level of the roof and medial wall of the right orbit with involvement of the lesser wing of the sphenoid. A tumor within the diploë of the orbital roof without intracranial or intraocular involvement was removed and, on histological examination, proved to be a meningioma. Possible starting-points are discussed in the light of the literature consulted, but in view of the clinical, X-ray and operating data, it is concluded that the tumor is an intraosseous ectopic meningioma.

Symptomatic cervicogenic headache.

Cervicogenic headache is a little-known clinical condition whose true importance has only recently been recognized. A number of causes may lie at the basis of the onset of headache (symptomatic cervicogenic headache). However, despite exhaustive attempts, sometimes it is not possible to identify a clear cause responsible for the onset of the syndrome (primitive cervicogenic headache). The genesis of symptomatic cervicogenic headaches sometimes may be easy to identify as a result of a close, pre-existing, cause-effect relationship (i.e. trauma). On other occasions it may be much more laborious to pinpoint the pathology responsible for headache (some cranio-cervical anomalies, etc.). Clinically, it is necessary to perform a thorough preliminary clinical and anamnestic evaluation which can orient subsequent investigations to achieve a diagnosis in the least time possible with the minimum discomfort to the patient and his relatives, not to mention lower costs for society.

[The osseous location of echinococcal cysts: comments on a clinical case].

Primary bone location of hydatid disease is extremely rare. The clinical case herein reported represents a primitive bone location of echinococcus in the ileal and sacral area. The aspecific symptoms did not allow for a preoperative diagnosis. The latter was possible only after a CT scan performed for other reasons. The patient underwent a CT scan and MIR which showed a diffuse involvement of the ilium and sacrum along with the presence of two cysts (anterior and posterior). Surgery was performed through an anterolateral extraperitoneal and posterior access in the gluteal region. Asportation "en bloc" of the cysts was not possible. Therefore, they were opened, aspirated and washed with hypertonic solution (33% NaCl). Following surgery Albendazole was prescribed to the patient. At 22 days from surgery, a CT scan confirmed the disappearance of the cysts along with the presence of bone lesions.

Interhemispheric subdural hematoma (ISH). Case report.

Interhemispheric subdural hematoma (ISH) is a rare form of post-traumatic intracranial hemorrhage. The Authors report a further case and review the relevant literature. ISH most frequently gives a clinical picture corresponding to the falx syndrome. The possibility of an interhemispheric hematoma evolving into a chronic subdural hematoma of the convexity is considered together with both surgical and medical therapeutic indications.

Molecular organization of oncogenes and potentially oncogenic regulatory genes in primary human brain tumors.

We review here results obtained from a wide screening of 72 primary human brain tumors, in order to investigate the molecular organization of proto-oncogenes and potentially oncogenic genes. We demonstrated alterations in the restriction pattern of c-sis oncogene in 1 anaplastic astrocytoma and in 1 endotheliomatous meningioma, and amplifications of c-myc oncogene in 2 endotheliomatous meningiomas. In the same screening of primary brain tumors, we have recently published evidences for a specific alteration in the restriction pattern of estrogen receptor gene sequences which code for the DNA binding domain of the protein. These results, although preliminary and not still sufficient for a conclusive determination of the role in brain tumor growth and/or development, provided evidences of relatively frequent molecular alterations of genes involved in the control of cell proliferation and differentiation.

Association of intracranial meningiomas and aneurysms. Report of three cases and review of the literature.

The Authors report three personal cases of association between meningioma and arterial aneurysm, both intracranial. Twenty-three similar cases are referred in the literature, bringing the total to 26. Of these 26, the overall average age at diagnosis was 48.3 years: 17 were females and 9 males. The predominant site for the meningioma was the cerebral convexity and for the aneurysm the anterior communicating artery. Meningiomas were multiple in 5 cases and the aneurysms were multiple in the same number of cases. In 21 cases the meningioma was removed: in 11 of these the aneurysm was treated simultaneously. In the majority of cases the aneurysm was an accidental finding during neuroradiological investigation to document the meningioma; panangiography is reported to have been performed only in a few cases. Greater use of the latter would probably have revealed a higher incidence of this rare association. The greater frequency found of the association of aneurysm and multiple meningiomas makes it advisable to perform panangiography in all such cases. The best long-term results were obtained in the cases where the meningioma and aneurysm were operated simultaneously.

Chronic damage after spinal trauma in rat: neurophysiological and ultrastructural investigations.

A group of 10 rats underwent spinal trauma by epidural spinal compression according to Tator's procedure. After 4 months from injury cortical and spinal SEP were recorded, and the spinal injured tract was examined by electronic microscope. We studied morphologic and functional changes showed after some months behind the trauma. This report demonstrates and compares the varying sensitivity of cortical and spinal SEP for revealing functional spinal damage; ultrastructural investigations testify to the existence of reparative processes of the nervous tissue.

Small aneurysm clips for surgery of neck vessels.

Surgery of the internal carotid artery and the extracranial vertebral artery is of a fundamentally preventive nature, and justifiable when intra and postoperative complications (intraoperative stroke, postoperative vessel occlusion, postoperative TIA and/or stroke) remain within very low limits (less than 5% according to major case-series'). To minimize damage to the arterial wall that could cause vessel occlusion and even neurological damage, the authors propose the use of intracranial aneurysm clips for temporary closure of the neck vessels, explaining the methods used for selection of these clips.

Considerations on a case of intranasal cerebral ectopia.

The Authors report a case of intranasal cerebral ectopia; the problems concerning the nosographic and diagnostic definition of this condition as well as the differences from other similar pathologies with the same location (such as meningoencephalocele and nasal glioma) are discussed. Particular attention is payed to the various surgical techniques that can be used in these cases.

Na+/K+ ATPase and cell growth IV: a metabolic marker of human brain tumors?

The activity of total, Mg++ dependent and Na+/K+ ATPase as well as the content of cAMP and cGMP in homogenates of human brain tumors have been investigated. Results are compared to values obtained from normal cortices. Na+/K+ ATPase and cAMP are decreased with a close relationship with the degree of malignancy, while Mg++ dependent activity is lower than in normal cortex but with no differences between the various tumors, and cGMP is unaffected. We conclude with a discussion on the metabolism of brain tumors and the suggestion of Na+/K+ ATPase activity as a marker of the malignancy of neoplastic growth.

Epidural hematomas.

A consecutive unselected series of 66 patients operated on for epidural hematoma between 1974 and 1981 is presented. Several parameters (age, sex, etc.) are analyzed and compared with those of the literature. The outcome is referred and discussed. The mortality rate was 18%. These were the 5 factors that most influence the outcome: (1) patient's age; (2) rate of development of the hematoma; (3) associated brain lesions; (4) neurological status at the time of surgery; (5) interval between injury and surgery. The introduction of Computed Tomography (CT) in neurotraumatological practice, has been followed by improvement of the results also in epidural hematomas.

Traumatic dislocation of the cervical spine without neurological deficit. Eleven cases.

The authors present 11 cases of traumatized patients without neurological deficit in spite of a gross dislocation of the cervical spine. In the effort of explaining this discrepancy various parameters have been taken into account. The vertebral canal has been noted to be abnormally wide in these cases (average 19.8 mm). The authors suggest that the great width of the vertebral canal has in some way prevented the spinal cord of these patients being damaged by the dislocation.

Delayed neurological bladder after cervical trauma.

We report a case of neurological bladder (upper motor neuron type) which appeared after a few years after vertebral trauma with retrodislocation of C6. We presumed that there was a traumatic and spondylosic etiopathogenesis of the disturbance. Surgery was done following the surgical techniques used in spondylogenetic myelopathies and there was an immediate improvement of the urinary disturbance.
