Proliferative glomerulonephritis associated with reflex nephropathy.

A case of unilateral reflux nephropathy treated by initial ureteral reimplantation and subsequent nephrectomy is reported. Pathologic examination of the resected kidney showed unsuspected proliferative glomerulonephritis. The relationship of chronic atrophic pyelonephritis, as seen in reflux nephropathy, and glomerulonephritis is discussed to emphasize that reflux nephropathy does not exclude the presence of other renal disease that may complicate the long-term care of the patient.

Implicit preadmission screening criteria in an alcoholism treatment program.

The fact that alcoholism programs are biased in terms of the kind of patients admitted to treatment is well recognized. The authors examined the possible bias of preadmission screening procedures in an alcoholism treatment program and investigated some of the criteria used by staff clinicians in determining an applicant's acceptability for treatment. They found that accepted and rejected applicants had highly similar characteristics, differing only in age (accepted applicants were slightly younger than those rejected) and source of referral. The overall findings suggest that clinicians implicitly evaluate problem drinkers along dimensions related to their perceived "treatability" in a given therapeutic setting.

Validation of a subjective helplessness measure.

Investigated the potential utility and validity of a measure of subjective helplessness, the H25. Helplessness is defined as the degree to which the individual perceives him/herself to be unable to influence or control the initiation and outcome of a variety of potentially reinforcing activities. Alcoholic subjects were classified into three levels of self-reported helplessness. An initial multivariate analysis of variance indicated that the groups differed with respect to severity level across a number of dimensions of depressive symptomatology, with the High Helplessness group appearing significantly more depressed on each of the measures than Low Helplessness subjects. Subsequent analyses supported the construct validity of the H25. Those measures found to be most descriminating between groups and most predictive of the level of helplessness reflected a dimension of behavioral retardation consistent with the motivational dificits noted in the learned helplessness model. Recommendations for the future validation of individual difference measures of helplessness are discussed.

Influence of cognitive style on an incidental memory task.

The present study investigated the interaction between cognitive style, imagery, and memory. The Tactual Performance Test Location Score from the Halstead-Reitan battery was used as a measure of incidental tactual memory and mental imagery. The Group Embedded Figures Test was used to assess cognitive style. Results for 38 Caucasian males of mean age 49.9 yr. suggest that cognitive style is related to an individual's ability to perform a non-verbal, non-visual memory task. Further, cognitive style may be an important mediating variable influencing intrapersonal behaviors such as non-verbal memory and mental imagery.

Cognitive impairment and treatment outcome with alcoholics: preliminary findings.

The relationship of cognitive impairment to treatment outcome in a sample of male alcoholics (N = 30) was explored. The Brain-Age Quotient (BAQ), an index of problem solving and adaptive abilities based on a battery of neuropsychological tests, was used as the summary measure of cognitive impairment. High BAQ scores were found to be more highly related to clinical ratings of patients' level of functioning on the treatment ward, to the successful completion of inpatient treatment, and to fewer relapses, longer abstinence periods, and lower rates of alcohol consumption at a 1 year follow-up than were lower BAQ scores. The implications of these findings with regard to appropriate treatment planning are discussed.

A pathophysiologic basis for the diagnosis and treatment of the renal hypertensions.

Hypertension is a major medical problem with significant short- and long-term sequelae. Previous investigators have reported childhood hypertension to be secondary and when sexondary, renal. However, recent studies have shown primary (essential) hypertension to be more prevalent than secondary. Children with blood pressures at the 95th percentile for age deserve comprehensive evaluation and follow-up. Those with systemic evidence of side-effects will require therapy. Modern therapy should be based on pathophysiologic principles which include an understanding of volume-dependent and renin-dependent hypertension. The stepwise approach to therapy will produce the maximal therapeutic benefit.

Extrarenal pelvic hydronephrosis: report of five cases.

Five patients are reported with uretropelvic obstruction and hydronephrosis in which the renal pelvis is predominantly involved. Because of the extrarenal position of the pelvis in these patients, its dilatation is apt to cause distortion and displacement of the calyces and infundibulae simulating an intrarenal mass. Calyceal and infundibular dilatation is much less apparent and general renal outline and cortical thickness is better preserved in these patients than in those with classical hydronephrosis. Careful attention to radiographic technique including delayed films, ultrasonography and occasional arteriography should prevent any confusion with a solid mass lesion.

Home culturing program for children with recurrent bacteriuria.

One hundred forty-one children were selected from a clinic for children with recurrent bacteriuria to participate in a home culturing program. The parent was taught to employ a semiquantitative method to test the child's urine for bacteria. The degree and persistence of bacteriuria as detected by the parent strongly correlated with results obtained in the clinic, although there was a significant incidence of false-positive results as detected by the parent. Home culturing proved to be an inexpensive, reliable technique for following up children with recurrent urinary tract infections.

Renin-dependent renovascular hypertension in infant with abdominal aortic atresia.

A newborn infant was seen with severe and progressive arterial hypertension, congenital atresia of the abdominal aorta, and a renal venous renin activity ratio greater that 1:1.5 (1:3.1). Preoperative studies indicated that the renin-angiotensin axis was responsible for the production and maintenance of the severe hypertensive process. Right nephrectomy cured the arterial hypertension both above and below the area of coarctation. The possible roles of the pathogenic mechanism are discussed.

S-adenosylhomocysteine metabolism in various species.

Eleven microorganisms, four plants, and major organs from the chicken, dog, rat and rabbit were assayed for the presence of S-adenosylhomocysteine hydrolase, S-adenosylhomocysteine nucleosidase, and S-ribosylhomocysteine-cleavage enzyme. All bacteria (procaryotes) were found to possess S-adenosylhomocysteine nucleosidase and S-ribosylhomocysteine-cleavage enzyme but not S-adenosylhomocysteine hydrolase; All eucaryotes tested, including yeasts, plants, birds, and mammals, possessed S-adenosylhomocysteine hydrolase but not S-adenosylhomocysteine nucleosidase or S-ribosylhomocysteine-cleavage enzyme. Of all the organs assayed in the vertebrates, the level of S-adenosylhomocysteine hydrolase was highest in liver, pancreas, and kidney, lower spleen and testis, and very low in brain and heart; In all systems tested, equilibrium of the hydrolase reaction always favored synthesis over hydrolysis. We studied some of the kinetic properties of the hydrolase from rat liver; In the direction of synthesis, the Km value was 1.5 mM for adenosine and 4.5 mM for L-homocysteine, whereas marked substrate inhibition was observed with L-homocysteine. The condensation reaction is subject to product inhibition, and was inhibited by adenine. Results from in-vivo experiments revealed that the cells of the various organs of the dog are impermeable to the exogenously administered S-adenosylhomocysteine.

Cephalexin in the management of bacteriuria.

This report evaluates the effectiveness of cephalexin in the treatment of bacteriuria in 93 children. Cephalexin therapy eradicated sensitive organisms in 97 per cent of the cases regardless of recurrence, structural abnormality, or status of renal function. Nevertheless, recurrences with a new, resistant organism occurred significantly in certain patients, expecially those with major anatomic abnormalities, during a six-week follow-up period. The incidence of drug reactions was low.
