Thoracoscopy: a review of 121 consecutive surgical procedures.

The records of 121 patients who underwent surgical thoracoscopy between 1976 and 1987 were reviewed. The indications for thoracoscopy were pleural effusion (88%), pleural thickening (7.4%), and a mass on the chest roentgenogram (15%). All procedures were performed under general anesthesia with a rigid thoracoscope and 91 patients (75%) were diagnosed as having a malignant process. Although thoracotomy was undertaken in 20 patients (16.5%), no patient with benign disease underwent diagnostic thoracotomy. Thoracoscopy had a 98.9% sensitivity and a 93.3% specificity in this series. One patient died of a myocardial infarction after the procedure, and 11 patients (9.1%) had other, predominantly respiratory, complications. We confirm that surgical thoracoscopy is a useful procedure in the diagnosis of pleural processes.

Stab wound of the neck: potential pitfalls in management.

The authors describe the case history of a patient who was stabbed in the back of his neck with a knife and who later presented with a Brown-Séquard syndrome attributable to cervical spinal cord damage. Myelography and CT revealed a compressive extradural lesion shown at exploratory operation to be a loculus of cerebrospinal fluid (CSF). The loculus had formed as a consequence of leakage of CSF through a dural tear caused by the knife. Evacuation of this loculus resulted in some neurological improvement. The necessity of obtaining a clear history and of performing a thorough clinical examination is explained. The need to admit patients in whom stab wounds of the neck have transgressed subcutaneous fat is reiterated. Early referral to a neurosurgeon is advised for those patients with neurological deficits.

Trigeminal schwannomas: has the 'new technology' made any difference?

Computerised axial tomography (CT) and magnetic resonance imaging (MRI) should make detection of small trigeminal Schwannomas easier. Review of histories of five patients with trigeminal Schwannomas suggests that CT (and in one patient MRI) does not appear to lead to earlier detection of these tumours. Despite the availability of CT, three patients presented with large Schwannomas. A high index of clinical suspicion remains crucial to the diagnosis. Recent advances in diagnostic radiology have, however, considerably facilitated the logical planning of the operative approach. In that respect, it may be anticipated that the prognosis for these rare tumors will be improved.

Prostaglandin F2 alpha metabolite levels in normal and uterine-infected postpartum cows.

The stable metabolite of prostaglandin F2 alpha,15 keto-13,14-dihydroprostaglandin F2 alpha (PGFM), was measured from peripheral blood samples collected at specified intervals postpartum from 7 normal dairy cows and 4 cows with apparent endometritis. Plasma PFGM levels were significantly (P less than .05) elevated for the first 5 days postpartum in the cows with endometritis (ranging from 4.0 to 5.0 ng/ml) compared to the controls (approximately 1.0 ng/ml). Beyond 5 days postpartum, plasma PGFM levels were not significantly different and decreased to approximately 0.4 ng/ml by day 13 in both groups. Time to uterine involution was not different between groups (less than 30 days). Therefore, uterine infections in cows during the puerperium was associated with elevated circulating PGFM levels. These findings and the observation that PGF2 alpha is not uterotonic in the puerperal cow do not suggest a therapeutic use of PGF2 alpha in order to evacuate the uterus.

Freezing bovine embryos with a portable liquid nitrogen freezer requiring no external seeding.

One-hundred and twenty excellent morula to blastocyst stage bovine embryos were obtained nonsurgically from superovulated Holstein heifers. Completely portable, nonelectric (manual) liquid nitrogen (LN) freezers combined with simplified freezing curves utilizing self-seeding were compared to a programmable LN freezer (Planner-R204) following the conventional freezing rate for freezing embryos. Seeding was self induced in ampules at -6.8 degrees C and at -5.5 degrees C in straws in the manual freezers. Glycerol was used as the cryoprotectant at 1.5 M concentration. Post-thaw appearance, fluorscein diacetate testing (FDA), and growth after 12 and 24 h incubation were used as indicators of embryo viability. There were no significant differences between embryos frozen in the two types of freezers in terms of the viability tests used. Pregnancy rates resulting from transfer of embryos frozen in the two types of freezers will be determined in subsequent field trials. The manual LN freezers used in this study are capable of successfully freezing bovine embryos. The simplified nature of these freezers and the freezing procedures used with them greatly decreases the complexity and expense of freezing embryos.

Cognitive dysfunction and aging among male alcoholics and social drinkers.

The relationship between aging and various drinking styles was examined. Four age groups (25-34, 35-44, 45-54, and 55-65 years) and four drinking styles (nondrinkers, social drinkers, alcoholics, and abstinent alcoholics) were compared. A battery of eight neuropsychological tests was administered to 322 men; 72 nondrinkers, 100 social drinkers, 58 abstinent alcoholics, and 92 alcoholics. Cognitive dysfunction related to aging was found to be a more significant factor than decline with alcohol use. Cognitive dysfunction associated with alcohol use was significant for three Wechsler Adult Intelligence Scale subtests; Vocabulary, Digit Symbol and Block Design. Alcohol-related differences in intellectual functioning tended to diminish with increasing subject age.

Morphological changes in the skin of Rana pipiens in response to metabolic acidosis.

The skin of Rana pipiens excretes H+ and this excretion is increased by metabolic acidosis. The mitochondria-rich (MR) cells of the skin have been found to mediate this H+ transport. The purpose of this study was to determine if there is a change in the MR cells of the skin during metabolic acidosis and if the isolated split epithelia of frog skin maintains its capacity to excrete H+. Metabolic acidosis was induced by injecting 120 mM NH4Cl (0.025 ml/g body wt) into the dorsal lymph sac three times a day for 2 days. The frogs were sacrificed and collagenase-split skins from the abdomen of normal and metabolic acidotic frogs were mounted between 2-ml chambers. H+ fluxes into both the mucosal and serosal media were measured and reported in units of (nmol) (cm2)-1 (min)-1. An increase in H+ flux was seen on both the mucosal and serosal sides of the acidotic split skins. The isolated epithelia were fixed, postosmicated, and dehydrated in the chamber. They were then embedded in Spurr's resin and 1-micron sections were cut and stained with Paragon multiple stain. Coded slides were used to count various cell types. Sections were randomly selected and approximately 40,000 cells were counted. Four basic cell types were noted and confirmed by TEM photomicrographs; basal (B) cells, granular (G) cells, keratinized cells, and MR cells. The ratio of G + B cells:MR cells in the normal skins was 1.0:0.021. The ratio in acidotic skins was 1.0:0.34. The average percentage of cell population of MR cells in the normal skins was 2.08 + 0.18 and in acidotic skins 3.20 + 0.36 (P less than 0.005). We conclude that the split skin maintains the capacity to acidify the mucosal fluid. Additionally, during metabolic acidosis there is an increased number of MR cells in the skin and this increase may be an adaptive mechanism of the skin to excrete excess H+ during acidosis.

The effect of freezer type, cryoprotectant, and processing methods on viability of frozen embryos.

Two hundred forty excellent-quality blastocysts flushed from 53 superovulated Holstein heifers were frozen by 1 of 16 procedures in a 2 x 2 x 2 x 2 factorial design. The main effects included a simple, inexpensive, portable mechanical freezing unit instead of a programmable Liquid Nitrogen (LN) freezer for freezing bovine embryos, cryoprotective agents dimethyl sulfoxide (DMSO) and glycerol, addition rates of the cryoprotectants and freezing rates. Embryo viability was assessed morphologically and by fluorescein diacetate (FDA) evaluation. Neither the type of freezer, the cryoprotectant nor the rate of cryoprotectant addition affected embryo viability. Embryo survival after 12 h of incubation was higher (P < 0.05) using a conventional freezing rate than a two-step method (37.2 vs 16.5%). The correlation coefficient between viability evaluation methods (morphology vs FDA) was influenced by cryoprotectant and embryo processing methods and ranged from -0.13 to +0.70. This study indicates that more simplified embryo freezing equipment and handling procedures may provide protection equal to that of more complicated, expensive equipment and more time-consuming methods. Economical on-farm embryo freezing is feasible.

Luteinizing hormone response to pulsatile luteinizing hormone-releasing hormone in prepubertal heifers.

The effects of 12 hourly 5-micrograms injections of luteinizing hormone-releasing hormone on luteinizing hormone release, were examined in 18 prepubertal Holstein heifers at 4, 7, or 10 mo of age. During a 6-h pretreatment period, mean serum luteinizing hormone concentrations and mean number of endogenous luteinizing hormone episodes per hour were not influenced by age. The 12-h treatment regimen induced a pulsatile release of luteinizing hormone in all heifers. The magnitude, pattern, and total amount of luteinizing hormone released were not influenced by age. However, in the 4 and 10-mo-old age groups, magnitude of luteinizing hormone response to the 3rd hourly injection of luteinizing hormone-releasing hormone was greater than the response to the second injection. Magnitudes of luteinizing hormone responses decreased with time after the 4th hourly injection through the 12th injection and patterns of decline appeared similar among the three age groups. The pituitary of the prepubertal dairy heifer is able to respond to an hourly pulsatile administration of luteinizing hormone-releasing hormone and this treatment regimen appears to produce a self-priming effect on luteinizing hormone release.

Alcoholism treatment with environmental support contracting.

Environmental support contracting was evaluated as an ancillary treatment to more traditional alcoholism rehabilitation. Eighty-eight male alcoholics, who scored 10 or higher on the Environmental Deprivation Scale (EDS), were randomly dichotomized between experimental and control groups. The control group participated only in the routine, eclectic, 28-d inpatient alcoholism treatment program at this medical center. The experimental group additionally completed training to improve environmental support. Dependent variables included the EDS, the Maladaptive Behavior Rating (MBR), days of hospitalization, medical visits, and the number of medications. Each dependent variable was assessed prior to treatment, 3 months following initiation of treatment and/or 6 months following treatment. A high positive correlation was obtained between EDS and MBR scores on the 3 and 6 month followup. Both the experimental and control groups improved with regard to their level of environmental and control groups improved with regard to their level of environmental support and relative coping ability, showing less maladaptive behavior and hospitalization following treatment. The experimental group demonstrated statistically significant treatment benefit over the traditionally treated control group on both the EDS and MBR subcategories of employment at 6 months following initiation of alcohol treatment. Environmental support contracting appears to be a worthwhile addition to alcohol treatment regimen.

Role of age and length of oestrous cycle in alteration of the oocyte and intrauterine environment in the rat.

Zygotes were transferred, on the day of fertilization, from young and old rats with 4- or 6-day oestrous cycles into the ovarian bursa of young recipients with 4-day cycles, and zygotes from young rats with 4-day cycles were transferred into young and old recipients with 4- or 6-day cycles. Young rats with 4-day cycles served as controls for both donors and recipients. An increase in length of cycle or maternal age of donor caused an increase in unfertilized and/or abnormal eggs at the pronuclear stage (non-transferred zygotes). Increased age of donor or length of cycle decreased the implantation rate observed on Day 11 of pregnancy. Likewise, increased age of recipient or length of cycle decreased implantation rate observed on Day 11 of pregnancy. The increase in both age and length of cycle of donor or recipient caused the greatest decline in implantation rate and percentage of normal embryos observed on Day 11 of pregnancy.

Cerebral dysfunction associated with alcohol consumption.

Intellectual dysfunction, usually involving short-term memory, abstract reasoning, visuomotor coordination, intellectual flexibility, and spatial reasoning, is a well-documented finding among alcohol imbibers of various drinking styles. Despite the diversity of groups examined and the attempted control of variables other than alcohol consumption, little concensus has been reached with regard to the chronology of dysfunction (as related to lifetime drinking style, duration of alcoholism, quantity consumed per session, or type of potable). In many instances, diet, age, and even sex have outweighed the relative influence of alcohol in the degree of intellectual dysfunction. Morphologically, brain changes tend to be in the anterior/basal areas and may involve the third ventricle, diencephalic areas, and other focal variations. Cerebellar changes also are not uncommon yet may reflect a subgroup of alcoholics rather than a generalised trend. Generally, many intellectual deficits partially rebound rapidly within the first two or three weeks after cessation of drinking, with perhaps modest further improvement if sobriety is sustained for several months. Because of the extreme heterogeneity of the alcoholic population and the potent effect of ancillary variables upon measures of intellectual functioning, control and matching of demographic factors is a paramount consideration. Demographic variables of primary concern include age, sex, socio-economic status, education, duration of alcoholism, quantity of alcohol consumed per session, diet, psychotropic medications, secondary psychiatric diagnoses, secondary medical diagnoses, exact duration of abstinence, and prior exposure to psychometric instruments.

Introductory remarks: environmental and endogenous hazards to the female reproductive system.

The toxic action of environmental chemicals on reproduction and the toxiclike effects of ovarian hormones have been shown by numerous investigators. Some toxic effects of environmental chemicals on reproduction are through their action on hormonal secretion. Such effects on the endocrine system could provide additional approaches to investigations of toxicity of compounds. A better understanding of interactions of environmental chemicals with the endocrine system is needed. Therefore, it becomes important for a greater exchange of information and more interaction between toxicologists and endocrinologists.

Efficacy of a three- versus a five-week alcohol treatment program.

Eighty-six male alcoholic inpatients were randomly assigned to comparable 3- or 5-week treatment progrrams. Psychometric testing at the beginning and the end of treatment was supplemented by follow-up data 6 months after discharge. While both the 3- and 5-week groups demonstrated significant improvement on both MMPI and follow-up data, few differences between the groups were revealed. The comparable results obtained offer little justification for prolonging inpatient treatment beyond 3 weeks. It is suggested that the inpatient phase of alcohol treatment be brief and oriented to developing a well-structured extended outpatient program.

EMG biofeedback applicability for differing personality types.

Divided thirty-two male alcoholics into two groups on the basis of their MMPI profile. Group 1 consisted of Ss determined to be tense and anxious, while Group 2 included a more heterogeneous sample of personality types. Eight Ss from each group received 14 days of progressive relaxation training and EMG biofeedback; the remainder received control sessions of taped music and EMG monitoring. Group 1 experimental S achieved greater levels of relaxation than either their matched controls or Group 2 Ss. POMS scores revealed a significant difference over time for all Ss in the direction of improved mood states. No significant between-groups difference was found. Generally, these results suggest the following: (1) patients may be selected credibly for relaxation-biofeedback treatment by use of the MMPI; (2) patients who exhibit significant anxiety and tension may achieve lower levels of tension during training than patients with lower levels of pretreatment anxiety.
