Since there is very little available systematic information on the psychological profile of patients with cardiac arrhythmia, a battery of 12 standardized personality inventories was administered to 102 patients ranging in age from 19 to 69 years. Thirty-eight patients with frequent ventricular premature beats (more than 30 per hour) without myocardial infarction were significantly more psychologically symptomatic than 34 age- and sex-matched general medical/surgical patients. The variables found to be significant portray the patient with frequent ventricular premature beats without myocardial infarction: high scores for hysteria, less moral orientation, more anxiety, depression and social alienation, and an inhibited and low respectful style. This combination of psychological variables produced a discriminant function (p less than 0.001) that accounted for 53 percent of the variance between the arrhythmia/no myocardial infarction group and the medical/surgical control group and could correctly predict group membership in 83.3 percent of cases. These results may have further implications in nonpharmacologic and psychotropic adjuncts to antiarrhythmic therapy.
Between January of 1978 and December of 1983, 41 patients developed deep sternal infections with mediastinitis after cardiac operations. Between January of 1978 and December of 1981, 19 of these patients were treated with débridement, primary wound closure, and mediastinal antibiotic irrigation (Group I). Between January of 1982 and December of 1983, 22 patients were treated with débridement, open "clean" packing, and delayed wound closure by the technique of pectoral muscle flap mobilization, which preserves the thoracoacromial pedicles and the pectoral humeral attachments (Group II). The purpose of this study was to compare the results of the treatment of deep sternal infections after cardiac operations with these two techniques. The perioperative hemodynamic, operation, functional, and pathological profiles of both groups of patients were the same. The cosmetic and functional results were the same in both groups as were shoulder girdle and torso mobility. We conclude that either technique is equally effective in the management of patients in whom the serious complication of deep sternal infection with mediastinitis develops after cardiac operation, and we now recommend débridement and pectoral muscle flap closure in one stage.

Orbitofacial neurofibromatosis may cause severe soft-tissue deformity along with pulsating exophthalmos. The orbit is enlarged, and the greater wing of the sphenoid is absent. The eye in some cases may be functionally useless, immobile, displaced, and blind. In these, orbital exenteration with soft-tissue excision and orbital reconstruction is recommended. External cover is provided by the thin eyelid skin, and a prosthesis ultimately allows a very satisfactory end result. Four such cases with a minimum of 2 years of follow-up are presented.
No seasonal variation in any of the hormones measured was apparent in males or females. Testosterone levels in males increased around puberty (10-11 years) and remained significantly higher in adult than prepubertal males. This was not accompanied by any significant change in levels of LH, FSH or prolactin. In non-pregnant females there was no apparent difference in levels of LH, FSH or prolactin with age. There was a significant increase in progesterone around puberty (12 years) but there was considerable overlap in values between prepubertal and adult females. During pregnancy, progesterone levels were significantly higher than in non-pregnant females with maximum levels occurring at mid-pregnancy (9-12 months). However, there was considerable overlap in values between non-pregnancy and pregnancy. Concentrations of LH and FSH decreased significantly during mid-pregnancy while prolactin levels increased dramatically during pregnancy; after 7 months of gestation until term levels were always at least 8 ng/ml greater than in any non-pregnant female. It is suggested that this consistent increase in plasma/serum levels of prolactin can be used to diagnose pregnancy in the elephant.

Despite the common occurrence of trauma to the hand, arterial aneurysms in this area are rare. The authors describe the symptoms, signs and microvascular treatment of a true aneurysm of the ulnar artery in the palm of an 18-year-old youth. Although there is no clear evidence that vascular restoration is needed after resection of the aneurysm, the authors believe that reconstruction of the ulnar artery using microvascular techniques will ensure that blood supply to the digits is adequate.

The role played by fantasy in moderating the overt expression of aggression was investigated by examining the relationship between aggressive behavior and hostile content on the Thematic Apperception Test. A Behavior Rating Scale, Behavior Checklist, and Aggressiveness of Offense Scale (Matranga, 1976) were used to measure the aggressive behavior of 15 incarcerated women between 16 and 20 yr. old. No support was found for the hypothesis that hostile content on the TAT is negatively correlated with behavioral measures of aggression. Instead, results suggested that there is no significant relationship between these variables, at least with an incarcerated, predominantly Native female population.

A small group of patients is seen because of massive benign facial tumors, which include both soft tissue and bone overgrowth. These people cannot live a normal life because of their facial deformities. Four patients have been chosen to describe a one-stage technique of soft tissue removal and repositioning the facial skeleton utilizing a reverse facial osteotomy. Detailed planning by an experienced craniofacial team, together with preoperative embolization and profound hypotensive techniques have made surgery feasible with minimal blood loss. Rapid and effective rehabilitation of these unfortunate people now can be achieved.
