The transmandibular implant: a 13-year survey of its use.

During the period from August 1976 until July 1989 the transmandibular implant was introduced in a total of 31 oral and maxillofacial surgery departments in the Netherlands and 1,466 patients were treated. This study includes 1,356 treated patients from 28 departments. The treatment course of 1,201 patients (89.6%) was uneventful. Of the 157 complications, 58 related to the anatomic contour of the mandible, 49 to the surgery, 25 to the prosthetic reconstruction, 8 to the psychosocial attitude of the patient, 8 to the use of medication, 5 to trauma, and 4 to the presence of a skin graft. Treatment of the complications included removal of the implant in 44 patients; in 106 patients the complication was treated satisfactorily. In the remaining 7 patients, hypesthesia was present. The reasons for the removal of the 44 implants were infection (11), operative error (5), premature loading of the implant (3), insufficient width of the mandible after a visor osteotomy (1), loss of osseointegration caused by the prosthesis (14), psychological problems (4), trauma (3), drug-related gingival hyperplasia (2), and inexplicable pain (1). The present survey shows a consistent success rate of 96.8%.

The effects of mechanical strain on osteoblasts in vitro.

The effect of mechanical strain on bone is important to the field of oral and maxillofacial surgery. Oral and maxillofacial surgeons are involved on a daily basis with problems including alveolar ridge resorption, implant stability, craniofacial growth, and bone resorption related to trauma and pathology. To understand and control these effects on bone, it is important to examine the effects of mechanical strain on the osteoblasts. This series of experiments provides the changes in alkaline phosphatase, collagen synthesis, and protein synthesis in osteoblast-like cell subjected to mechanical strain.

Alkaline Phosphatase

An evaluation of porcelain strength and the effect of surface treatment.

This investigation evaluated the flexural strengths of five porcelains commonly used in the all-porcelain margin technique and the effect of surface treatment on the flexural strength. Thirty samples were made by using each of five different porcelain margin systems. The five porcelain groups were subdivided into groups of 10 each to allow the evaluation of three surface treatments. A three-point flexural test was used to test the specimens on a universal testing machine. Crystar shoulder porcelain with distilled water as the binder was significantly stronger than the other porcelains tested, and porcelain treated with an overglaze was stronger than porcelain treated with autoglaze or autoglaze and polish.

Dental Casting Technique

Remount casts for removable partial dentures made in a laboratory.

A remount cast for a removable partial denture can be made in the laboratory by making an elastomeric impression of the prosthesis on the cast after processing but before removing it from the cast. After the prosthesis is finished and polished it is placed in the impression, undercuts are blocked out, and a mounting cast is made.

Dental Occlusion

Fabrication of an alloplastic implant for the cranial defect.

This article describes the presurgical fabrication of an inlay/onlay alloplastic cranial implant prosthesis. The technique eliminates the need to modify an artificial stone master cast, thus simplifying the technique and decreasing the laboratory time involved. The final prosthesis replaces lost cranial contours and has a positive seat on the unaltered cranium adjacent to the defect.

Acrylic Resins

Immediate fixed partial dentures.

This technique offers immediate fixed partial denture replacement of dentition lost in either an anterior or posterior location. It allows modification of the preextraction tooth or the diagnostic wax-up. Because the provisional restoration is made indirectly, the extraction site is not contaminated. Postoperative comfort and function for the patient is achieved.

Denture Design

Infection control during elastomeric impressions.

This technique uses an individual applicator per application of an elastomeric adhesive to an acrylic resin tray. To help prevent contamination of the adhesive in the bottle, the Toothette toothbrush should not be dipped into the bottle a second time. A sufficient quantity of adhesive is absorbed with a single immersion into the adhesive to coat an entire tray. The uniformity of the surface application is more easily controlled compared with that of a brush applicator. Because only one immersion is required to coat a tray and the Toothette toothbrush is discarded after one application, cross-contamination of patients is prevented.

Communicable Disease Control

Acid-etched splinting to a ceramometal abutment.

This technique describes an acid-etch metal splint with a ceramometal abutment. The internal surface of the DuraLingual Wing Form patterns provide undercuts necessary for composite bonding. The external surface provides a smooth solid metal surface when cast. Since this system uses mechanical undercuts, the resin-bonded splints can have multiple try-ins without the detrimental effects of burnishing and contamination that occur with electrolytically etched metal surfaces. If an abutment for a resin-retained fixed partial denture requires a ceramometal crown, a DuraLingual Wing Form can be incorporated onto its lingual surface thereby providing mechanical undercuts for bonding. The opposite undercuts of the crown and splint provide excellent bond strength for the system.

Acid Etching, Dental

Using ultrasonic scalers to remove fractured root posts.

The removal of an existing cast or cemented stainless steel post has proved difficult, time consuming, and to require excessive removal of tooth structure. The use of the ultrasonic scaler alone and in combination with other techniques provides a conservative, efficient method for retrieval of broken posts from root canal spaces. The primary advantage of these techniques in the removal of posts is the conservation of remaining tooth structure.

Bicuspid

Multiple cast post and cores for severely worn anterior teeth.

A technique has been presented to simplify the fabrication of multiple direct post and core patterns. When combined with techniques for indirect provisional restorations, this technique merely requires the fabrication of an additional plastic template to form the direct core patterns.
