Cerebrospinal fluid concentrations of beta-endorphin in chronic low back pain patients. Influence of depression and treatment.

Cerebrospinal fluid (CSF) beta-endorphin concentrations were determined before and after treatment in 28 patients suffering chronic neuralgic low back pain/sciatica. Nine patients carried the additional diagnosis of major depressive disorder. Pain treatment was multimodal and resulted in variable pain reduction. CSF beta-endorphin concentrations spanned a wide range with no association to age, gender, pain ratings, depressive symptomatology, and drug intake. CSF beta-endorphin concentrations were not influenced by the presence of major depressive disorder and did not change with successful treatment of pain and resolution of depression.

Management of grief in the hypochondriac.

This paper illustrates the presentation and management of a grief reaction in a chronic hypochondriacal patient seen in a family medicine office. The manifestations of grief in the hypochondriacal patient are influenced by the unique characteristics of the chronic, somatic complainer in whom the grief reaction appeared as an intensification of the chronic complaints. The treatment of the grief reaction in these patients should take into account the techniques for managing the hypochondriacal patient.

Referral of patients from primary care physicians to a community mental health center.

The referral process between primary care physicians and a community mental health center was examined in a community which lacks a geographically based psychiatrist. The method included inspection of referral completion rate and evaluation or patients' characteristics and attitudes concerning mental health care. The findings indicated that only 10 per cent of referred patients who have had no prior contact with the community mental health center completed the referral. Reasons for this low referral completion rate are discussed.
