Molecular mechanism for feedback regulation of C4 biosynthesis in guinea pig peritoneal macrophage.

Previous reports have shown that regulation of local extrahepatic production of complement may not reflect the regulation of plasma concentrations of the corresponding proteins and, further, that alteration of the tissue microenvironment can affect local macrophage protein synthesis. This report describes the molecular basis for control of the biosynthesis and secretion of a class III major histocompatibility complex gene product, the fourth component of complement (C4), from guinea pig macrophages by extracellular native C4 protein. The effect is specific for C4 synthesis, since production of C2 and total secreted protein was unaffected by fluid phase C4. C4 synthesis by extracellular C4 is regulated at a pretranslational level, without an effect on posttranslational proteolytic cleavage, glycosylation, or secretion. Specific C4 and factor B cDNA probes were used to demonstrate, by dot hybridization and Northern blot analysis, a decrease in messenger RNA coding for C4 that paralleled the inhibition of C4 biosynthesis, while the amount of total RNA and mRNA specific for factor B remained constant. Inhibition of C4 biosynthesis and the disappearance of mRNA encoding C4 occurred between 4 and 6 h after exposure of the macrophages to biologically active or methylamine-inactivated C4 protein. These data demonstrate that regulation of C4 biosynthesis by guinea pig macrophages serves as a model for the study of the molecular mechanisms of macrophage activation as well as the control of production of a component of the inflammatory response.

Controlled trial of rifampicin and ethambutol in Crohn's disease.

We pursued the possibility that Mycobacterium kansasii might be an aetiological agent in Crohn's disease by carrying out a trial of treatment with antimycobacterial drugs. Twenty seven patients with Crohn's disease took part in a two year randomised double blind, crossover, controlled trial of rifampicin plus ethambutol against placebo. Fourteen patients completed the trial; four required an operation; five were withdrawn as poor compliers, and four because of adverse effects. There was no significant difference in response to the active drugs compared with placebo when expressed in terms of a Crohn's disease activity index or any clinical indicator of disease activity. There was no suggestion that any subgroup of patients - for example, different regions of bowel affected or previous operation - were favourably affected by the drugs. There was no consistent pattern of change in prednisolone requirements although eight patients on long term sulphasalazine had a significant reduction in their plasma sulphapyridine concentrations during the active treatment period. A significant reduction in total white blood count and an increase in plasma ALT were seen during active therapy. The results of the study do not suggest that rifampicin and ethambutol have a role to play in the treatment of Crohn's disease.

QUEST--a package for processing questionnaire text and data.

An easy-to-use interactive program (QUEST) has been developed which facilitates the input, updating and checking of patient records from completed questionnaires possibly destined for statistical analysis by SPSS. Using the information about the data available in the SPSS control file, blank questionnaires can be generated, and up to date completed questionnaires printed. Selected cases can be printed out. QUEST is currently being used in four medical studies and one legal study. The program is written in standard Fortran-77 and makes full use of the new features of the language.

Evaluation of low back pain and assessment of lumbar corsets with and without back supports.

A method is described for evaluating the progress of patients with back pain. Various symptoms were scored on analogue scales and spinal motion was measured by various techniques. These data were integrated to make subjective and objective indices respectively. Reasonable reproducibility of the measurements was obtained. This technique was used to elucidate the role of the lumbar support in surgical corsets in relief of back pain. Sufferers from back pain were randomly allocated to corsets with and without lumbar supports. There was significant improvement in those with a support compared with those without. On the other hand objective changes measured with the corset removed did not differ between the 2 groups. This study indicates that the spinal support in a lumbosacral corset makes a significant contribution towards the relief of symptoms.

Functionally deficient differentiation of HL-60 promyelocytic leukemia cells induced by phorbol myristate acetate.

The human promyelocytic leukemia cell line HL-60 undergoes terminal myeloid differentiation in vitro in response to a wide variety of chemicals. The tumor promoter phorbol myristate acetate induces these cells to develop macrophage-like morphology, adherence, and enzymatic characteristics. The present study confirms those observations and further documents the induction, by 16 nM phorbol myristate acetate, of 5'-nucleotidase activity, another human macrophage marker enzyme. However, more importantly, functional studies show that phorbol myristate acetate-induced HL-60 cells fail to increase above base line uninduced levels of hexose monophosphate shunt activity, superoxide generation, nitroblue tetrazolium reduction, bacterial ingestion, or complement secretion. These cells therefore possess some macrophage-like properties but do not meet several important functional criteria of macrophage identity.

Progeny testing for bovine syndactyly.

Syndactylous donors, superovulation, embryo transfer, and fetal removal via cesarean section were used to test four Holstein-Friesian bulls suspected of being heterozygous for syndactyly. Two bulls produced syndactylous fetuses, and thus were proved heterozygous. The other two bulls each sired eight normal fetuses and were assumed to be homozygous normal.

A controlled trial of disodium cromoglycate in the treatment of Crohn's disease.

The effect of oral disodium cromoglycate (1.2 g/day) was compared with placebo in 18 patients with Crohn's disease using a double-blind cross-over trial. No improvement occurred in symptoms, signs or Crohn's disease activity index and no change in the frequency of relapses occurred. Disodium cromoglycate has not been shown to be of benefit in the management of patients with moderately active or quiescent Crohn's disease. Unexpected findings were a statistically significant, but probably unimportant, increase in mean corpuscular volume and a fall in serum urate concentration during treatment with cromoglycate.

Revascularization and healing of onlay particulate autologous bone grafts in primates.

Two sizes of bone chips were compared to determine the size that revascularizes faster and what effect this has on the resorption of the graft. To accomplish this, autologous chips of iliac corticocancellous bone of either 2 X 2 X 2 mm or 5 X 5 X 2 mm were placed in the mandibular cortex of monkeys bilaterally. In general, the small-particle graft was quicker to revascularize, showed more osteoclastic activity, and therefore resorbed much more quickly and completely than did the large-particle graft. The resultant net gain in alveolar ridge contour was, therefore, less with the small-particle grafts.

Squamous-cell carcinoma arising in a lateral periodontal cyst.

A rare case of well-differentiated squamous-cell carcinoma arising in the epithelial lining of a lateral periodontal cyst is reported. Submission of surgically removed tissues for histopathologic evaluation is emphasized. The surgical site has been reconstructed and functions well. Because of early diagnosis and treatment, the patient has no evidence of clinical disease 2 1/2 years postoperatively.

Long-term results of alveolar ridge augmentation.

Autologous onlay rib grafts in 22 patients were evaluated for bone remodeling patterns. The patients had been followed up for four to ten years; the mean was 6.2 years. The unquantitated increase in bony mass of the mandible, with altered ridge form and subsequent soft tissue surgery, provided consistent, predictable results and return to excellent denture function with long-term stability.

Successful use of a soya-maize mixture in the treatment of kwashiorkor.

Forty-eight children aged 5 years or less admitted consecutively with a clinical diagnosis of kwashiorkor were randomly allocated to two groups. One group was treated with milk while the other group was fed soya-maize porridge; both groups received the same supportive treatment. The recovery rate in both groups was comparable. Protein intake and the rate of rise in serum albumin and transferrin concentrations were greater in the milk-fed group. The incidence of diarrhoea was significantly less in the children fed soya-maize. Soya-maize porridge is recommended as an effective and inexpensive treatment for kwashiorkor.

Vital root retention in humans: a preliminary report.

With increasing experience in the technique, it has become apparent that more generous contouring of the retained root after amputation is desirable. The effect is to produce a more rounded ridge instead of the "boxy" one that would result from mere horizontal sectioning of the crown structure. The surgical sectioning of vital teeth, followed by anequate mucoperiosteal eversion suturing, has proved to be successful. Periodontal evaluation of each tooth seems to be paramount to success or failure. Accurate impression-making and postsurgical treatment including soft acrylic resin relines and removal of pressure spots appear to be next in importance. Age of the patient, condition of the clinical crowns, and length of surgical procedures did not seem to affect normal healing in the patients involved in the project.
