The purpose of this analysis was to determine the most important factors contributing to operative wound infections for patients with head and neck cancer. Four hundred cases were studied prospectively at M. D. Anderson Cancer Center within an 18-month interval. Potential risk factors were categorized based on the patient, the disease, and the treatment. Sixty-three (19.75%) wound infections were recorded. Univariate analysis identified the following factors to significantly alter the incidence of the infection: nutritional status and alcohol consumption (patient factors); T stage and N stage (disease factors); and duration of surgery, type of surgical wound, complexity of the procedure, use of flaps, blood replacement and the use of drains, nasogastric tubes, and tracheostomies (treatment factors). A logistic regression analysis identified the type of surgery, the choice of antibiotic, the presence of concomitant disease, and the N stage to represent the combination of factors most predictive of infection. The initial step toward preventing surgical wound infection is to identify the high-risk factors. The results of this study help to define these parameters so that specific measures can be taken to counteract wound infection.
Cultured astrocytes exposed to ethanol were prelabeled with [3H]inositol and the accumulation of [3H]inositol phosphates was determined following stimulation with norepinephrine (NE). Acute doses of alcohol (25-200 mM) had little effect on phosphoinositide (PI) hydrolysis. However, chronic exposure for 7 days produced significant increases in hydrolysis with ethanol concentrations as low as 50 mM. The onset of alcohol-induced increases occurred within 4 hr of exposure, reaching maximum values by Days 3 to 5. Withdrawal resulted in the return of stimulated PI hydrolysis to pre-alcohol exposure levels after 2 days. The evidence provided suggests that chronic exposure of ethanol alters the alpha-adrenergic receptor characteristics of astrocytes and/or enhances the effector pathway leading to PI hydrolysis at a site distal to the receptor.
Despite the widespread use of liquid nitrogen, there are few reports of injuries caused by the use of this substance. We report a case in which a severe liquid nitrogen burn, requiring digital amputation, occurred despite the wearing of protective gloves. This case illustrates the potential dangers of working with liquid nitrogen, and raises questions as to whether adequate precautions are being taken to prevent such injuries.
We have developed smaller, lighter hardware (MCA and shielded detectors) that has been successfully transferred to the commercial sector. We have developed software that has received limited testing, the next release should be sufficiently robust for wide release to the general public. We have had initial success with more sophisticated analysis methods. We are pursuing medium-resolution tors and response function fitting in the expectation that both will cause significant improvements in this measurement technique.
This tool, consisting of software with a compact detector, an M3CA, and an Intermec datalogger: 1. acquires and manages several hundred spectra in an hour; 2. produces prompt and reliable quantitative analyses; 3. is self-powered, easily carried and operated by a single user; 4. accomodates varying degrees of user expertise; 5. corrects for the effects of equipment geometry; 6. corrects for the effects of equipment attenuation; 7. tolerates facility variables such as temperature fluctuations and equipment accessibility; 8. has been subjected to quality testing under field conditions; and 9. does not require extensive specialized training for the operators.
The effects on immunotoxin efficacy of fusogenic peptides derived from influenza virus hemagglutinin have been studied. These peptides have an amphipathic nature and change conformation from random at pH 7 to helical at pH 5. Fusogenic peptides are reported to destabilize endosomal membranes, resulting in the release of contents into the cytoplasm. The use of two related fusogenic peptides to enhance the efficacy of anti-HIV immunotoxins is described. The direct toxicity of the peptides was tested on HIV-infected H9/NL4-3 cells. Peptide HA24 was considerably more toxic than HA23. The peptides were mixed with two different immunotoxins. Immunotoxin action was enhanced by both peptides, with HA24 providing greater enhancement than HA23. Immunotoxins were then constructed by coupling HA23 or HA24 to the targeting antibody with disulfide-containing linkers. Peptide HA23 enhanced the activity of the immunotoxin 4-5-fold. Surprisingly, HA24 significantly inhibited immunotoxin activity. Coupling the peptides to the immunotoxin had no effect on antigen binding characteristics or the activity of the toxic moiety. Bafilomycin A1, an agent that inhibits vacuolar acidification, markedly potentiated the effects of all immunotoxins. These results demonstrate that amphipathic peptides can influence the efficacy of immunotoxins, but in sometimes unpredictable ways.
Methamphetamine abuse is increasing and methamphetamine is second only to alcohol as a positive finding in cases submitted to the San Diego Sheriff's Crime Laboratory. In general, whole blood specimens are submitted more often than urine. A modified version of a commercially available radioimmunoassay, Coat-A-Count (CAC) Methamphetamine, was investigated as a screen for methamphetamine in whole blood and urine. The assay was modified by using 100 microL of sample, making up standards in whole beef blood, extending the incubation time to 2 h or overnight, and using a cutoff reference of 50 ng/mL methamphetamine. The detection limit for the CAC Methamphetamine kit was 20 ng/mL methamphetamine in whole blood. The CAC Methamphetamine results were compared to Abuscreen Amphetamine High Specificity results and to gas chromatography/mass spectrometry (GC/MS) quantitation of amphetamine and methamphetamine for 157 positive and 48 negative blood specimens. With the CAC Methamphetamine assay there were 2 false negatives detected, both less than the 50 ng/mL cutoff level. There were 12 (6%) false positives with the CAC Methamphetamine assay and 29 (14%) false positives with the Abuscreen Amphetamine assay. Of the positive samples, 95% contained only methamphetamine, with an average concentration of 308 ng/mL, range 25-2030 ng/mL.
Six genera of rotifers including Philodina, Monostyla, Epiphanes, Euchlanis, Brachionus, and Asplanchna were exposed to oocysts of Cryptosporidium parvum cleaned of fecal debris. Unstained oocysts and those stained with fluorescein-conjugated monoclonal antibody were added to suspensions of viable rotifers and were examined by phase-contrast, differential interference contrast, and fluorescence microscopy. Rotifers of all six genera were observed ingesting oocysts. A maximum of 25 oocysts was observed in the stomachs of Eauchlanis and Brachionus. Euchlanis and Epiphanes were observed excreting boluses containing up to eight oocysts. It was not determined whether rotifers digested or otherwise rendered oocysts nonviable.
