Strategies for positioning in the managed health care marketplace.

Managed health care is becoming increasingly common as the demands of cost containment are placed on providers of care. This article defines managed health care, illustrates its continued growth, demonstrates its effect on clinical decision making and reimbursement issues, and suggests strategies for optimal positioning in the managed care marketplace. The hand therapy specialist, whether based in a hospital, an institution-based ambulatory care setting, or a private practice, must be aware of a managed plan's contractual limitations. Parameters discussed are patient length of stay, documentation, reimbursement, patient responsibility, alternatives to conventional treatment protocols, and the potential effects of utilization review on patient treatment. A heightened awareness of managed health care is critical for the manager and practitioner, especially because national health care reform is on the horizon. A provider must be well prepared to ensure delivery of quality care within the myriad restrictions imposed by managed care regulations.

Making sense of physical therapy.

Facility-based and payer-based case managers are increasingly having to interact with physical therapists in acute care hospitals, skilled nursing facilities, rehabilitation hospitals, out-patient clinics, and home care settings. Each level of care offers challenges to the case manager and physical therapist. It is critical that communication between the two be efficient and timely. Misunderstandings will interrupt the flow of patient care and have the potential to negatively impact outcomes of care. There are many resources available to the case manager that will help eliminate any of the confusion about services provided by physical therapists. This article explains the role of the physical therapist and strategies offered for successful collaboration between case manager and therapist.

Cortical coding.

Physiological photic and sound stimulation generated almost synchronous summated cortical evoked peak interval patterns over all regions of the head studied in man. Although the peak interval synchrony in all regions was obvious, it was observed that the peak amplitudes, and even phase relations varied over the different brain regions. The almost invariant peak time interval patterns for the input modalities studied, suggested a simple, generalized direct cortical coding that signaled immediate environmental change to the entire brain.
