Factors contributing to physicians' success in the advanced trauma life support program in Israel.

BACKGROUND: Since its introduction in Israel, more than 4,000 physicians from various specialties and diverse medical backgrounds have participated in the Advanced Trauma Life Support course. OBJECTIVES: To analyze the factors that influence the success of physicians in the ATLS written tests. METHODS: A retrospective study was conducted of 4,475 physicians participating in the Israeli ATLS training program between 1990 and 1996. Several variables in the records of these physicians were related to their success or failure in the final written examination of the course. RESULTS: Age, the region of medical schooling, and the medical specialty were found to significantly influence the successful completion of the ATLS course. CONCLUSIONS: Physicians younger than 45 years of age or with a surgical specialty are more likely to graduate the ATLS course. The success rate could be improved if the program's text and questionnaires were translated into Hebrew.

[Impact of a multidisciplinary approach for monitoring prescribing of antibiotics in a hospital].

OBJECTIVE: The aim of this study was to examine the impact of a rationalized antibiotic prescription program in a 600-bed hospital. PATIENTS AND METHODS: The program was based on a local consensus policy, developed with the prescribers themselves, together with individual nominative dispensing (IDN) of the most costly antibiotics, and training/information programs aimed at institutional prescribers. The impact of the program was assessed in terms of the relevance of prescriptions (6-monthly audits), changes in the overall cost of anti-infectives, the average daily cost per hospital patient, and changes in the pattern of antimicrobial resistance among pathogens isolated in the institution. RESULTS: Reference recommendations were established for most clinical settings encountered in the hospital (104 for adults, 78 for children). The four audits conducted since June 1996 show that the rate of unjustified prescriptions has remained below 6%. The cost of antimicrobials has fallen regularly, both in absolute terms and as a proportion of total drug costs. Finally, resistance rates among the different pathogens studied have fallen or remained stable. CONCLUSION: Rationalization of antimicrobial chemotherapy in a short-stay hospital necessitates an institution-based policy with long-term objectives, a large initial investment and, above all, a participative approach among all prescribers.

Monitoring brain activity of human subjects during delayed matching to sample tasks comparing verbal and pictorial stimuli with modal and cross-modal presentation: an event related potential study employing a source reconstruction method.

The time course of the event related potentials evoked within a delayed matching to sample task employing verbal and pictorial stimuli was analyzed with a source reconstruction method (minimum norm method). During signal stimulus presentation pictorial stimuli evoked more activity than verbal stimuli. Activity was particularly prominent in left frontal areas for the match of verbal-verbal stimulus pairs and over right posterior regions for the match of verbal-pictorial stimuli. Anticipation of the to-be-matched stimulus produced more pronounced activity for pictorial stimuli and generally stronger left and frontal activity. Results are discussed referring to a biological model of language processing.

A comparison of staging systems for nasopharyngeal carcinoma.

BACKGROUND: The fifth edition of the American Joint Committee on Cancer staging manual defines new rules for classifying nasopharyngeal carcinoma. The authors tested the value of this new system by applying these rules retrospectively to their previously treated patients and comparing the results with those obtained using the fourth edition of the AJCC staging manual or the Ho staging system. METHODS: Information from 107 patients who had biopsy-proven squamous cell carcinoma of the nasopharynx that was treated in a constant fashion with definitive-intent radiation therapy alone at one institution provided the data base for this analysis. The extent of disease of each patient was staged according to the rules of 1) the fourth edition of the AJCC staging manual, 2) the Ho staging system, and 3) the fifth edition of the AJCC staging manual. RESULTS: The new system appears to be better than the two previous systems. It separated patients into cohorts of more equal size than did either of the other systems. It also correlated with outcome for the study population more appropriately than did the fourth edition of the AJCC staging manual or the Ho staging system. CONCLUSIONS: The fifth edition of the AJCC staging manual appears to be an improvement over the previous AJCC or Ho staging systems for the staging of nasopharyngeal carcinoma.

[Utilization of rapid diagnostic tests for group A streptococcus and and bacteriologic and clinical correlations with acute angina in general medicine].

OBJECTIVES: A prospective study was conducted between November 1995 and May 1996 by 130 general practitioners in France to assess feasibility of a rapid routine diagnostic test for group A streptococcus infection in a general medicine setting and to search for bacteriological and clinical correlations. METHODS: A routine diagnostic test was performed in all patients presenting acute pharyngitis and cultures were ordered in case of positive tests. Among the 2,800 patients included, there were 563 children under 14 years and 2,226 adults. The routine diagnostic test was positive in 393 cases (14%). A culture was obtained in 375 case and isolated group A streptococcus in 324 (11.5% of the total population). The positive predictive value of the routine diagnostic test was 86.4% in this general medicine setting. RESULTS: Comparing clinical signs with the results of the routine diagnostic test showed that an erythematous pultaceous aspect of the pharynx, severe dysphagia and the presence of enlarged nodes were more frequent in patients with streptococcal pharyngitis. The association of these 3 clinical signs with fever > 38 degrees C was also more frequent in patients with a positive routine diagnostic test (OR = 3.3; 95% CI = 2.5-4.4). The triad hoarseness + cough + rhinorrhea was more frequent in subjects with a positive routine diagnostic test (22.7% versus 9%; OR = 2.6; 95% CI = 2.1-4.3). CONCLUSION: The general practitioners who participated in this study found the routine diagnostic test for group A streptococcal pharyngitis was easy to use and compatible with everyday practice. This diagnostic tool was seen as a progress in the management of acute pharyngitis, but only 53.7% of the practitioners were willing to use antibiotics only for cases where a highly specific routine diagnostic test performed during the consultation identifies group A streptococcus. Lack of reimbursement by the national health assurance however makes it impossible to use this test routinely in the general medicine setting.

Development of liposomal anthracyclines: from basics to clinical applications.

The pharmacokinetics of liposome-encapsulated drugs are controlled by the interplay of two variables: the rate of plasma clearance of the liposome carrier, and the stability of the liposome-drug association in the blood stream. The pharmacokinetic properties of the liposomal drug, the vesicle size of the liposome carrier and the vascular permeability of individual tissues will determine the extravasation and biodistribution profile. The pharmacokinetics of polyethylene-glycol-(PEG)-liposomal doxorubicin are characterized by an extremely long circulating half-life, slow plasma clearance and reduced volume of distribution compared to free doxorubicin. These carrier systems show an improved extravasation profile with enhanced localization in tumors and superior therapeutic efficacy in comparison to doxorubicin in free form. These properties are the result of an optimized liposome composition and of a special drug-loading method which produces stable and long-circulating carriers. In clinical studies, doxorubicin encapsulated in PEG-coated liposomes shows a unique pharmacokinetic-toxicity profile and promising antitumor activity.

Calcitonin receptor mRNA expression in TT cells: effect of dexamethasone.

Among the four isoforms of the calcitonin receptor (CTR) described in humans, two differ by the presence of h-CTR1 or absence of h-CTR2 of 16 amino acids in the first intracellular loop. Both receptors are biologically active. The TT cell line derived from a human medullary carcinoma of the thyroid is characterized by the secretion of large amounts of calcitonin. We have recently shown that this cell line expresses h-CTR2. In the present work we have studied the expression of CTR during TT cell proliferation and used dexamethasone to modify calcitonin expression in order to establish if an autocrine regulation involving calcitonin and its receptor was functional in the TT cells. The expression of this receptor and of calcitonin during TT cell proliferation was studied by reverse transcriptase-polymerase chain reaction (RT-PCR). Dexamethasone, a potent inhibitor of TT cell proliferation, levels (day 6 of culture) specifically increased receptor levels from day 8 onwards. CT peptide and CT mRNA levels decreased or were similar during experimental time. CTR regulation by glucocorticoids is suggested in TT cells. Autocrine regulation of CTR is also suggested by relation between CT mRNA levels and CTR mRNA.

A comparison of multileaf-collimator and alloy-block field shaping.

PURPOSE: The purpose of this report was to compare the dose distribution at a field edge defined with divergent alloy blocks to the distribution obtained with a multileaf collimator (MLC). The comparison is made for simple block replacement situations. METHODS AND MATERIALS: A tertiary multileaf collimator mounted on a linear accelerator operating at 6 MV was compared to divergent alloy blocks positioned at the level of the blocking tray. The leaves of the MLC were positioned to give maximum stepping (leaf displacement equals leaf width), and the blocking produced the same field shape. Three different treatment plans were compared: single field, opposed fields, and a four-field "box." Dose distributions were determined using radiographic film scanned with a laser densitometer with a 0.45-mm spot size. One experiment was repeated using radiochromic film with reduced energy dependence. Dose distributions were examined on the isocenter plane, and on planes displaced by 1.0 and 2.5 cm. The effect of daily setup variations was also studied by comparing a single fraction treatment with a fractionated treatment consisting of 15 fields slightly displaced relative to each other. The magnitude of these displacements was determined using available literature on treatment reproducibility. RESULTS: For a single field plan, maximum stepping of an MLC-defined edge produces an obvious undulating dose pattern compared to an alloy block edge. At the isocenter plane, this pattern is unchanged when parallel opposed fields are used. However, blurring occurs for both MLC and block edges when planes displaced from the isocenter are examined. The gradient for the block edge is 8%/mm for opposed fields and a plane 2.5 cm from the isocenter, compared to 15%/mm for the isocenter plane. Adding two additional fields does not change the dose pattern in the isocenter plane, but does reduce the gradient across the steepest portion of the penumbra to 8%/mm, and shifts the isodose line with the most pronounced stepping to higher values (from 50 to 80%). Introducing daily setup variations results in a reduction of the sharp dose gradient along the sides of a single field, and around the periphery of the beam at the isocenter plane of opposed fields. Smaller changes are found for edges already blurred by other factors. Radiochromic film was generally noisier than radiographic film, but comparison of the two films did not show a significant difference, indicating that the energy dependence of the radiographic film was not a problem. CONCLUSIONS: The obvious dose stepping seen on a portal image of a single field with MLC shaping is shown to be partially erased by the addition of other fields, and for planes away from the isocenter. However, the effects of daily setup variations must be included to more effectively blur dose stepping along the external envelope of a single field or near the isocenter plane of opposed fields. This result conflicts with attempts to improve immobilization.

Ectopic pancreas as the cause of gastric outlet obstruction in a newborn.

This case of ectopic pancreas found in the pre-pyloric channel of a 2-day-old infant is unique. A review of the literature reveals no other cases of symptomatic ectopic pancreas in an infant of this age. In this patient, signs and symptoms were consistent with pyloric stenosis. Upper gastrointestinal study and esophagogastroduodenoscopy (EGD) revealed the diagnosis. This case is examined and the literature is reviewed.

A multicenter, randomized, double-blind trial of 5 versus 10 days of antibiotic therapy for acute otitis media in young children.

BACKGROUND: All but 2 of the 15 published trials have failed to show a difference in efficacy between short (3 to 5 days) and standard (7 to 10 days) antibiotic regimens for acute otitis media (AOM). These studies involved relatively few patients under 2 years of age, who are at a higher risk for treatment failure. METHODS: In a prospective, comparative, double-blind, randomized, multicenter trial, we compared amoxicillin/clavulanate in 3 divided doses for 10 days with an identical 5-day regimen, followed by a 5-day placebo period. RESULTS: Between February 1995 and May 1996, 385 children (mean age, 13.3 months) were enrolled, 194 in the 5-day treatment group and 191 in the 10-day treatment group. In the per protocol analysis, clinical success was obtained on days 12 to 14 after the beginning of treatment (main analysis) in 125 (76.7%) of the 163 children receiving the 5-day regimen and 148 (88.1%) of the 168 receiving the 10-day regimen (P = .006). Clinical success persisted on days 28 to 42 among 57 (40.4%) of the 141 assessable patients in the 5-day group and 64 (46%) of the 139 assessable patients in the 10-day group. (P = .34). Multivariate analysis showed that the 10-day course was statistically superior only among children cared for outside their homes (86.8% vs 70.8%; P = .008). CONCLUSIONS: When assessed on days 12 to 14 after the outset of treatment, a 5-day regimen is not equivalent to a 10-day regimen among young children with AOM.

Aberrant crypt foci in human colons: distribution and histomorphologic characteristics.

Aberrant crypt foci (ACF) are one of the earliest putative preneoplastic, and in some cases, neoplastic lesions in human colons. These microscopic lesions, identified on methylene blue-stained mucosa with a low-power-magnification microscope, are thought to be closely related to the earliest steps in multistage colonic tumorigenesis. We investigated the distribution pattern and histomorphological features of ACF in 74 patients with sporadic colorectal cancer. The distribution pattern shows a slightly higher prevalence with older age. The prevalence of the ACF in sigmoid colon was significantly higher in patients with colorectal cancer as compared with patients with benign colonic diseases. Also, significantly more ACF were detected in distal parts of the large bowel (descending, sigmoid colon, and rectum) than in proximal parts. Of 42 microdissected lesions, 12 were dysplastic and 30 were hyperplastic foci. The average size of dysplastic lesions was significantly larger than hyperplastic foci. More apoptotic bodies were found in dysplastic lesions. These lesions also showed an upward expansion of proliferative compartment and higher proliferation indices expressed as proliferating cell nuclear antigen-labeling index. Lymphoid follicles were frequently observed in the base of both hyperplastic and dysplastic foci (40% and 66.6%, respectively). The coincidence of lymphoid follicles was 2.5 to 8 times higher than expected. These features may be related to further progression of selected ACF during colorectal tumorigenesis.

A multicenter, placebo-controlled study of twice daily triamcinolone acetonide (800 microg per day) for the treatment of patients with mild-to-moderate asthma.

BACKGROUND: National and international guidelines recommend the use of inhaled antiinflammatory medications in patients with all but the mildest forms of asthma. Twice daily dosing may increase compliance with therapy. OBJECTIVE: We sought to evaluate the safety and efficacy of 400 microg twice daily triamcinolone acetonide (TAA) compared with placebo in adult patients with mild-to-moderate asthma who were poorly controlled by beta2-agonist therapy. METHODS: We performed a multicenter, randomized, double-blind, placebo-controlled study, including a screening visit, a 7- to 21-day pretreatment baseline phase, and a 6-week double-blind treatment phase. Efficacy was measured by weekly spirometry and daily diary recordings of peak flow rates, asthma symptom scores, and albuterol use. Eligible patients used albuterol four or more times per day, had total asthma symptom scores of 15 or greater (possible total, 60) over 5 of 7 baseline days, and had FEV1 measurements of 60% of predicted value or greater. RESULTS: One hundred twenty-one patients were randomized to treatment. TAA was superior to placebo for all efficacy measures, with significant improvements in asthma symptoms, albuterol use, morning and evening peak flow rates, and forced vital capacity evident at Treatment Week 1. Significant improvements in other pulmonary function measurements were observed after 2 or more weeks. All efficacy variables improved progressively throughout the study. CONCLUSIONS: Twice daily TAA (400 microg) decreased asthma symptoms and improved lung function in patients with mild-to-moderate asthma compared with placebo. Therapeutic benefit was evident within 1 week and increased throughout treatment.

Treatment of severe, drug resistant obsessive compulsive disorder with the 5HT1D agonist sumatriptan.

The serotonergic system has been implicated in both the aetiology and pharmacological treatment of obsessive compulsive disorder. In pharmacological challenge tests, mCPP, a 5-HT agonist, with an affinity for 5HT2C as well as 5HT1A and 5HT1D receptors, was associated with a transient exacerbation of obsessive compulsive symptoms. Chronic administration of mCPP was found to bring about some relief of these symptoms. Sumatriptan is an antimigraine agent, which interacts most potently with 5HT1D receptors. In the cases to be presented, we report the effects of chronic administration of Sumatriptan to three severe, treatment resistant, OCD patients. Following chronic administration of sumatriptan, these patients, who have been resistant to any former pharmacological treatment, responded with an improvement in their depression and a modest reduction in their obsessive compulsive symptoms.

[Urinary tract infection in ambulatory pediatrics].

A survey by questionnaire on urinary tract infection (UTI) in children was conducted over a 1-year period among paediatrician practitioners (April 1997-March 1998). The aim was to provide epidemiological data and to describe the current therapeutic attitude on UTI in children in paediatrician practice. The preliminary results will be presented during the meeting on UTI in children.

[Plea for an evaluation of beta hemolytic streptococcal tonsillitis: from diagnosis to treatment].

There is no justification to maintain a systematic antibiotic treatment of all tonsillitis as currently practiced in France. Indeed, it is today possible to exclude the streptococcal origin of tonsillitis with a fair probability using a rapid diagnosis test. It is therefore mandatory that these tests are made available to the French practitioner. Additionally, alternatives to the classical 10-day penicillin V treatment of streptococcal tonsillitis must be seriously considered: shorter duration treatment with oral cephalosporins, macrolides or amoxicillin have proved to be as efficient and of lower cost; they are also probably safer due to a better compliance related to the shorter duration.

Obstructive sleep apnea: oral appliance therapy and severity of condition.

OBJECTIVE: The purpose of this study was to determine whether an oral appliance can effectively treat severe obstructive sleep apnea. DESIGN: The study was conducted at a tertiary care military facility with an accredited sleep laboratory. Results of the treatment of the first 25 patients with obstructive sleep apnea referred for oral appliance therapy were retrospectively analyzed. Each patient received a mandibular advancement appliance and underwent polysomnography 2 weeks after delivery of the appliance. Patients were divided into two groups: those with slight-to-mild obstructive sleep apnea who had a respiratory disturbance index less than 21, and those with more severe disease. Treatment was considered to be successful if the posttreatment respiratory disturbance index was less than 5. RESULTS: Nine (90%) of the 10 patients with slight-to-moderate disease were successfully managed with the oral appliance. Of the 15 patients in the moderate-to-severe group, 9 (60%) were successfully managed. CONCLUSION: Oral appliances have commonly been recommended only for mild obstructive sleep apnea. This study indicates that they may also have a role to play in selected cases in which the condition is more severe. There is a paucity of information about long-term success. This short-term (2-week) study should be followed by others evaluating the effect over longer periods.

Manual and saccadic reaction time with constant and variable preparatory intervals in schizophrenia.

Saccadic reaction time (RT) has been shown to be unimpaired in schizophrenia. Could this be due to its not requiring controlled information processing? The authors gave 49 schizophrenia patients and 34 controls manual and saccadic RT tasks with preparatory intervals of 1, 3, and 5 s given in regular and irregular sequences. If saccades require mainly automatic processes, they should not be affected by variations in the preparatory interval that are mediated by controlled processing. The manual task showed typical slower RT and larger preparatory interval effects in patients than in controls. Although the saccadic task showed significant effects of both the preparatory interval and the preparatory interval on the preceding trial similar in kind to those in manual RT, there were no group differences in these or in RT. The results are attributed to greater stimulus-response compatibility in the saccadic task, which puts fewer demands on working memory.

The naming deficit in early Alzheimer's and vascular dementia.

Patients with early Alzheimer's disease (AD) were compared to normal controls and patients with early vascular dementia (VaD) on their naming errors using the Boston Naming Test (H. Goodglass & E. Kaplan, 1983). All naming errors were classified into three general error categories: visuoperceptual, semantic, and phonemic. Semantic errors were further classified into coordinate errors (responses that belong to the same semantic category as the target words), superordinate errors (responses that belong to a broader semantic category than the target word), and functional-circumlocutory errors (circumlocutions and responses that functionally describe the target word). The findings indicated that AD participants display more overall naming errors than VaD participants, although the pattern of general errors was similar between the patient groups. However, the qualitative difference between the patient groups was observed within the semantic errors because the AD group made more superordinate errors.
