Cost of treatment of bleeding esophageal varices.

We examined the cost of four methods of treatment of bleeding esophageal varices--medical treatment, sclerotherapy, variceal ligation operations, and portal systemic shunts--in 49 consecutive patients from 1977 to 1979, and correlated the two-year outcome with cost. We found that, despite bias imposed by selection, the cost per patient and cost per survivor at two years was lowest in patients who received sclerotherapy, even though they were more seriously ill than patients who received other treatments. Patients treated with sclerotherapy also had the lowest mortality during primary hospitalization and the lowest readmission rate during a two-year period.

Diet-related toxemia in pregnancy. I. Fat, fatty acids, and cholesterol.

Toxemia in pregnancy (preeclampsia)is characteristerized by a combination of at least two of the following clinical symptoms: hypertension, edema, and proteinuria. In three successive trials over three consecutive years, the dietary intake of a selected number of young pregnant women attending a Maternal and Infant Care Program at Tuskegee Institute were evaluated for total lipids, individual fatty acids, and cholesterol. Women with toxemia or with any of the individual symptoms were identified and women without toxemia or these symptoms served as controls. Results were variable from repetition to repetition in all but the toxemia group and the edema group. The consumption of total lipids and cholesterol was significantly greater in all three trials by both the toxemia and edema groups. Also, total saturated, monounsaturated, and polyunsaturated fatty acids were eaten in greater amounts. The greatest differences were in palmitic acid, stearic acid, oleic acid, and linoleic acid. The proportion of unsaturated fatty acids consumed in all groups was very low. All differences could be attributed primarily to breakfast and dinner meals and were found in the milk, meat, and egg food groups. Although satistical correlations were found between lipid intake and toxemia of pregnancy any specific relationship between the two is still unclear.

Rapid closure of defects in serosal membranes.

The use of surgical clips (Hemoclips) for closure of defects in omentum and mesentery is an efficient and safe alternative to sutures. It avoids the problem of hematoma formation from inadvertent injury of vessels by the needle.

Role of protein binding of cytotoxic anions in post-traumatic pulmonary insufficiency.

Anions of fatty acids infused intravenously into dogs clearly duplicate the clinical syndrome of post-traumatic pulmonary insufficiency (congestive atelectasis, shock lung, traumatic wet lung). Neutral fat embolized to the lungs is incapable of producing this syndrome within the time investigated. The incubation of fatty acid anions with human serum albumin before injection obviates the severe desaturation of blood and ultrastructure disruption caused by FAA alone.
