[Treatment of advanced prostatic cancer with a gonadorelin agonist, dTrp6 LHRH. 41 cases].

Forty-one patients with prostatic cancer were treated with DTrp6 LH-RH, a luteinizing hormone releasing hormone agonist. Thirty-eight of them had advanced cancer with either metastases or major local spread. In all cases with plasma testosterone levels were normal before treatment, the LH-RH agonist brought them down to values observed after surgical castration. An objective response ("improvement" or "stabilization") was obtained in 68% of the patients, but virtually all "improved" patients were those without previous hormonal treatment. As with all LH-RH agonists, the pharmacological castration produced by DTrp6 LH-RH is of considerable value in the treatment of prostatic cancer, especially since this compound, unlike estrogens, does not seem to cause cardiovascular accidents. However, transient hyperandrogenism may occur during the first days of treatment (as it did in 9 of our patients) and may be responsable for an acute flare-up of the tumoral process, as suggested by 2 of our cases. The concomitant administration of an anti-androgenic drug should perhaps be considered.

[Treatment of prostatic cancer with an LH-RH agonist: the D Trp6 LH-RH. Preliminary results in 30 cases].

Thirty patients with advanced prostatic cancers (8 stage C, and 22 stage D) were treated for at least three months with D Trp6 LH-RH, a powerful LH-RH agonist. In all the cases in which the testosterone levels had been normal before treatment, the treatment led to a decline to the levels observed after castration. In seven cases, the drop was preceded by a rise in the first few days of treatment. Twenty two patients reacted favorably to the "medical castration" resulting from LH-RH agonists (11 improved and 11 remained stable). The improvements were mainly noted in patients who had not undergone previous treatment. Four deaths were registered in patients already at an advanced stage. Two of these patients presented with visceral metastases in circumstances such that a sudden deterioration due to the agonists cannot be excluded.

[Urogenital complications of abdominoperineal resection of the rectum for cancer].

While operative traumas of the excretory tract are rare, mictional and sexual sequelae often complicate rectal resections for cancer. Although relatively rare, mictional disorders, which mainly affect males, are as often neurogenic as obstructive, and are, in that event, frequently due to iatrogenic urethral stenoses. The neurogenic disorders require an extremely sensitive semiological analysis before contemplating the slightest procedure on the prostate, which may provoke definitive urinary incontinence. Retrograde ejaculation and impotence are frequent, and the latter can be attenuated by a penial prosthesis.

[Acid phosphatases].

The authors discuss the pathological value of acid phosphatase measurement. One of its main applications is the diagnosis of prostatic cancer. The emergence of immunological methods of measuring the enzyme, which are more sensitive and more specific, has led to renewed interest as acid phosphatase measurement can now be used in the early diagnosis of prostatic cancer, or at least in the early diagnosis of its extraprostatic extensions.

[A new therapeutic approach to urogenital tuberculosis].

The authors comment on the new treatment methods used in cases of urogenital tuberculosis. The progress made has led to a reduction in the duration of medical treatment. The introduction of rifampicine, and the combination of medical treatment with transurethral techniques, have to a great extent reduced surgery and consequently morbidity from urogenital tuberculosis.

The challenging vesicovaginal fistula.

The authors report herein their experience in treating 75 cases of vesicovaginal fistulas. 80% were caused by gynecologic surgery and 20% were of obstetrical origin. The authors favor the transvaginal approach and describe in detail the two technics they recommend. The most commonly used method is the Chassar-Moir procedure. 70% of the patients were cured after one attempt with this procedure, and 93% after two operations. In complicated fistulas (irradiated or extended to the urethra) the Martius procedure was used. In this technic a flap formed from the fat tissue of one of the labia majora is interposed between bladder and vagina. 10 of 11 patients so treated were cured.

Upper tract urothelial tumors. Diagnostic efficiency of radiology and urinary cytology.

86 patients presenting with upper tract urothelial tumor were investigated by conventional X-ray techniques and urinary cytology. Pyelography, intravenous or retrograde, was diagnostic in 85% of the cases and urinary cytology in 43%. Pyelography can stage correctly 80% of the infiltrating tumors, and urinary cytology 66% of the high grade lesions. Altogether 97% of the high grade infiltrating tumors can be accurately staged preoperatively on the basis of pyelography and urinary cytology. In the future retrograde brush biopsy and CAT scanning should optimize these figures.
