[Laryngotracheal stenosis treated by the Rethi operation : anatomical and functional (respiratory and vocal) results (author's transl].

On the basis of 25 cases of laryngotracheal stenosis of mescellaneous etiology, the authors report their experience of Rethi's operation carried out in 14 cases, 8 patients having received medical and instrumental treatment, 1 undergoing surgery for resection and anastomosis according to Pearson's technique and 3 receiving symptomatic treatment by tracheotomy because of inoperable stenosis. The frequency of postop-operative complications of Rethi's operation was analysed (essentially infectious, but also as a result of obstruction of the calibration prosthesis which represents a definite vial risk). Functional results were also assessed, being relatively satisfactory from a respiratory functional standpoint, but less good from a phonetic standpoint. The authors then define their present therapeutic attidude which leans, in the case of pure sub-glottal and sub-glotto-tracheal stenosis, towards a direct approach with resection anatomosis according to Pearson's technique rather than calibration procedures which remain the treatment ofa choice for glotto-sub-glottal stenosis.

[Deafness due to renal failure. Clinicopathological study (author's transl)].

In this series of 54 cases of chronic renal failure, the largest number of curves showed a curve of a presbyacousia type (3/4 of the total). This hypoacousia may be related to the length of time for which the renal pathology has existed and, parat from group 0 (patients treated without haemodialysis nor grafting), may be correlated with the curve of the speed of nervous conduction. This latter correlation is particularly close in patients with a strictly isolated renal disease. The aetiology of this deafness would appear to be related to the premature aging provoked by chronic renal failure. The method of surface preparation applied to 5 petrous temporal bones from 3 patients was felt to be of particular value in the study of this pathology. Whilst one case of unilateral sudden deafness of vascular aetiology is reported, no vascular changes were seen in the other petrous temporal bones. Case 7-11 was particularly demonstrative since bilateral deafness with a rapidly progressive descending occurred in the presence of changes in the pre-ganglionic cochlear fibres charaterised by demyelinisation. This was associated with a cell loss of approximately 25% in the spiral ganglion and less marked demyelinisation of the fibres of the vestibular nerve.

[Facial nerve tumors: three cases (author's transl)].

The first one is a neurofibroma associated with a Charcot-Marie Tooth's disease involving all the tympanic segment of the facial nerve: extirpation and graft. Improvement of facial motricity to 42%. The second one is a small Schwannoma lateral to tympanic segment: extirpation without paralysis. The third is a large Schawannoma of the tympanic mastoïd segments involving the facial nerve to its division in the parotid gland, and with a limited extension in the cerebello-pontine angle. The place of cryosurgery in the treatment of subglottic angiomas of the infant.

The treatment of subglottic angiomas of the infant is not clear since radiotherapy was rejected. Tracheotomy is very often indicated. Spontaneous recovery of angioma may be hoped for, but it is slow. Cryotherapy with liquid nitrogen using a 2.5 mm cryoprobe has cured, in a few stages, 3 cases. It has to be employed carefully.
