Production of 7.6-minute potassium-38 for medical use.

A method is described for generating 20--30 mCi of 7.6-min potassium-38 by means of a small cyclotron. Sodium chloride is mounted on a water-cooled tantalum plate, by evaporation from an aqueous solution. It is bombarded with 14.7 MeV helium-4 ions, at 50 microA. The K-38 is produced free of other radionuclides. For intravenous injection the bombarded NaCl is dissolved in sufficient pyrogen-free water to make an isotonic saline solution, which then is sterilized by filtration. Other methods of production investigated were the bombardment of: carbon tetrachloride with He-4 ions; calcium oxide with 7.8-MeV deuterons; and potassium chloride with 23-MeV He-3 ions. These gave products that were unsuitable for clinical applications. Chiefly because of the short half-life of K-38, the whole-body radiation exposure is estimated to be only about 12 mrad/mCi, and exposures to the heart and kidneys are approximately ten times greater.

Malakoplakia of the brain. A report of two cases occurring in childhood.

Malakoplakia was found in the brain biopsy specimens of two children with meningoencephalitis probably due to herpes simplex. Histologically, there was extensive parenchymal destruction and cellular infiltration. Numerous von Hansemann histiocytes and typical Michaelis-Gutmann bodies were present. Ultrastructurally these bodies had a dense central core and concentric alternating rings. Large intracytoplasmic phagolysosomes were present. Malakoplakia most frequently involves the urinary tract of adults. Only a few children with malakoplakia have been described. There are two previous reports of cerebral malakoplakia associated with neonatal herpes simplex. The pathogenesis is obscure. Several reports suggest an altered histiocytic response to a bacterial agent as the pathogenetic mechanism. Our findings raise the possibility of an etiologic role of a virus, such as herpes simplex.

Liver/pleural cavity scan for diagnosis of subphrenic abscess.

The interpretation of a combined liver/lung scan can be difficult in patients with pleural or pulmonary disease. A procedure is suggested to circumvent these difficulties by injecting 99mTc-sulfur colloid into the pleural cavity. A scan of the pleural cavity can then replace the lung scan in the conventional liver/lung scan. Two examples of liver/pleural cavity scans are shown.

111In Desferal: a new radiopharmaceutical for abscess detection.

Desferal was labeled with 111In and its distribution was studied in normal mice, rabbits bearing turpentine-induced abscesses, and rats bearing Staphylococcus aureus-induced abscesses. The results indicate that 111In Desferal may be used in abscess detection and localization.
