Upper vs lower extremity functional loss in neuromuscular disease.

Functional rating scales have been developed for evaluation of upper extremity (UE) and lower extremity (LE) function in Duchenne muscular dystrophy (DMD). The Vignos scale is accepted for LE function and the Brooke scale for UE evaluation. The assumption that UE functional loss parallels LE loss has never been tested. Furthermore, there are no studies examining the relationship between UE/LE strength. This study tests the hypothesis that UE/LE strength and functional losses are parallel in DMD and other neuromuscular disorders. Vignos grades, Brooke grades, and manual muscle test scores were determined for 27 patients with DMD, 10 patients with fascioscapulohumeral muscular dystrophy (FSH), 24 patients with myotonic muscular dystrophy (MMD) and 29 patients with other (Becker's dystrophy and limb-girdle syndrome) proximal myopathies (PM). Spearman correlation coefficients were calculated for UE vs LE functional grade and strength scores and subjected to significance analysis. While all correlations were statistically significantly different from zero (p less than 0.01), all were also significantly different from one (p less than 0.01), none representing perfect equivalence. Coefficients of determination indicated that a maximum of 86% (FSH) of variation in UE rank could be explained purely by observing LE rank. While there is a significant relationship between UE/LE strength and functional grade, these measurements were not found to be entirely equivalent. Therefore, while measures of lower body function or strength might be used as a substitute for UE measurements in drug trials in which global effects are expected, the two measurements are not equivalent when evaluating an individual's clinical status.

Regulation of prostaglandin biosynthesis by human ovarian follicular fluid: a mechanism for ovulation?

We have studied the effect of human ovarian follicular fluid on PG production by bovine seminal vesicles in vitro and found that hFF1 contains a factor of high molecular weight (Mr greater than 30,000) which inhibits PG synthase in a dose-dependent manner. Exposure of this substance to protease activity produced a factor of lower molecular weight (Mr less than 1000) which stimulated PG synthase activity. If this is true of ovarian follicles in vivo, it is possible that increased follicular protease activity stimulates PG synthesis at the time of ovulation.

Relative levels of thecal blood flow in atretic and non-atretic ovarian follicles of the conscious sheep.

The relative level of blood flow through thecal capillaries (RTF) in individual atretic and non-atretic follicles greater than 1 mm was measured in conscious ewes by injecting 15 micron radioactive microspheres into the aorta near the origin of the ovarian artery, and determining the ratio of radioactivity per mg protein in theca of individual follicles to that in the non-luteal portion (stroma plus follicles less than 1 mm diameter) of the corresponding ovary. In follicles greater than 1 mm diameter, RTF in those showing advanced stages of atresia (stages IV and V) was significantly less than that in non-atretic (stage I) follicles. RTF values in non-atretic (stage I), early atretic (stage II) and moderately atretic (stage III) follicles did not differ significantly. There was no effect of follicular diameter (P greater than 0.05) on the relative level of blood flow within individual thecae or of atresia on thecal protein content. These data indicate that reduced thecal blood flow is associated with advanced atresia of antral follicles and are in accordance with the findings of previous histological studies. Whether thecal ischaemia is a factor involved in the onset of atresia, or occurs as a consequence of the degenerative process, remains uncertain.

Development and steroidogenic activity of preantral follicles in the neonatal rat ovary.

The neonatal rat ovary is completely devoid of antral follicles until the twelfth day of age. During this period the ovary becomes steroidogenically active and responsive to gonadotrophins. The aim of this study was to correlate the onset of ovarian androgen and oestrogen production in vitro with the first appearance of distinct granulosa and theca cells. Although ovarian aromatase activity increased significantly on day 7 of age, ovarian oestrogen production was limited by low progesterone and testosterone production until day 12 of age. Increased aromatase activity on day 7 and androgen production on day 12 were coincident with the first appearance of granulosa and theca cells respectively. These functional and morphological changes were not associated with significant alterations in ovarian weight or concentrations of LH or FSH in serum.

Alternative statistical models for the examination of clinical positron emission tomography/fluorodeoxyglucose data.

This article describes a method for partitioning metabolic variability found in positron emission tomography/[18F]fluorodeoxyglucose studies. For the 15 subjects examined, 74.8% of the total metabolic variability could be ascribed to individual differences in global metabolic rate, whereas 15.8% of the total variability was consistent regional variation or pattern across subjects. Subsequently, the method of Q-component analysis is described for the identification of strong- and weak-pattern subjects. In addition, a standardization procedure that amplifies the observed pattern by removing systematic individual differences is described. Finally, the implications of these findings and methods for future and clinical studies are discussed.

Multiple-radionuclide autoradiography in evaluation of cerebral function.

We have developed the mathematical model and experimental technique for quantitative simultaneous multiple-radionuclide autoradiography. The technique is an extension of previously described dual-tracer methods and offers the advantage of a general approach in which any two or more radionuclides with different half-lives can be used to measure multiple parameters of cerebral function. The model allows the selection of nuclide doses and exposure times to balance cross-contamination of the multiple images with nuclide requirements. The technique was validated with homogeneous tissue sections containing combinations of 123I, 124I , 131I, 18F, 99mTc , 68Ga, and 14C, and then used in qualitative and quantitative investigations of interrelationships of local cerebral blood flow, glucose metabolism, and protein synthesis in normal and several pathological conditions in rats.

EM reconstruction algorithms for emission and transmission tomography.

Two proposed likelihood models for emission and transmission image reconstruction accurately incorporate the Poisson nature of photon counting noise and a number of other relevant physical features. As in most algebraic schemes, the region to be reconstructed is divided into small pixels. For each pixel a concentration or attenuation coefficient must be estimated. In the maximum likelihood approach these parameters are estimated by maximizing the likelihood (probability of the observations). EM algorithms are iterative techniques for finding maximum likelihood estimates. In this paper we discuss the general principles behind all EM algorithms and derive in detail the specific algorithms for emission and transmission tomography. The virtues of the EM algorithms include (a) accurate incorporation of a good physical model, (b) automatic inclusion of non-negativity constraints on all parameters, (c) an excellent measure of the quality of a reconstruction, and (d) global convergence to a single vector of parameter estimates. We discuss the specification of necessary physical features such as source and detector geometries. Actual reconstructions are deferred to a later time.

Identification and differentiation of resting myocardial ischemia and infarction in man with positron computed tomography, 18F-labeled fluorodeoxyglucose and N-13 ammonia.

Studies have shown that the extraction of glucose per unit flow is increased in moderately ischemic myocardium primarily due to anaerobic glucose metabolism manifested as lactate production, whereas myocardial infarction is characterized by the loss of metabolically active myocardium. To determine the feasibility of demonstrating these metabolic abnormalities reflecting both ischemia and infarction, we used positron computed tomography (PCT) to evaluate relative regional myocardial exogenous glucose utilization and perfusion in 15 patients with recent myocardial infarction. The positron-emitting tracers of glucose metabolism and perfusion, 18F-2-fluoro-2-deoxyglucose (FDG) and N-13 ammonia, respectively, were used. Fourteen of 19 documented infarctions were demonstrated by PCT to have concordantly decreased glucose utilization and perfusion. However, in an additional 11 regions, glucose utilization was disproportionately increased relative to perfusion, consistent with ischemic glucose consumption. These findings correlated with the presence of postinfarction angina, the site of ischemic electrocardiographic changes during chest pain, and the presence of regional left ventricular dysfunction and severe coronary artery disease. Because three ECG infarct zones not detected by PCT demonstrated ischemic glucose utilization, only two of 19 electrocardiographically defined infarctions had no detectable metabolic abnormality. We conclude that the changes in regional FDG and N-13 ammonia concentrations detected with PCT in patients who had had a recent myocardial infarction are consistent with regional exogenous glucose utilization and perfusion in moderately ischemic and irreversibly infarcted myocardium. This approach has the potential to identify and differentiate resting myocardial ischemia from infarction and to assess tissue viability after an ischemic event.

An analysis of signal amplification using small detectors in positron emission tomography.

Spatial resolution in computed tomography (CT) has been limited because of the excessive increases in statistical noise associated with increasing resolution. Reconstruction filter functions have been designed to produce tomographic images with resolution as close as possible to the intrinsic detector resolution. Since these filter functions apply an increasing weight to increasing spatial frequencies in the measured data, this produces a large amplification of noise at the upper end of the spatial frequency band of the filter function where the signal from the detector has a low amplitude. This mathematical approach to spatial frequency amplification cannot distinguish between signal and noise and thus both are amplified equally. An approach for improving tomographic resolution is the use of very small detectors to selectively amplify the signal but not noise. This signal amplification technique (SAT) can provide improvements in image resolution without increases in the cutoff frequency of the reconstruction algorithm. Alternatively, in cases in which image counts are low, statistical noise can be reduced by lowering the cutoff frequency. A possible system design for a positron CT system with SAT is described.

Hypothalamic-pituitary function in normal ewes and ewes which grazed oestrogenic subterranean clover for several years.

Plasma luteinizing hormone (LH) concentrations were measure in normal Merino ewes and in Merino ewes with lowered fertility which had resulted from prolonged grazing of Trifolium subterraneum L. cv. Dinninup. During the anoestrous season, LH was measured at frequent intervals before and following administration of oestradiol-17 beta or gonadotropin releasing hormone (GnRH), and around the time of spontaneous oestrus. All ewes responded to GnRH treatment and there were no differences between the two groups in terms of the amount of LH released or the time to reach maximal plasma hormone concentration. Comparable proportions of ewes from both groups responded to oestradiol-17 beta treatment with LH levels showing initially a negative feedback response (i.e. suppression to approximately 80% of pretreatment levels) followed by positive feedback response. During the positive feedback phase, the quantity of LH which was released was comparable in both groups; however, the time interval over which this hormone was secreted was significantly shorter for clover-infertile ewes (P less than 0.05). During the anoestrous season the mean LH level was lower in normal ewes than in clover-infertile ewes [0.45 +/- 0.41 (n = 32) versus 0.82 +/- 0.73 (n = 48 ng/ml, P less than 0.01]. In the oestrous season the mean LH level for normal ewes rose to 1.23 +/- 0.65 ng/ml (n = 76) but there was no change for the clover-infertile ewes (0.77 +/- 0.59 ng/ml, n = 73). These experiments indicate that in the clover-infertile ewe LH concentration remains static throughout the breeding and non-breeding seasons but the results suggest that this altered endocrine status in not due to any differing level of sensitivity to negative feedback effects of oestradiol-17 beta as compared with normal ewes.

Unusual clinical and ultrastructural features in a boy with biochemically typical mannosidosis.

A 4 1/2-year-old boy with a history of recurring respiratory tract infections and seizures, and evidence of severe retardation of psychomotor development and growth, lacked the coarse facial features, skeletal changes and other clinical stigmata generally associated with mannosidosis, but the total alpha-mannosidase activity in his leukocytes, cultured fibroblasts and liver were no more than 10% of the control mean. Studies of the residual alpha-mannosidase enzyme suggest a specific deficiency of the thermostable isoenzyme with an acidic pH optimum. The alpha-mannosidase in the fibroblasts of our and another (control) patient with mannosidosis had a reduced affinity for the substrate 4-methylumbelliferyl-alpha-D-mannoside. Light microscopy of the liver biopsy showed an increase in connective tissue often distorting the hepatic architecture; numerous tiny vacuoles, small dense and lipid bodies in most hepatocytes, and similar but more extensive changes in sinusoidal cells; and sinusoidal pools of hepatocytic debris. Electron microscopy of hepatocytes revealed vacuoles similar but not identical to those described in reported mannosidosis patients, and in addition several forms of secondary lysosomes; prominent peroxisomes (microbodies); increased numbers of profiles of smooth endoplasmic reticulum; dilated rough endoplasmic reticulum containing traces of fine granulo-fibrillar material; increased numbers of rosettes of alpha particles of glycogen and reduced numbers of mitochondria with alterations in their distribution, size and configuration. It is believed that the usual clinical and hepatic ultrastructural features in our patient reflect another variant of mannosidosis.

The Nakasuk Project- the conservative treatment of chronic otitis media in Inuit elementary school children.

Modalities of local medical treatment of chronic otitis media in Inuit elementary school children were evaluated over a period of three and a half years in a project associated with the provision of amplification (hearing aids) for students who had significant conductive hearing deficits which interfered with their ability to communicate. The significant medical and educational benefits derived are reviewed.

Adult adjustment among high school graduates with mild disabilities.

This study investigated the adult adjustment of a statewide random sample of 737 young adults with learning disabilities, 59 labeled behaviorally disordered, and 142 labeled mentally disabled, all graduates of special education resource teacher programs. Results are reported in terms of (a) general status information, such as marital status and living arrangements; (b) information about those competitively employed, such as wages, hours worked per week, and percentage of living expenses paid; and (c) comparison of competitively employed versus unemployed individuals, in terms of high school vocational training and work experiences. Information is also provided on postsecondary education and overall "successful" adult adjustment. Data are compared across disability groups and across gender, where relevant.
