Chondroblastoma. A review of seventy cases.

We reviewed the cases of seventy patients with chondroblastoma who were treated at the Istituto Ortopedico Rizzoli between 1949 and 1983 and found that the proximal end of the humerus was the most common location (eighteen), with the proximal end of the femur (fifteen), distal end of the femur (fifteen), and proximal end of the tibia (twelve) being the other frequently involved sites. Sixty-three of the patients were between eleven and thirty years old. Fifty-eight patients sought medical attention because of an aching pain, usually referred to the adjacent joint. Fifty patients were followed for two years or longer after treatment. Seven patients had a local recurrence: four were successfully treated with a repeat curettage and one, by two subsequent marginal excisions; one was advised to have a repeat curettage; and the seventh was advised to have a resection and arthrodesis of the knee. The final functional results were considered to be excellent in forty-seven of the fifty patients who were followed for two years or more.

Extra-articular resection of the shoulder (Tikhoff-Linberg).

The authors have reviewed 21 cases of extra-articular resection of the shoulder, using the Tikhoff-Linberg technique, carried out at the Tumour Centre of the Rizzoli Institute. The effectiveness of this method has been evaluated both oncologically (by a study of the resected specimens) and functionally. The possible complications are considered. The authors describe a modification of the technique for tumours of the proximal humerus in which only part of the scapula is removed.

Phenol as an adjuvant in the control of local recurrence of benign neoplasms of bone treated by curettage.

Throughout the present century phenol has been used to "sterilise" the edges of cavities left in bone after the clearance of benign tumours in order to reduce the risk of local recurrence. The results have been analysed of 165 benign bone tumours capable of local recurrence (aneurysmal bone cyst, chondroblastoma, giant cell tumour) treated with and without the use of phenol. The overall recurrence rate was 41% in 98 cases treated by simple curettage and 7% in 27 cases treated by curettage followed by the application of phenol.

[Resection with preservation of the lower limb in chondrosarcoma of the pelvis].

Twenty-seven chondrosarcomata of the pelvis were treated by local resection with preservation of the limb. Twenty-two were grade I or II, and 5 were grade III or IV. Attention is drawn to the importance of tomodensitometry in assessing the extent of the tumor before operation. An en bloc resection was well wide of the tumour in 16 cases, close to the tumour in 10 cases and involved the tumour in one case. Post-operative complications were varied. There were 6 nerve paralyses after iliac resection, 3 cases of sepsis after resection of the anterior part of the pelvic ring, and 3 cases of sepsis and 2 vascular complications after peri-acetabular resections. The average follow-up was 5 years. Three had metastases and three had local recurrences, one of whom died later from pulmonary metastases. No local recuRrence was seen in cases with wide resection. The functional results were satisfactory in 6 out of 7 partial resections of the ilium and in 4 out of 5 resections of the anterior part of the pelvic ring. They were only fair after peri-acetabular resections and complete resections of the ilium. After peri-acetabular resections, the results were about the same after ilio-femoral arthrodesis or Girdlestone operations. It is not appropriate to reconstruct the pelvic ring after resection limited to the anterior part of the pelvis but it is indicated after resections of the whole of the ilium, particularly in children to avoid shortening of the limb with pelvic shift.

Malignant fibrous histiocytoma of bone. The experience at the Rizzoli Institute: report of 90 cases.

Ninety cases of malignant fibrous histiocytoma (MFH) of bone are reported. An analysis is presented with regard to sex, age, site, symptoms, radiography, macroscopic, and microscopic features. The staging was assessed according to the following classification: I B: 3 cases; II A: 11 cases; II B: 66 cases; III: 10 cases. Among nonmetastatic patients, 68 had surgical treatment (adequate, 46; inadequate, 22); 8 had radiotherapy, and 3 refused any treatment. Among surgically treated patients, 20 had adjuvant chemotherapy, also. Follow-up information was obtained in all cases. Overall survival rates were 34% at 5 years, and 28% at 10 years. Surgery alone did not appear to be successful (5-year survival, 28%). Adjuvant chemotherapy improved the survival rate only in patients who underwent adequate surgery (5-year survival, 57%). The recurrence rate after surgical treatment was high (31%) with significant differences after inadequate (64%), wide (19%), or radical (6.5%) surgical treatment. Adjuvant chemotherapy was ineffective in preventing local recurrence. Radiation therapy effected a clinical cure in three cases; the authors believe that radiation therapy has to be used only in inoperable tumors.

Contrast examination as a prognostic factor in the treatment of solitary bone cyst by cortisone injection.

Local injection of radiopaque medium demonstrated the presence of intracystic fibrous septa in 13 patients with solitary bone cyst. Contrast examination was helpful in predicting the response of solitary bone cysts to treatment by injection of methylprednisolone-acetate (MPA). As the number of septa increased, an increased difficulty in obtaining an equal distribution of MPA inside the cyst and a higher incidence of incomplete healing of the cyst was encountered.

Primary central (medullary) fibrosarcoma of bone.

Eighty cases of primary fibrosarcoma of bone from the Rizzoli Institute are reviewed. There was a slight male predominance with a wide patient age distribution. Roentgenologically, low grade fibrosarcomas had generally well-defined margins and a "soap-bubble" appearance. High grade tumors appeared permeative and more aggressive. Histologically, the majority of tumors were high grade (3 and 4). Prognosis correlated well with the grade of the tumor. The 10-year survival rate was 83% for low grade sarcomas and 34% for high grade tumors. Local recurrence was a bad prognostic sign.

Mesenchymal chondrosarcoma of bone and soft tissues.

Mesenchymal chondrosarcoma of bone and soft tissues treated at the Istituto Ortopedico Rizzoli are reviewed. The skeletal locations were prominent in five cases. Only two cases were in the soft tissues. Radiographic picture in the bone shows an aggressive osteolysis, with soft tissues invasion. Histologic picture is the same in bone and soft tissues, and is highly distinctive: islands of well-differentiated chondrosarcoma embedded in undifferentiated mesenchymal cells with high malignancy characteristics. The cases showed a poor prognosis, and no patient survived more than four years.

The treatment of pulmonary metastases in osteosarcoma. Preliminary observations on 30 cases.

Thirty cases of pulmonary metastases in osteosarcoma are analysed from the point of view of evaluating the indications for localised excision, and also to report the results obtained. This group is compared with 42 analogous cases in which the metastases were not treated surgically. The results of pulmonary surgery is shown to be effective in the multidisciplinary treatment of osteosarcoma with pulmonary metastases.

[A very frequent radiologic sign in dorsolumbar localization of chordoma: increase in the interpeduncular distance].

The X-ray picture of 11 cases of chordoma of the thoraco-lumbar spine at the Tumors Centre of the Rizzoli Orthopaedic Institute is studied. In 7 cases an increase in the interpeduncular distance of the affected vertebrae was noticed. The high incidence of this radiologic sign, never reported previously in the literature, represents a semeiologic element of great importance in the diagnosis of this neoplasm. The ways of measurement are then indicated and the significance of this report is stressed.

Posterior hemiresection of the distal femur in parosteal osteosarcoma.

The writers describe the technique of posterior hemiresection in parosteal osteosarcoma of the distal femur by means of two incisions, medial and lateral. In not previously operated cases, this operation permits wide excision and offers a good guarantee of cure. The stability and mobility of the knee remain intact. The strength of the remaining bone can be increased by autoplastic and homoplastic bone grafts.

The natural history of unicameral bone cyst after steroid injection.

Ninety-five consecutive cases of unicameral bone cyst treated at the Istituto Ortopedico Rizzoli by intracystic injections of methylprednisolone acetate were retrospectively reviewed. Eighty per cent of the patients had a satisfactory result. Of the 20% failures, 13.5% had a recurrence of the cyst. Recurrence after methylprednisolone acetate treatment is a previously unrecognized entity, with recurrence risk factors associated with multiloculation, cyst size and age of the patient.

Synovial chondromatosis and chondrosarcoma of the hip: indications for surgical treatment.

Five cases of cartilaginous neoproliferation of the hip, of synovial origin, were treated by different surgical techniques. From the clinical point of view these cases can be classified as simple synovial chondromatosis and progressive synovial chondromatosis. The writers, on the basis of their own experience and on data collected from the literature, conclude that simple synovial chondromatosis should be treated by partial or total synovectomy, while progressive synovial chondromatosis should be treated by total synovectomy with or without arthroplasty with total hip prosthesis. Synovial chondrosarcoma should be considered a neoplasm with at least local malignancy and should be treated by wide surgery.

Use of the Swanson prosthesis in Kienböck's disease.

The authors report twenty-one cases in which the Swanson prosthesis was used in the treatment of Kienböck's disease. Eighteen of the patients were followed up after between six months and seven years. The results were good in 94.5 per cent of cases. The operation resulted in all cases in the disappearance or reduction of pain. In many instances it also improved the mobility of the wrist and the strength of the muscles. The prosthesis was found to be well tolerated with time. The results were also good in the advanced stages of the disease.

Osteogenesis imperfecta: results obtained with the Sofield method of surgical treatment.

The authors report the results obtained in thirteen patients with osteogenesis imperfecta who were treated by the Sofield operation. This operation is indicated both in the congenital and delayed forms of the disease. The results are promising: prevention of deformities and fractures in a high percentage of cases, recovery of the ability to walk in all the patients. The incidence of complications was low, infection being the most serious. Although the use of extending rods reduces the number of reinterventions, it does not appear to improve the results and carries the risk of a greater number of complications. However, the authors have no experience of this particular technique.

Results in the late treatment of congenital dislocation of the hip.

The results obtained in the treatment of seventeen cases of long established (more than six years) congenital dislocation of the hip are reported. They were followed up for one to five years after operation. The writers consider osteotomy to correct leg length discrepancy to be unnecessary. Triradial osteotomy of the pelvis ensures optimal epiphyseal coverage and permits moderate lengthening of the limb. In the writers' experience, osteochondrosis is not related to the type of osteotomy, nor to operations to produce leg lengthening; it can be avoided by the use of prolonged post-operative traction.

Endoprosthesis of the humerus: description of a new model and its application.

A new type of endoprosthesis of the humerus is presented. It is indicated in resection of the proximal humerus for primary and secondary neoplasms. After describing its construction and the technique for fitting it, the writers analyse the clinical results in thirteen cases. They emphasize the advantages over prostheses of other kinds: simplicity of fitting, wide possibilities of adaptation to individual cases, and the very satisfactory aesthetic and functional aspect.
