[Physical therapy in inflammatory rheumatic diseases].

The physiotherapy of inflammatory rheumatic diseases is interpreted at the instance of the clinical pictures of rheumatoid arthritis, ankylosing spondylitis and progressive sclerodermia. Issuing from the clinical picture in each case the aim of therapy and the application of physiotherapy are described. For practice it is particularly referred to the significance of the temporary phases of the disease with regard to therapeutic purpose and planning of physiotherapy.

[Characteristics of physical therapy in internal medicine].

Internal physiotherapy is interpreted as supplementary level of performance (clinical feature--aim of therapy--means of physiotherapy) both of the internal specialist and the physiotherapeutist. It is particularly referred to the commonness of the responsibility for the aim of therapy. The importance of the phase of the disease when the plan of therapy is established is described.

[Indications for and results of postoperative radiotherapy of vulvar cancer].

Between 1968 and 1979, 132 patients with squamous cell carcinomas of the vulva were submitted to a more or less radical operation. Then they were irradiated at the Radiologic Center of the GHS Essen. All patients received irradiations of the vulvar region with high energy electrons combined with photons (n = 78) as well as photons or electrons alone (n = 15 and 39, respectively). The dose was generally 40 to 60 Gy. 81 patients (61.4%) were additionally irradiated in the inguinal region. The overall three-year and five-year survival rate is 48.5 and 34.1%, respectively. The median survival time is 31.5 months. Contrary to expectation there were no significantly different recurrence rates after radical vulvectomy and large excision combined with postoperative irradiation. This is attributed to the favorable effect of radiotherapy. A graduated conception has been elaborated based on the results of our retrospective analysis and the communications of literature. Radical vulvectomy and inguinal lymphadenectomy is the therapy of choice. Radiotherapy in the vulvar region should be applied if radical surgery seems problematic or is not possible. Irradiation of lymph node regions is indicated if they are involved or if a resection is not possible.

[Treatment results in malignant tumors of the vulva, a retrospective analysis of 119 cases].

Between 1960 and 1980 111 patients with malignomas of the vulva were treated. Among these 86 cases were admitted primarily with view of treatment. Within the period conception of therapy was changed drastically. Where as in former time treatment consists in excision of the tumour with following conventional X-ray therapy, since 1973 therapy of choice was radical vulvectomy with inguinal lymphonodectomy. Based on these facts a retrospective comparison of the survival rates in possible, particularly distribution of stages is likely the same in the two groups. The group treated since 1973 has essential better survival rates. This can be attributed to the conception of radical operative treatment preferred in literature predominantly. Mortality rate is below 5 per cent which will be favourable in the geriatric clientele. To make the results of treatment comparable internationally a postoperative classification should be proposed like in breast cancer. The question of the position of an additional postoperative irradiation is to be answered only with aid of a prospective multicenter study.

[ROC-analysis in testing and training the interpretation of radiographs with mammography serving as an example (author's transl)].

ROC (receiver operating characteristics) curves taken from the results obtained in evaluating the performance of ten different viewers with different experience in the interpretation of 120 xeromammograms and 120 conventional film mammograms each, serve as a basis to discuss the value of ROC-analysis in evaluating and improving the interpretation skill of individuals reading radiographs.

Surgical ablation of multiple leiomyomas.

A case is described of numerous leiomyomas on the lower part of a leg that were removed en bloc where in conglomeration or individually removed by punch excisions followed by filling of the defects with punch transplants where they were disparate.

[Efficiency of postoperative electron therapy for localized cancer of the vulva (author's transl)].

The results from 198 own patients with squamous cell carcinoma of the vulva, having undergone surgical treatment and postoperative irradiation with high-energy electrons and photons, are represented and analysed. The own results are compared to results from the literature obtained by means of larger collectives and of various techniques. A differentiated concept for therapy is presented, taking into account the prognostically important parameters, and integrating surgical and radiation treatment.
