Alcoholism--the second great imitator. An introduction to the problem of alcoholism.

The problem of alcohol abuse and alcoholism pervades our society. The economic losses run into billions of dollars annually. What is more important is the number of fatalities associated with the problem and the effect of alcohol on decision-making processes within all areas of government and nongovernment. The pathologist must understand the fundamental diagnostic processes, recognize the anatomic and clinical findings, and treat alcoholism as a medical problem.

Sudden unexpected death from cardiac concussion (commotio cordis) with unusual legal complications.

Sudden and unexpected death may result from cardiac concussion following blunt force trauma to the thorax. Undiagnosed pathologic disease must be carefully evaluated as a possible contributory element. Legal complications may arise from any autopsy. It is recommended that a photograph be taken upon completion of the autopsy. This photograph and adequate records can be used to refute any charges against the pathologist or assistants for the poor condition of a body after its release.

The medicolegal investigation: evaluation of cases with potential medical litigation.

A medicolegal autopsy protocol must serve as something more than an esoteric scientific document. It will be read by all persons interested in knowing how and of what the person died, and these readers might include family members, insurance representatives, police officials, attorneys, judges, and juries.... The writing of a medicolegal autopsy protocol is a difficult task since it must not only enable the author to recapture the details of the event at a later date if he is called to testify, but it must also provide necessary and understandable information to a variety of other individuals with varying backgrounds whose understanding of the findings is essential to a final disposition of the case.

The investigation of drug abuse deaths (legal, law enforcement, and medical aspects).

The pathologic investigation of a death caused by drug abuse need not be considered an onerous task. The investigator must be aware that it is a medicolegal investigation and proceed with the examination in a scientific and methodical manner, having verified all the circumstances, ascertained the evidence, and--most important--approached the problem as a multidisciplinary investigation requiring coordination of all information with proper authorities. The drug abuse problem cannot be considered to be fading, and with the advent of new sources of drugs, increased availability of others, and the emergence of newer synthetic compounds, the problem of drug abuse requires not only scientific knowledge but also challenges the detective in the pathologist.

The laboratory management of a medicolegal specimen.

The medicolegal specimen refers to any material, biologic or nonbiologic, that is analyzed by the laboratory as part of an investigative procedure, the results of which may be presented as evidence in a court of law. The medicolegal specimen is an example of physical evidence and has an intrinsic credibility based on the reliability of the person handling it, the reliability of methods of laboratory analysis and the reliability of testimony with relationship to the interpretations of the findings. Aspects of specimen integrity, re: chain of custody, sample collection, specifications for samples and utilization of samples, are outlined. The responsibilities for the management of the specimen are defined and discussed with suggested guidelines recommended.
