Experience with atypical mycobacterial infection in the deep structures of the hand.

Two new cases of atypical mycobacterium infections of the deep structures are reported. With two reported previously by the authors and a review of 24 others recorded by others, the symptoms and signs are reviewed. Typically it occurs in the middle-aged person, some of whom give a history of a puncture wound within 6 weeks of onset of symptoms. Synovium in the finger is involved commonly and a carpal tunnel syndrome may be the result of involvement of the bursae. Fever does not occur and no systemic signs are present. Biopsy and cultures are essential for diagnosis, but a presumptive diagnosis indicates that, after synovectomy, treatment should be started with antituberculous drugs, isoniazid with ethambutol, Rifampin, or both and continued for 18 to 24 months, unless in vitro sensitivity tests indicate a change of medication. The usual organisms are M. kansasii, M. marinum, M. intracellulare, and M. avium.

Progressive hypereosinophilia with lung nodules due to thyroid carcinoma.

An elderly woman had dyspnea, a thyroid mass and multiple lung nodules on chest radiographs. She developed hypereosinophilia over a three-month period in which multiple diagnostic procedures were performed, without definition of the disease process. At post-mortem examination, the primary lesion was found to be a poorly differentiated carcinoma of the thyroid, metastatic to lung. This is the second case of hypereosinophilia related to metastatic thyroid carcinoma. The intensity of the hypereosinophilia in this case was increased by the administration of thyroid-stimulating hormone and by the development of necrosis of the lung nodules.

Extrinsic allergic alveolitis after Aspergillus fumigatus inhalation. Evidence of a type IV immunologic pathogenesis.

Three weeks after a massive inhalation of mold present on infected oats, a farmer's wife had extrinsic allergic alveolitis. Aspergillus fumigatus was cultured from the moldy oats and from deep bronchial washings obtained at fiberoptic bronchoscopy. Spores and hyphae characteristic of Aspergillus species were demonstrated within granulomas in the pulmonary tissue obtained by transbrochial biopsy. Serum precipitins, delayed (48 hour) cutaneous hypersensitivity and in vitro lymphocyte transformation to A. fumigatus were demonstrated. The findings in this case suggest that a type IV immunologic response and subsequent (lymphocyte-mediated) tissue inflammation may underlie the pathogenesis of this and other forms of hypersensitivity pneumonitis.

Observations on a small group of mountain gorillas (Gorilla gorilla beringei).

On the basis of a 5-month field study, data are presented on home range, patterns of daily movement and social behaviour in a small free-living gorilla group. Changes in group composition that occurred during the study are analysed with special reference to their effect on inter-individual distance. The relationships with other groups and lone individuals whose home ranges overlapped with that of the group studied are examined on the basis of intergroup contacts.
