The moving fluid field in ophthalmic surgery.

A microscope mounted dripper and a cottonold wick for removal of excess fluid provide a moving fluid field for anterior segment microsurgery. The advantages of the moving fluid field include protection of the corneal epithelium, a field cleaned of debris and blood, and optimum visualization of the anterior segment.

Management of patients with intraocular lenses. Guidelines for those who do not perform this operation.

Patients with intraocular lenses can be managed quite well by any experienced ophthalmologist. Every exmination must include the measurement of intraocular pressure and a slit-lamp examination, specifically noting: Corneal oedema, local or diffuse, Increasing corneal guttata, Touch of lens or iris suture to the cornea, AENS DISPLACEMENT, Iris suture problems, Pupil margin erosion, Secondary cataract, Vitreous face loss or thickening, Retrolenticular membrane formation, Vitreous cells. Lens implants should be removed only after very carefully consideration and consultation with an implant surgeon.

Acetone sterilization in ophthalmic surgery.

Acetone is a potent bactericidal agent and has considerable value for the routine disinfection of surfaces. The reason for the poor showing of acetone in previously reported tests was that it was not used in concentrated form, and our tests also confirmed its relative ineffectiveness when diluted. The inability of acetone to eliminate spores is an important disadvantage but most commonly used bactericidal agents also are deficient in this respect. Although acetone is active in the presence of protein it does not penetrate blood clots. Therefore, instruments should be cleaned of blood and tissue fragments before using acetone to disinfect them, as with other methods of sterilization. Acetone can make ordinary sterilizers unnecessary in our offices. I believe that it is a superior antiseptic for use in sterilizing sharp instruments in the operating room and can be used for all instruments when autoclaving or gas sterilization is not feasible or not available.
