Isolation and characterization of rat alpha-lactalbumin: a glycoprotein.

alpha-Lactalbumin was purified to homogeneity from rat milk. Rat alpha-lactalbumin, in contrast to other alpha-lactalbumins, is a glycoprotein and exhibits an abnormally high molecular weight when obtained by gel filtration or electrophoresis in sodium dodecyl sulfate. The molecular weight by sedimentation equilibrium is 15 400 +/- 5% and of the reduced and alkylated protein is 16 000 when determined by thin-layer chromatography in 6 M guanidine hydrochloride. At least, three major charge forms, all containing carbohydrate and active in the lactose synthetase reaction were demonstrated. The amino acid composition reveals a high proline content which is reflected in a low alpha-helical content.

Medulloblastoma. A review of the LDS hospital experience.

A review of 14 patients with an initial diagnosis of medulloblastoma treated with irradiation therapy is presented. Follow-up data are available on all patients. When one evaluates those patients who were treated with total CNS irradiation in ours and other institutions, an excellent five-year survival of approximately 70% is observed. The point of significance is that the dose to the spinal axis was in the range of 2500 rads for the majority of the survivors with a boost to the posterior fossa between 4500 and 5000 rads. Only one patient had metestetic disease, and that was a patient who had a ventricular atrial shunt placed initially for control of increased intracranial pressure. These survival data add to already published data from Hope-Stone and others reporting excellent five-year survival with total CNS irradiation and are contradictory to the reports of approximately 40% five-year survival. The implications of the following review are that one can question the necessity of taking all patients with medulloblastoma to 3500 rads to the spinal column. We would suggest that if no clinical or laboratory evidence exists for spinal involvement, the use of a low dose to the spinal column of approximately 2500 rads decreases the potential for spinal column arrest in this young group of patients.

A comparison between lymphangiography and pelvic node dissection in the staging of prostatic cancer.

On 40 consecutive patients with prostatic cancer who had pedal lymphangiography during the initial evaluation and, subsequently, underwent pelvic node dissection or biopsy, a surprisingly high number had falsely positive (59 per cent) or negative (36 per cent) x-ray findings. Initially the tumors were considered clinically to be stage B in 24 cases, stage C in 13 and stage D in 3. After lymph node dissection only 17 tumors were still considered to be stage B and 7 were stage C, while 16 tumors were actually stage D. This surgical staging is important for the further management of the patient as well as the prognosis, Pedal lymphangiogrpahy alone is unreliable for accurate assessment of the regional lymph node status in clinically localized prostatic cancer.

Transmammary passage of Strongyloides sp. larvae in the human host.

The prevalence of infection with Strongyloides fuelleborni and hookworms (Ancylostoma duodenale and Necator americanus), and the possible transmammary passage of these parasites, was studied in the people of a village in Bulapé, Zaire, Africa. Stool examinations revealed that 34% of 76 infants under 200 days of age were infected with S. fuelleborni and 8% were infected with hookworms. Infection rates in the general population were 44% for S. fuelleborni and 90% for hookworms. The examination of milk from nursing mothers revealed the presence of Strongyloides larvae in one case. The finding suggests that S. fuelleborni may be transmitted via the milk in humans.

Hospital admissions for peptic ulcer during 1958-72.

Unpublished data from the Hospital In-patient Enquiry in England, Wales, and Scotland between 1958 and 1972 show that the frequency of admissions to hospital for peptic ulcer, particularly gastric ulcer, has fallen. This seems more likely to have been due to a true fall in the incidence of ulcer than to changes in treatment. Some three times as many people are admitted to hospital with duodenal ulcer in the north as in the south, but the frequency of admissions for gastric ulcer seems to vary little. The reasons for these differences are not understood.

Malignant thymoma with penetration into the gastrointestinal tract: report of two cases.

Two cases of thymoma, one epiithelial and one seminoma type are presented. In both instances, the thymoma infiltrated and later perforated the gastrointestinal tract by direct extension. One is the 30th reported case of thymoma with extrathoracic metastasis; this patient had metastasis to the bone marrow and kidney. Neither patient had myasthenia gravis associated with the thymic tumor.

Determination of regional pulmonary mechanics using a scintillation camera.

Electrical circuit R-C analogue models that reflect the difference in disease state between normal and diseased subjects, and normal and diseased regions of a diseased lung, were used to evaluate distal and proximal resistances and compliances for lung regions. Experimental values of regional flows and volumes for normal and diseased human lungs were obtained from a noninvasic scintillation camera technique in which the subject breathed Xe133-seeded air. Values of resistance and compliance were obtained with the additional use of an esophageal balloon. Total volumes and flows were measured through spirometry. Regional volumes were determined and regional flows were found through numerical differentiation. In analytical modeling, values of resistances and compliances were chosen so that the computed flow and volume curves corresponded to the actual flow and volume curves. Differences in resistance were an order of magnitude higher for the diseased subjects while compliances were generally several times lower than those for the normal subjects. However, the absolute values do not correlate well with those values generally found in the literature because compliance is usually measured statically and resistance measured during tidal breathing or panting. Since the forced vital capacity maneuver used here is extremely dynamic, there is significant airway collapse which results in functionally higher resistances and lower compliances. This maneuver tends to accentuate the differences between normal and diseased subjects and a subject's normal and diseased lung regions.

Lymphaticoperitoneal fistula.

Two cases of lymphaticoperitoneal fistula are reported. One patient had a recurrent carcinoma of the cervix and the other had a recurrent vaginal carcinoma. A brief discussion of the pathophysiology is presented.
