Comparison of the diagnostic value of serum pancreatic isoamylase and immunoreactive trypsin measurement in patients with cystic fibrosis.

We have measured serum immunoreactive trypsin (IRT) and serum pancreatic isoamylase (PIA) activities using commercially available kits in 37 cystic fibrosis (CF) patients and 46 hospital controls of similar age range. Immunoreactive trypsin was more often abnormal than PIA (26/37 v 18/37 abnormal respectively); IRT will be particularly useful as an additional diagnostic test in older children, in whom interpretation of the sweat test may be difficult, as 14/15 CF patients aged over 10 years had abnormal IRT results. Less than half of our patients who were aged between one and nine years had abnormal IRT activity, limiting the value of the test, though a low activity would still support the diagnosis of CF. Comparison with faecal fat estimations in 31 patients suggests that neither IRT nor PIA can be used as a non-invasive test of pancreatic function in order to identify those few CF patients who do not require pancreatic enzyme supplements.

Percutaneous nephrostomy with immediate dilation.

Percutaneous nephrostomy may serve as an alternative to an open operation for urinary diversion. Difficulties with poor drainage and premature catheter dislodgement have prompted the development of a technique for dilation of the needle tract and placement of a large, self-retaining catheter at the time of the initial puncture.

Gastritis and cirrhosis--no association.

Endoscopically sited gastric mucosal biopsies were taken from 98 patients with hepatic cirrhosis, and from 48 control patients. Nineteen patients with cirrhosis were found to have gastric ulcers, including eight with multiple erosions. In four of these eight, erosions were shown to arise in histologically normal mucosa. Among the remaining 79 patients with cirrhosis, the prevalence of chronic gastritis was not increased compared with controls of the same age-group. There was no increase in chronic gastritis in patients with alcoholic cirrhosis compared with either controls or patients with chronic active hepatitis and cirrhosis. The increased proneness of patients with cirrhosis to acute and chronic gastric mucosal lesions cannot be explained on the basis of an underlying chronic gastritis.

Xeroradiography of radical vulvectomy specimens. Identification of groin lymph nodes.

Twelve surgical specimens from radical vulvectomy and bilateral groin dissection were examined by xeroradiography to identify lymph nodes. Eight of the specimens were handled in a routine fashion, and four were chemically cleared. The xeroradiograph was not helpful in identifying small lymph nodes in the specimen, nor could any xeroradiographic characteristics specific for tumor-involved lymph nodes be identified.

The in vitro activities of pathogenic mineral dusts.

The pathogenic properties of mineral dusts are described and the range of relatively new materials requiring study is indicated. Since the animal facilities available for testing these potential hazards are limiting the authors describe an in vitro screen being used to select interesting dusts for in vivo experimentation, the type of results obtained are illustrated with some data for dusts derived from the industrially important Zeolites.

Splenic size and function in adult coeliac disease.

Splenic scintigraphy was performed after measurement of 99Tcm-labelled heat-damaged red cell clearance in 29 patients with treated coeliac disease. Good correlation was found between splenic size and function using these techniques. 99Tcm-labelled sulphur colloid scintiscans were also performed in 24 coeliacs. Splenic volumes computed from colloid scans correlated well with those computed from red cell scans. Almost 50% of adult patients with treated coeliac disease have impaired splenic function, the incidence rising with increasing age of starting treatment. Sulphur colloid scintigraphgy provides a simple quantitative method of assessing splenic function in treated coeliac disease which can be used in exploring the possibilities of avoidance of the adverse effects of hyposplenism.

Cystine calculi are radiopaque.

Cystine calculi are said to be either radiolucent or radiopaque. In the past, contamination of the calculi with calcium has been given as the reason for a radiopaque appearance. However, most cystine stones are pure cystine and contain essentially no calcium. When compared to adjacent fluids and tissues, they are radiopaque because of their higher physical density and their higher effective atomic number.

The activity of two types of asbestos in tissue culture.

The effects of chrysotile and crocidolite asbestos samples on the plating efficiency of V79-4 cells has been studied. The interaction of cells with chrysotile is more rapid than that with crocidolite, and these interactions are affected differently by changes in experimental technique. It is concluded that the differences detailed are not related to the pathogenicity of those fibres.
