Severe hypoxemia due to shunting through a patent foramen ovale: a correctable complication of right ventricular infarction.

A patient with recent inferior myocardial infarction with right ventricular involvement developed severe hypoxemia unresponsive to 100% oxygen. Contrast two-dimensional echocardiography revealed right to left shunting through an aneurysmal fossa ovalis with a patent foramen ovale. This was confirmed by cardiac catheterization. Surgical closure of the defect was probably life-saving. This case report illustrates that right to left shunting through a foramen ovale should be considered in the differential diagnosis of hypoxemia in patients presenting with inferior myocardial infarction.

Echocardiographic features of a mycotic aneurysm of the left ventricular outflow tract caused by perforation of mitral-aortic intervalvular fibrosa.

We present the apparently unique M-mode and two-dimensional echocardiographic features of a surgically confirmed pseudoaneurysm of the left ventricular outflow tract, which probably developed as a result of perforation of the mitral-aortic intervalvular fibrosa. Echocardiographic studies revealed an aneurysmal sac situated between the aortic root and the left atrium. The aneurysm expanded in systole and collapsed or emptied in diastole, suggesting direct communication with the left ventricle. Such an aneurysm must be differentiated from various pathologic findings of the aortic root. The location and characteristic motion during the cardiac cycle should alert the clinician to the correct diagnosis of such an aneurysm.

Feasibility of detailed two-dimensional echocardiographic examination in adults. Prospective study of 200 patients.

This study highlights the applicability of two-dimensional echocardiography in a group of unselected adult patients referred to the clinical echocardiographic laboratory. The two-dimensional echocardiographic imaging technique permits one to visualize large portions of cardiac anatomy in the majority of patients. The more conventional image presentation and the ability to appreciate spatial relationships make two-dimensional echocardiography an extremely useful imaging technique.

Isatin-enzyme interactions. VII. Mechanism of inhibition of rat kidney alkaline phosphatase.

Isatin has been found to inhibit rat kidney alkaline phosphatase (EC 3.1.3.1). The inhibition is dependent on isatin concentration and is of un-competitive type. The hydrolysis of disodium phenyl phosphate by the enzyme at different temperatures (17--37 degrees C) obeys the Arrhenius equation. Energy of activation in the absence and presence of isatin has been found to be 9.84 and 10.24 kCal/mol. The hyperbolic profile of isatin inhibition; the lowering of both Km and Vmax in the presence of isatin, and, small changes in enthalpy, free energy and entropy in the presence of isatin suggest a non-allosteric un-competitive inhibition of the enzyme.

Nature of inhibition of rat testicular alkaline phosphatase by isatin.

Isatin has been found to inhibit rat testicular alkaline phosphatase (EC 3.1.3.1). That the inhibition is non-competitive as well as non-allosteric is evident from a) the hyperbolic curve relating inhibition as a function of inhibitor concentration; b) the small change in enthalpy, free energy and entropy; c) the number of isatin molecules associating with 1 molecule of the enzyme (n=1.29); and, d) the decrease in the values of both Km and Vmax in the presence of isatin.

[Isatin-enzyme interactions. VI. Inhibition of rat kidney alkaline phosphatase].

Interaction of isatin with rat kidney alkaline phosphatase has been studied. Mode of attachment of isatin with the enzyme protein is most likely through amino group(s), which is also imperative for catalysis. Sulphydryl group(s) do not seem to be involved in enzyme action. Zinc is also needed for enzyme activity. Use of sulphydryl compounds suggests that isatin inhibition of the enzyme is through attachment at the metal site. However, this inhibition may not only be due to simple chelation of the metal by isatin.

Right aortic sinus of Valsalva-to-right ventricle fistula complicating bacterial endocarditis of membranous ventricular septal defect: evaluation by two-dimensional, color flow, and Doppler echocardiography.

Rupture of the aortic sinus of Valsalva into the right ventricle is a rare complication of bacterial endocarditis. The majority of previous cases have been reported in patients with aortic valve endocarditis. This report describes this complication in association with infective endocarditis of a congenital membranous ventricular septal defect. Color flow imaging showed a mosaic-colored continuous flow signal from the right aortic sinus to the right ventricle. Continuous wave Doppler interrogation showed high-velocity systolic and diastolic flow signals due to continuous left-to-right shunt from the aorta to the right ventricle. The left-to-right shunt through a restrictive membranous ventricle septal defect was visualized as a mosaic-colored high-velocity systolic flow signal. These echocardiographic-Doppler findings permitted surgical correction of these defects without cardiac catheterization.

Blue digit syndrome: transesophageal echocardiographic identification of thoracic aortic plaque-related thrombi and successful outcome with warfarin.

We describe two patients with blue digit syndrome in whom transesophageal echocardiography was able to identify mobile thrombotic masses attached to the irregular intimal surface of the descending thoracic aorta. These patients were treated with heparin and warfarin and did not have recurrent episodes of peripheral arterial embolization. In this article we discuss the diagnostic and therapeutic approaches in patients with peripheral arterial embolization and blue digit syndrome.

Biplane transesophageal echocardiography: technique, image orientation, and preliminary experience in 131 patients.

Transesophageal echocardiography with use of a uniplane phased-array transducer with a transverse or horizontal scanning plane has become a well-established tool for evaluating cardiovascular diseases. Recent introduction of biplane probes has enhanced the diagnostic capability of this imaging technique. This article discusses the technique of biplane transesophageal echocardiographic examination and image orientation. The diagnostic value and advantages of various biplane transesophageal images of the heart, aorta, and aortic arch from various esophageal positions are described.

Embryonal carcinoma of the testis presenting as a left heart mass.

A 31-year-old man with a history of intravenous drug abuse and tooth abscess was admitted for evaluation of possible infective endocarditis. Echocardiography showed that he had a left atrial mass. The mass removed from the left atrium had the same histology as the primary embryonal carcinoma discovered in the right testicle during hospitalization. The patient made a smooth recovery after surgical intervention and chemotherapy. This is believed to be the first reported case of metastasis from embryonal carcinoma of the testis to the left side of the heart that was successfully removed at surgery.
