A new allele of human alpha1-antitrypsin: PiNhampton.

A new allele of alpha1AT is described. By isoelectric focusing, the microheterogeneous pattern of the variant was similar to but more cathodal than that of Pi N. This allele has therefore been tentatively designated PiNhampton(Nham). Further examination revealed that the minor bands of Nham are indistinguishable from the major bands of Z by isoelectric focusing, and a careful family study was necessary to clearly define the proband's phenotype. Pi Nham was found in association with M1, S, and Z, but to date its possession is not apparently related to clinical disorders or reduced serum levels of alpha1AT.

Conversion of jejunoileal bypass to gastric bypass to maintain weight loss.

We have performed jejunoileal bypass procedures in 114 patients during the last nine years. Seven of these patients have had complications develop that required restoration of normal gastrointestinal continuity. In the last four patients in our series, we have performed a gastric exclusion operation at the same time that normal anatomy of the small intestine was restored. These operations lasted only three and one-half hours and were performed without difficulty and without postoperative coomplication. The metabolic derangements have been corrected and weight loss has been maintained. We believe that the gastric bypass provides the surgeon with an effective additional option in managing patients with serious complications of their jejunoileal bypass that necessitates restoration of intestinal continuity.

A vertical cephalometric analysis of the human craniofacial complex.

A vertical analysis involving ten linear measurements was developed to localize skeletal and dentoalveolar dysplasias. The measurements are of biologic relevance, reflecting the amount of growth harmony or disharmony to the examination and record-taking time. The vertical dimensional analysis uses the age- and sex-related University of Michigan mean values for comparative data for each measurement. Ratios were computed for a five-unit proportional analysis. Both analyses were applied to assess the vertical problems of three female patients. The analyses detected specific areas of dysplasia for each patient.

Correlation of metabolic and chemiluminescent responses of granulocytes from three female siblings with chronic granulomatous disease.

Polymorphonuclear leukocytes (PMNL) from children with atypical chronic granulomatous disease (CGD), their mother and siblings, and normal controls were studied in regard to glycolytic and hexose monophosphate shunt activities in the resting, methylene blue-stimulated, and phagocytizing states. PMNL from the patients with CGD had normal glycolytic and hexose monophophate shunt activities in the resting state and after stimulation with methylene blue. However, stimulation of the hexose monophosphate shunt after phagocytosis was greatly decreased. These data were correlated with studies of both initial rate and integral counts of chemiluminescence. The chemiluminescent response from patients with atypical CGD was also greatly decreased. This decreased response probably reflects a defect in the oxidative destruction of the phagocytized microbe and correlates well with the decreased activity of the phagocytically activated hexose monophosphate shunt. The defect in generation of radical species of oxygen, singlet oxygen, and chemiluminescence by leukocytes from patients with CGD is discussed.

Evaluation of serum opsonic capacity by quantitating the initial chemiluminescent response from phagocytizing polymorphonuclear leukocytes.

Phagocytically activated polymorphonuclear leukocytes produced a chemiluminescence that could be correlated metabolically with the stimulated oxidation of glucose via the hexose monophosphate shunt, The chemiluminescence observed was considered to originate from the relaxation of electronically excited carbonyl groups produced during singlet molecular oxygen-mediated microbicidal oxidation of the ingested microbe. With adequate adjustment of leukocyte and bacterial concentrations, the rate of chemiluminescence increase was nearly constant for the first minutes after initiation of phagocytosis. This rate was dependent on the quantity of bacteria phagocytized by the leukocytes. If both leukocytes and bacterial concentrations were held constant, this initial rate of chemiluminescence reflected the opsonic capacity of the sera used for opsonization. The prior absorption of opsonins from serum resulted in a decresed rate of chemiluminescence related to the quantity of bacteria used for absorption. Heating of sera to 56 degrees C for 30 min resulted in a great decrease in the chemiluminescent responses and may reflect the role of complement in the opsonization process.

Carboxyarylindoles as nonsteroidal antiinflammatory agents.

An extensive series of carboxyarylindoles has been evaluated for antiinflammatory activity in the carrageenin paw edema assay. The requirements for optimal antiinflammatory activity in this series are relatively specific: a central pyrrole nucleus with (a) a 3-carboxy-4-hydroxyphenyl moiety substituted directly on the nitrogen, (b) a 2-phenyl group (R2) with a substituent of low electronegativity, (c) absence of a substituent in the 3 position (R3), and (d) a system fused across the 4,5 positions (X), which is lipophilic, quasiplanar, and does not interact sterically with the N-phenyl group. One derivative, 3-(3-carboxy-4-hydroxyphenyl)-2-phenyl-4,5-dihydro-3H-benz[e]indole (42), has been selected for further study.

Concanavalin A-horseradish peroxidase bridge staining of alpha-1 glycoproteins separated by isoelectric focusing on polyacrylamide gel.

The Coomassie Blue protein stain and the periodic acid-Schiff stain for glycoproteins are compared to a new method of staining glycoproteins resolved electrophoretically. The method utilizes a Concanavalin A-horseradish peroxidase sequence to visualize selectively glycoproteins with terminal or internal mannose or terminal N-acetylglucosamine. The method applied to characterization of M and Z allele products of alpha-l-antitrypsins separated by isoelectric focusing of polyacrylamide gels slabs have revealed differences in carbohydrate content of various components that were previously undetected.

The intraocular pressure response of conscious rabbits to clonidine.

A study has been made of the time courses of the pupillary and intraocular pressure responses of conscious rabbits to clonidine administered either topically or intravenously. Topical unilateral application of clonidine caused transient pupil dilatation and a biphasic intraocular pressure response; an initial hypertensive response preceded a hypotensive phase lasting several hours. Pupillary and hypertensive responses were absent in the untreated eye, but there was a rapid decrease of intraocular pressure. Intravenous administration of clonidine caused an immediate and large decrease of intraocular pressure in both eyes. Phenoxybenzamine given intravenously inhibited the pupillary dilatation and the hypertensive responses to clonidine. The role of efferent adrenergic neuronal activity in mediating the local biphasic pressure response was studied in rabbits with unilateral precervical and postcervical sympathotomy. The results showed the hypotensive response to be dependent on an intact adrenergic innervation of the ocular tissues.
