Milk composition of insulin-dependent diabetic women.

The macronutrient, trace mineral, and immunological contents of milk of five moderately controlled, insulin-dependent diabetic women were studied at 3 months postpartum. Concentrations of total nitrogen, lactose, fat, and energy were not distinguishable from those in milk of nondiabetic women. Glucose concentrations in milk from diabetic women were significantly higher (p less than 0.001) and more variable. The macro- and trace minerals (Ca, P, Mg, K, Zn, Cu, and Fe) were within normal ranges with the exception of Na which was higher in milk from the diabetic women (p less than 0.05). The concentrations of lactoferrin and secretory IgA were not statistically different from reference norms. Milk composition of five insulin-dependent diabetic women was not distinguishable from that of the reference population, with the exceptions of Na and glucose concentrations, which were slightly elevated.

Measurement of recovery from myocardial infarction using heart rate variability and psychological outcomes.

This pilot study was designed to describe heart rate variability (HRV), anxiety, anger, denial, and depression during the first 4 days and 6 months after acute myocardial infarction (AMI). The sample was composed of 21 post-AMI males aged 40 to 83. State anxiety was elevated early. At 6 months it had significantly decreased. The nonpower measure of HRV, the Kleiger global standard deviation, was decreased to a mean score of 86 (SD = 29) msec during AMI and was 117 (SD = 30) msec at 6 months. Although there were no significant correlations between HRV, psychological response, age, and education, there were moderate correlations between HRV and state anger (r = .33), denial (r = .35), and education (r = .45). HRV, as measured by the power spectral density function of an autoregressive model, demonstrated an increased peak across the band of frequency associated with the high-frequency components of the parasympathetic system and a decreased peak across the band associated with the low-frequency components of the sympathetic system at 6 months as compared to the AMI period.

Failure of low molecular weight dextran to prevent clotting during continuous renal replacement therapy.

Clotting of the extracorporeal circuit during continuous renal replacement therapy results in decreased ultrafiltration rates, impaired solute clearance and, ultimately, occlusion of the extracorporeal circuit. The authors conducted an open-label randomized controlled trial to determine whether low molecular weight dextran could prevent hemofilter clotting in patients undergoing continuous venovenous hemodialysis. Eleven patients were randomized to receive a continuous infusion of 10% low molecular weight dextran at 25 mL/hr; 8 patients served as control subjects. No differences in the frequency of hemofilter clotting or hemofilter lifespan were detected. The authors concluded that continuous infusion of low dose low molecular weight dextran is not effective in preventing clotting during continuous renal replacement therapy.
