American cutaneous leishmaniasis outbreak, Tartagal city, province of Salta, Argentina, 1993.

An American cutaneous leishmaniasis outbreak, with cases clustering during 1993 in Tartagal city, Salta, was reported. The outbreak involved 102 individuals, 43.1% of them with multiple ulcers. Age (mean: 33 years old) and sex distribution of cases (74.5% males), as well as working activity (70 forest-related), support the hypothesis of classical forest transmission leishmaniasis, despite the fact that the place of permanent residence was in periurban Tartagal. Moreover, during July, sandflies were only collected from one of the 'deforestation areas'. Lutzomyia intermedia was the single species of the 491 phlebotomines captured, reinforcing the vector incrimination of this species. Most infections must have been acquired during the fall (April to June), a pattern consistent with previous sandfly population dynamics data. Based on the epidemiological and entomological results, it was advised not to do any vector-targeted periurban control measures during July. Further studies should be done to assess if the high rate of multiple lesions was due to parasite factors or to infective vector density factors.

[Congenital tuberculosis].

Congenital tuberculosis is a rare disease of variable incidence in accordance with prevalence of the disease in the general population. We report a case in a neonate born prematurely, whose mother presented with miliary tuberculosis diagnosed in the puerperal period. The newborn had no contact with his mother after delivery, neither with other infected people. The clinical picture consisted mainly in signs and symptoms of septicaemia of gradual onset. Mycobacterium tuberculosis was isolated from gastric fluids and the response to specific treatment, was excellent. Although congenital tuberculosis is a rare disease, is should be suspected in any neonate developing signs and symptoms of sepsis of unknown etiology. The importance of an early diagnosis and treatment is stressed in order to improve neonatal survival.

Combined transplantation of heart and liver from two different donors in a patient with familial type IIa hypercholesterolemia.

A 12-year-old child with terminal ischemic heart disease as a result of homozygotous familial type IIa hypercholesterolemia received an orthotopic heart transplant and, 21 days later, an orthotopic liver transplant. Six months after heart transplantation, the patient is asymptomatic and evidences normal liver function and cholesterol levels; there are no signs of heart rejection. To our knowledge, this is the first instance of two-step heart-liver transplantation with organs from different donors.
