Effects of exercise-induced alterations of cardiac physiology on nuclear perfusion imaging.

The effects produced on nuclear perfusion images by exercise-induced changes in the heart and respiration rate and possible transient myocardial stunning are not well understood. In this study we attempted to indirectly evaluate the potential artifacts created by exercise-induced changes in cardiac physiology. Twenty patients with prior myocardial infarction and suspected peri-infarct ischemia were studied by 99mTc-MIBI imaging. Two SPECT perfusion studies were performed after 99mTc-MIBI administration at rest. The first acquisition was carried out 90 minutes after injection of the tracer. Immediately afterwards, the patients underwent a stress test followed by a second acquisition (exercise stress superimposed on rest tracer distribution). A standard stress perfusion scan was also performed 48 hours later. Superimposed exercise stress produced artifactual defects in the resting distribution of the tracer in 15 out of 20 patients (68 of 360 segments). Standard stress images demonstrated concordant defects in 48 of these segments, indicating the concomitant presence of ischemia and stunning. This study indicates that exercise-induced changes in cardiac physiology may result in artifactual perfusion defects in scintigraphic images acquired shortly after the stress.

[Gallstone ileus].

15 cases of gallstone ileus were seen during the period 1951-1981 at the 2nd Surgical Department of the University of Vienna which makes up 1,3% of a total of 1 162 cases of ileus during this time period. A review of this condition is warranted, because of the variability of clinical symptoms, the difficulty of establishing a diagnosis based on clinical and radiological findings, and because of letality (26%) in our patient group. Care has to be taken not to overlook air in the bile ducts on abdominal x-ray; the possibility of a gallstone ileus has always to be considered especially in cases with a history of cholelithiasis. Surgical therapy consists in enterotomy, removal of the stone and drainage. Closing of the fistula usually is unnecessary since in most cases the fistula tends to occlude spontaneously.

[Automatic suturing devices in lung surgery (author's transl)].

Automatic stapling devices can be used for suturing the bronchus, pulmonary parenchyma and pulmonary vessels in lung resections. In particular, TA staplers have proved of great value in suturing the bronchial stump, helping to prevent the feared complication of stump insufficiency. No bronchopleural fistulae were recorded in a series of 220 lung resections.

[Trans-placental passage of amiodarone: electrocardiographic evidence and pharmacological findings in a newborn infant].

Trans-placental passage of amiodarone has not yet been demonstrated, even if it has been suspected from the observation of some cases of congenital mixedema in babies born of amiodarone-treated women. A pregnant woman, suffering from recurrent episodes of resistant high frequency, 1:1 atrial tachycardia, with severe hypotension, was treated with oral amiodarone 200 mg daily during the last three months of pregnancy. The arrhythmia was satisfactorily controlled and pregnancy was carried on well. At birth, the baby was normal on physical examination and routine blood exams. Particularly, thyroid function was normal. After delivery, the plasma level of the drug and its electrocardiographic effects both in the mother and the newborn were determined. High pressure liquid chromatography (HPLC) method was used for the pharmacologic determinations and led to demonstrate the presence of amiodarone and one of its metabolites in the newborn plasma. Placental permeability for the metabolite resulted to be higher than for amiodarone, comparing mother versus newborn drug concentrations. Electrocardiographic changes due to amiodarone (i.e. lengthening of QT interval) were observed in both the ECGs of the mother and the newborn, but in the latter lenghthening of QT was much more evident. The authors briefly report another personal case of amiodarone-treatment during pregnancy and conclude, on the basis of their experience, that amiodarone can be used in pregnancy, but strictly in refractory, life-threatening arrhythmias and limitedly to the last three months.

[High-frequency electrostimulation of the atrium in therapy of atrial flutter in patients pre-treated with amiodarone].

Twenty-four consecutive episodes of atrial flutter in 20 patients were treated with overdrive atrial pacing after a pre-treatment with oral Amiodarone. This technique was effective in interrupting the arrhythmia in 23 instances (95,8%). Normal sinus rhythm was restored in 22 cases: in 18 of then (75%) it appeared immediately at the end of stimulation and in 4 after a short intervening period of atrial fibrillation. In accordance with the experience of other authors, the combined pharmacological and electrical treatment provides a higher rate of conversion of atrial flutter into normal sinus rhythm and a lower rate of early relapses of the arrhythmia.

[Acute myocardial infarct as the first manifestation of acute myeloid leukemia. Description of an anatomo-clinical case].

A case of a 46 year old man in whom acute myocardial infarction was the earliest outstanding clinical manifestation of an acute promyelocytic leukemia is reported. Reinfarction occurred a few days after, causing death of the patient. autopsy and histologic findings are reported and pathogenetic hypothesis are discussed. The Authors point out the role of an ipercoagulable state created by leukemia itself.

[Possibilities of control of regulation of the function of arteriovenous anastomoses and of capillary blood flow by means of acupuncture].

Acupuncture was carried out on a patient with rheumatoid arthritis and microangiopathy of the lower limbs. Photoplethysmographic waves appeared at the right inferior limb where before they had been absent; at the left inferior limb normalization of postural reflexes was achieved, suggesting that acupuncture is effective in vasodilatation of capillaries and preterminal arterioles and in the reactivation of the arterio-venous blood flow.

[Surgery of the mitral valve (author's transl)].

A report is given on the results of different forms of mitral valve operations in 866 patients. Closed mitral valvulotomy was performed in 519 patients and mitral valve replacement in 291 cases. Open mitral valve anuloplasty was performed as reconstructive surgical procedure in 15 patients. The overall mortality was 10.4%. Follow-up of the surviving patients showed that the long-term results of mitral valve replacement are significantly worse than those of mitral valve reconstruction.

[Three decades' experience in the treatment of bronchogenic carcinoma by pulmonary resection (author's transl)].

A review of the development of thoracic surgery is followed by a description of the results obtained by us in the surgical treatment of bronchogenic carcinoma from the difficult beginnings in 1947 up to the present day. The diagnostic investigations, indications for operation and the operative risks are discussed on the basis of 1532 pulmonary resections. The subsequent fate of this patients is known to us almost without exception over a period of three decades. Diagrams show the ten-year survival rates. The importance of early diagnosis and the priority of surgical treatment are two basic conclusions which still hold true today.
