Peripheral retinal changes and axial myopia.

The relationship between the presence of peripheral chorioretinal changes and axial myopia in 513 eyes (513 patients) 24 mm or more in length, or beyond the upper range of normal axial length, is studied. Axial length was measured with A-scan ultrasonography (immersion technique) and retinal periphery was studied by biomicroscopic examination and scleral indentation. The presence of lattice degeneration, pavingstone degeneration, pigmentary degeneration, white with or without pressure (WWP), retinal holes, tears, or both (RHT), retinal detachment, and posterior vitreous detachment was reported. The correlation between presence of a lesion and a longer axial length was always significant, except with retinal detachment, pigmentary degeneration, and RHT. Pavingstone degeneration was significantly related to older age and WWP and RHT to younger age. Posterior vitreous detachment was frequent in almost all axial length groups and age classes. No significant relationship was found between any kind of lesion and gender of the patient, or between axial length and patient age.

Can the diode laser (810 nm) effectively produce chorioretinal adhesion?

In this study, argon green (514 nm), krypton red (647 nm), and diode (810 nm) lasers were used to produce transpupillary photocoagulations on rabbit chorioretina to simulate a retinal buckle treatment. To evaluate the efficacy of the diode laser in creating a retinopexy effect, the morphologic aspects of the acute lesions and scar development were compared. The study of the acute lesions revealed that the chorioretinal thermal damage produced by the argon green laser involved all the retinal layers and the retinal pigment epithelium. The use of the krypton red and the diode lasers resulted in deeper chorioretinal thermal damage. Two months after the treatments, all the lesions resulted in adhesive chorioretinal pigmented scars. However, the diode lesions produced a deeper scar, characterized by marked chorioretinal atrophy.

Compliance in antiamblyopia occlusion therapy.

Antiamblyopia occlusion therapy relies on compliance. We retrospectively reviewed the charts of 496 amblyopic subjects. Measures of non-compliance included patient reporting and patient records of broken appointments. Out of 496 subjects, 92 (18%) failed to follow the occlusion regimen. Compliance with treatment was analyzed by age group, refractive error and type of strabismus. The failure rate was 82.6% for the unilateral high myopia group and 37.5% for the monofixation syndrome group. The risk for non-compliance appeared to be higher in the 1 to 2-year-old group (37%). The chi 2-test showed the differences to be highly significant (P less than 0.0001). Children having lower initial visual acuity were also significantly less complaint (P less than 0.007). Several factors such as age, parental understanding, initial visual acuity and improvement rate seem to be involved in compliance.

Capillary and cavernous hemangioma of the optic disc. Echographic and histological findings.

The ultrasonography features of optic disc hemangioma have been described in various reports but the extreme rarity of this ocular tumour makes any univocal interpretation of echographic findings difficult. The two cases of hemangioma of the optic disc described here, one of capillary hemangioma and the other of cavernous hemangioma, presented different echographic patterns. In the capillary hemangioma B-scan showed a mass lesion with smooth anterior border, acoustic solidity and no choroidal excavation. With A-scan there was an initial high spike with low/medium internal reflectivity. In the cavernous hemangioma, B-scan showed an elevated dome-shaped mass, with an anechoic area inside, and no choroidal excavation. With A-scan there was a high initial spike and irregular reflectivity. The eyes were enucleated for intractable neovascular glaucoma. Histological examination of the lesion explained the different echographic patterns. Ultrasonography is useful in the differential diagnosis of capillary and cavernous hemangioma.

Ocular pharmacokinetics of rufloxacin a new fluoroquinolone antibiotic.

Ocular pharmacokinetics of rufloxacin (MF 934), a new monofluorinated quinolone derivative, has been investigated in rabbits. A long half-life, good g.i. absorption and a higher tissue/plasma concentration than that of other quinolones, are its interesting pharmacokinetic properties. However, there is reason to believe that drug accumulation may occur in deep body compartments. We determined plasma, aqueous, and vitreous concentrations of the drug at 1, 4, 8, and 24h after a single 50 mg/kg i.v. administration of rufloxacin. Our data show that rufloxacin, administered by the i.v. route, rapidly reaches chemotherapeutically useful levels in aqueous and vitreous fluids. Although still present in plasma 8 hours after administration, it proved to be undetectable in ocular fluids, signifying that the depletion of the deep compartments occurs well in advance of the next invasion. Due to its antibacterial effectiveness and pharmacokinetic properties rufloxacin may take a relevant place among the quinolone derivatives in the treatment of ocular infections.

Bifocal versus monofocal intraocular lenses in bilateral pseudophakia.

After a mean follow up of 19 months, the visual characteristics of eyes with bifocal diffractive intraocular lenses (IOLs) ("bifocals") in one eye were compared with those of fellow eyes with monofocal IOLs ("monofocals") in 29 bilateral pseudophakic patients. All of the eyes had a distance-corrected acuity of 0.6 or more, but 24.1% of the bifocals and 48.3% of the monofocals had an acuity of 1.00. Near acuity with distance correction was J2 or more in 93.1% of the bifocals and in 17.4% of the monofocals (without correction: 79.3% and 41.4%, respectively). In 55.2% of the bifocals and 20.7% of the monofocals, the combined uncorrected acuity was 0.5 or more for distance and J2 or more for near. Fogging revealed a similar depth of focus in the monofocals and a similar distance focus in the bifocals. The bifocals had two peaks of acuity but no "plateau" between them. The eye with the monofocal IOL was preferred by 37.9% and the eye with the bifocal IOL by 10.3%. Vision in no eye was rated "poor." Visual disturbances were more frequent in the bifocals. No glasses were prescribed for 20.7% of all of these patients; 65.5% required bifocal glasses; 44.8% managed most of daytime without glasses.

[Juvenile serous macular detachment of the pigmented epithelium].

Juvenile macular detachment of the retinal pigmented epithelium is rarely seen clinically. Single or multiple detachments can occur and are due to the accumulation of liquid in the subretinal space. The natural history of the disease, which usually affects young people, has not yet been defined. We examined and followed 19 eyes of 15 patients affected by macular juvenile serous detachment of the retinal pigment epithelium, not associated with other retinal disturbances. The patients were followed for a mean period of 12 months (range 6-33 months). The mean age was 48 years (range 40-59 years): 7 patients were male and 8 female. Mean initial visual acuity was 8/10 (range 1/10-10/10) and mean final visual acuity was 9.2/10 (range 6/10-10/10). Detachment of the retinal pigmented epithelium was unchanged in 58% of cases, decreased or disappeared in 37% of cases and increased in 5% of cases. These results indicate that the natural course is usually good and photocoagulation is not necessary.

Diode laser compared with argon laser for trabeculoplasty.

A randomized prospective study on two groups of ten patients compared the efficacy of diode laser and argon laser trabeculoplasty. In the diode laser group the intraocular pressure was 23.0 +/- 3.97 mm Hg before the treatment, 20.2 +/- 4.49 mm Hg at two hours, 16.3 +/- 3.13 at six months, and 16.9 +/- 2.80 mm Hg at one year. The differences from baseline were statistically significant at six months (P = .0001) and at one year (P = .0001) but not at two hours. In the argon laser group the intraocular pressure was 23.4 +/- 3.6 mm Hg before the treatment, 22.7 +/- 4.35 mm Hg at two hours, and 17.6 +/- 4.53 mm Hg at six months. One patient had uncontrolled mean high intraocular pressure and underwent surgery. In the nine patients who completed the study the intraocular pressure at one year was 16.7 +/- 3.00 mm Hg. The differences from baseline were significant at six months (N = 10; P = .0001) and 12 months (N = 9; P = .0001) but not at two hours. Differences between the two groups were not significant at two hours, six months, and one year. Laser trabeculoplasty may be effectively with a diode laser.

Hypertensive choroidopathy: a case report.

The authors report a case of exudative retinal detachment of the posterior pole and peripheral choroidal detachment in a patient with renovascular accelerated hypertension. The peculiar clinical situation of this patient was comparable with experimental models of hypertension recently reported in the literature. Both the retinal and the choroidal detachment regressed after blood pressure stabilization with systemic antihypertensive therapy, supporting a choroidal pathogenesis for the chorioretinal lesions.

Diabetic retinopathy after successful kidney-pancreas allotransplantation: a survey of 18 patients.

To investigate the effects of normoglycaemia on diabetic retinopathy, we evaluated 18 uremic diabetic patients before and after successful pancreas kidney transplantation. In all, 12 uremic diabetic patients who submitted to kidney transplantation alone served as the control group; 4 of these subjects received a kidney transplantation alone, whereas 8 underwent a double kidney-pancreas transplantation but lost the pancreas graft within the first few weeks post-surgery. The mean age and the mean duration of both diabetes and dialysis were comparable in the two groups. All patients were studied prior to and at 6 and 9 months after surgery, then at annual intervals. Subjects were divided into three groups according to follow-up: less than 1 year, between 1 and 3 years and greater than 3 years. At each control visit, a complete clinical examination was performed by two independent examinators; retinal fluorescein angiography was carried out as well. The following parameters were evaluated: visual acuity, capillary closure, macular oedema, neovascularization at the disk and elsewhere and vitreous haemorrhage. A score ranging from -2 to +2 was assigned to each parameter for quantification of the variation between baseline values and those obtained at the end of the follow-up. This score was assigned by two different ophthalmologists. Eyes that were affected at baseline by end-stage diabetic retinopathy (secondary retinal detachment, neovascular glaucoma) were not entered in the study. A total of 18 eyes were lost to follow-up in the 2 groups because of laser treatment, cataract extraction, anterior ischaemic optic neuropathy and cytomegalovirus retinitis.(ABSTRACT TRUNCATED AT 250 WORDS)

Effect of pancreas transplantation on diabetic retinopathy: a 20-case report.

In order to study the effects of normoglycaemia on diabetic retinopathy, 20 diabetic uraemic patients who underwent a kidney-pancreas transplantation were evaluated before and after surgery (6.9 months and once a year). The control group consisted of 12 uraemic patients who underwent kidney transplantation alone. At each follow-up examination a complete clinical examination and a retinal fluorescein angiography were performed. The eyes with end-stage retinopathy at baseline were excluded from the study. The analysis of the results showed no significant differences in the two groups. The diabetic retinopathy at the moment of the transplantation was already too advanced to benefit from the better glycaemic control.

Early ultrasonographic diagnosis of hereditary retinoblastoma.

A hereditary retinoblastoma (RTB) was identified by ocular echography in a newborn, whose predisposition to RTB had been assessed based on the family history and DNA testing of the chorionic villi at the eighth week of pregnancy. Ultrasonography was performed during pregnancies without an abnormality being demonstrated. On the third day of life a B-scan examination showed a small membranous lesion in the nasal parapapillary area, whilst on A-scan, the lesion appeared as a hyperreflective peak. Thanks to its early identification, the tumor was successfully treated by photocoagulation.

Clinical variability in keratometry, ultrasound biometry measurements, and emmetropic intraocular lens power calculation.

We calculated inter-examiner and intra-examiner variability in keratometry and ultrasound biometry when performed by two different examiners. The potential effect on intraocular lens power calculation was also considered. Intra-examiner variability was statistically and clinically significant for axial measurements, particularly in eyes with axial length greater than 24 mm. Thus, variability in biometric examination affected the emmetropic intraocular lens power, especially in myopic eyes.
