Portal fibrosis: intrahepatic portal vein pathology in chronic human schistosomiasis mansoni.

Thirty five cases of severe late chronic human schistosomiasis were studied. The attention was given to the portal vascular lesions. Four stages from isolated endothelial involvement to entire portal vein wall disorganization were observed. Ultrastructural evidence of smooth muscle cell dedifferentiation is noted with loss of well defined limits between periportal connective tissue and venous vascular wall. The synergistic action of hypertension, subendothelial transudation of serum and proliferation of vascular connective tissue cell lines, are considered to explain the self perpetuating character of portal fibrosis in chronic human schistosomiasis.

Adherent peritoneal mononuclear cells express chloroacetate esterases in vitro.

Murine peritoneal adherent mononuclear cells elicited by intraperitoneal glass implants express nonspecific esterases (alpha-naphthyl acetate esterases) as demonstrated by enzyme histochemical methods. After nine days of in vitro culture, 50% of cells obtained from normal mice and 100% of cells obtained from schistosome-infected mice expressed chloroacetate esterases (naphthol AS-D chloroacetate esterases). Fifty percent of the cells from normal mice continued to present nonspecific esterase activity, whereas cells from infected mice presented only chloroacetate esterase activity. This modulation of esterase expression can be interpreted to be a reflection of in vitro cell differentiation.

Establishment of a continuous cell line from fibrotic schistosomal granulomas in mice livers.

A continuous murine cell line (GRX) was obtained from fibrotic granulomas induced in C3H/HeN mice liver by experimental infection with Schistosoma mansoni. This anchorage-dependent line produces composite connective tissue/extracellular matrix, displays morphological characteristics of myofibroblasts, and can, under appropriate conditions, accumulate fat droplets. GRX cells produce viral particles of retrovirus type. We consider GRX cell line to be representative of liver connective tissue cells, responsible for fibroplasia in liver fibrotic and granulomatous reactions.

Schistosoma mansoni: extramedullar eosinophil myelopoiesis induced by intraperitoneal glass implants in chronically infected mice.

Intraperitoneal glass implants in mice with chronic Schistosoma mansoni infections induce intense local myeloid reactions involving essentially myeloid granulocytes. The same phenomenon is not observed in normal mice nor in mice with acute schistosomal infection. An association of myelopoiesis with differentiated macrophages mobilized on glass implants and with dense ameboid cells located inside myeloid foci was detected. A macrophage-dependent induction of this eosinophil reaction is postulated.

Liver connective tissue cells isolated from human schistosomal fibrosis or alcoholic cirrhosis represent a modified phenotype of smooth muscle cells.

Hepatic connective tissue cells associated with schistosomal fibrosis and alcoholic cirrhosis were studied in vitro. Primary cell lines were isolated from all biopsies: they were identified as specific homogeneous cell populations, named liver connective tissue cells (LCTC). They were recognized as analogous to smooth muscle cells, different from true fibroblasts by morphological and physiological criteria. The proliferative capacity of LCTC is directly proportional to the degree of fibrosis in hepatic tissues. LCTC are able to secrete type I, III and IV collagen, fibronectin, laminin and amyloid P component. Their relationship with specific pathology of intrahepatic vascular tree in schistosomiasis is hypothesized.

Chronic schistosomiasis mansoni: splenic myelopoiesis and inhibition of neutrophil granulocytopoiesis mediated by the sera of patients.

The correlation among schistosomal infection, splenomegaly, and modifications of circulating blood leukocytes was studied. Peripheral neutropenia was found to occur concomitantly with incomplete splenic myelopoiesis of the neutrophil line. Sera from patients with schistosomiasis modified the proliferation of murine bone-marrow cells in soft-agar cultures. A total inhibition of neutrophil differentiation was compensated for by a proportional increase in macrophage differentiation. The neutrophil-inhibitory activity of sera of patients with schistosomiasis may be responsible for the delayed in vivo maturation of neutrophils in the bone marrow and spleen of these patients.

Titres of antibodies to gut-derived antigens in patients with chronic schistosomiasis mansoni.

Serum titres of antibodies against six intestinal, one ubiquitous and one non-intestinal bacteria were determined in patients with severe hepatosplenic schistosomiasis mansoni, with light intestinal schistosomiasis and in normal subjects. No significant difference was observed among the three groups of subjects for levels of antibodies against two non-intestinal bacteria and four of the intestinal bacteria. Patients with hepatosplenic schistosomiasis had titres of antibodies against one strain of Pseudomonas aeruginosa and one strain of Escherichia coli lower than those observed in other groups of subjects. Despite the partial obliteration of the hepatic blood outflow and the elevated portal pressure, hepatic clearance of the portal blood is efficient in chronic human schistosomiasis, and unlike alcoholic liver cirrhosis, avoids excessive stimulation of the immune system by a gut-derived antigens.

Hepatic eosinophil granulocytopoiesis in murine experimental Schistosomiasis mansoni.

In livers of mice with acute schistosomiasis eosinophil granulocytopoietic folliculi are described. Exogen stem cells were observed in hepatic sinusoids. Their proliferation and progressive maturation are described. Specific reactions to the presence of worms and their eggs, and Kupffer-cell stimulation and hyperplasia are considered to be responsible for this extramedullary granulocytopoiesis.

Mother-child relation in human schistosomiasis mansoni : skin test and cord blood reactivity to schistosomal antigens.

Reaction to schistosomal antigen was studied in children born to mothers infected with Schistosoma mansoni. Values of immediate (15 min) skin reactions were more elevated and values of delayed (24 h) skin reactions were significantly higher in children born to infected mothers than in children born to uninfected mothers. The macrophage migration inhibition test, done on cord blood cells, was positive to schistosomal antigen in 40% of children born to infected mothers and negative in all children born to uninfected mothers. These results suggest prenatal sensitization to schistosomal antigen in children born to mothers infected with S. mansoni.

Mother-child relationship in human schistosomiasis mansoni. I. Parasitic antigens and antibodies in milk.

Immunoglobulins, anti-Schistosoma mansoni antibodies, complement components and schistosome antigens were investigated in milk from mothers infected with S. mansoni. No significant differences of immunoglobulins or complement component levels were observed between infected and control mothers. Anti-S. mansoni antibodies were detected in the milk of 8 out of 25 infected mothers. A significant relationship was observed between serum and "4", were demonstrated in milk from infected patients by the double diffusion micromethod. The function of these immunologically active substances transmitted by milk from mother to child is discussed.
