[Effect of pregnancy and delivery on the function of the cervico-urethral closure. I. Prospective study of 27 women].

Urinary incontinence is a complication of pregnancy. Having carried out urodynamic studies in pregnancy and immediately after delivery in 27 primigravid women, the authors have been able to clarify the alterations that take place in narrowing and closing the neck of the bladder and urethra, and especially in the tone of the urethra and of the functional character of the organ, in connection with hormonal changes that occur in pregnancy. They also demonstrate an increasing instability of the bladder which is due to compression by the pregnant uterus and which makes the function of bladder neck closure more difficult. On the other hand no significant short-term effect of obstetrical trauma could be found.

[Effect of pregnancy and delivery on the function of the cervico-urethral closure. II. Retrospective study on late consequences of obstetrical trauma].

Adding together the obstetric histories and the urodynamic investigations carried out on 178 incontinent women has made it possible for the authors to work out the role that obstetric trauma can play in urinary symptoms. They are able to demonstrate the effects of repeated normal deliveries and also the results of traumata over and above those that occur physiologically and are sometimes undergone by the perineum.

[Phospho-calcium metabolism and bone histomorphometry in chronic alcoholism. Apropos of 25 cases].

Twenty five subjects, chronic alcoholics for more than five years, without any past history or clinical signs of portal hypertension or oedemato-ictero-ascitic decompensation, were examined by laboratory tests reflecting phosphoro-calcium metabolism and by transiliac bone biopsy with histomorphometric examination of nondecalcified bone. The subjects were fairly homogeneous in terms of liver function and liver disease, when present, was only minimal. The phosphoro-calcium parameters were generally normal, but hypomagnesaemia was observed. On the other hand, the histomorphometric study often revealed marked alterations: hyperosteoidosis: raised index of osteoid thickness (IOT) in 15 of the 25 cases, increase in the relative osteoid volume (ROV) in 12 cases and increase in the osteoid area (OA) in 8 cases. The bony trabecular volume (BTV) was only mildly reduced (13 out of 24) and there was increased resorption, reflected by an increase in the area of resorption (AR) seen in 15 cases, associated with a moderate increase in the area of periosteocytic lacunae (APOL). The essential changes consist of hyperosteoidosis with morphological osteomalacia in 60% of cases and reduction of the trabecular volume in 54%. Comparison of the different parameters reveals the following histomorphometric profile: BTV; AR; ROV; OA; IOT; APOL. Thus, histological bone changes occur in chronic alcholics before the appearance of signs of liver failure. These quantitatively minor changes are suggestive deficiency type bone.

[A critical study of technetium 99m and xenon 133 lung scintigraphy in patients with cystic fibrosis].

During investigations for cystic fibrosis, 41 children, most of whom were aged 0 to 7 years, were submitted to lung scintigraphy with technetium 99m first and xenon 133 subsequently. This procedure ensured accurate location of pulmonary lesions. The reliability of the radio-isotopic study was assessed through comparison to conventional clinical and radiological criteria, arterial blood gas determinations and pulmonary function tests. Perfusion scintigraphy with technetium 99m labelled macroaggregated albumin (n = 90) was compared to functional parametrical xenon, 133 study (n = 25) (perfusion-ventilation). The degree of lung disease was evaluated during serial examinations in order to improve prognostic prediction. Technetium 99m scintigraphy proves a reliable and sensitive procedure which accurately reflects the patients' clinical status when it is performed in the absence of patent pulmonary superinfection. Xenon 133 scintigraphy, which is more difficult to perform and interpret, has the advantage of providing a topographic and quantitative evaluation of pulmonary blood flow and ventilatory efficiency.

[Basal C peptide levels in response to intravenous tolbutamide in the detection of incorrect insulin therapy (or prolonged misuse)].

Fifty nine insulin dependant diabetics were hospitalised for a trial withdrawal of insulin: 17 patients rapidly showed signs of lack of insulin, 18 did not develop cetoacidosis but could not be stabilised on diet and oral hypoglycemic agents, 24 were stabilised without insulin. A statistical study (multifactorial analysis of correlations, plotting of ROC graphs) validated the classification of these diabetics into 3 groups. It also showed that in patients with hypoglycemia, the values of C-protein, and after intravenous injection of tolbutamide, were good predictive factors for insulin-dependance: all patients with basal C-protein less than 1,9 ng/ml could not be stabilised without insulin; when the basal C-protein greater than or equal to 1,9 ng/ml and the amplitude of response at the 5th min was greater than or equal to 0,4 ng/ml, the diabetes could be stabilised by diet and oral hypoglycemic agents in 90 p. 100 of cases. This institutes an easy, reliable and economic method of detecting abusive insulin therapy.

Prolactin receptors in human breast cancer.

Prolactin receptors have been measured in 92 human breast carcinomas. Both free and total receptors (after desaturation by MgCl2) have been looked for. Free receptors have been found in 46% of the cases, total receptors in 72%. Specific binding ranges from 0.8 to 8.0%. No correlation could be found between prolactin receptors and age, weight, menopausal status and pathological features (differentiation, histoprognostic grading, cellular density). A highly significant correlation has been found between prolactin receptors on the one hand and estradiol and progesterone receptors on the other.

[Sleep in adolescents and its disorders. A survey in schools].

OBJECTIVES: Numerous studies emphasized the high prevalence of sleep problems in adolescents. However, it remains to be seen whether these problems are developmental or if they are related to pathological conditions. In order to try to answer this question, we conducted an epidemiological study investigating the prevalence and correlates of sleep disorders in a population of high school adolescents. METHOD: The sample for this investigation consisted of 652 high school students (344 males and 308 females), aged 13 to 19 years (mean age: 15.1 1.2 years), attending colleges in the North of France. Data for the study were obtained by means of revised versions of the INSERM self-report questionnaires for adolescents. One questionnaire included 113 response choice items investigating the sleep parameters and the sociodemographic, behavioural, health, and environmental condition of the subject. This questionnaire was completed by a self-portrait questionnaire including 27 response choice items about mood, anxiety, instability and the quality of relations with others. In addition, a clinical examination was performed by the clinicians of the school health department in order to investigate the medical history of the subject and his (her) pubertal development. RESULTS: In a first step, statistical analysis showed some significant differences in sleep rates and troubles between males and females. If the total sleep time was found similar in males and females, females were found exhibiting significantly earlier bedtime (p<0.01) and waking time (p<0.005) than males. In addition, trouble falling asleep (p<0.0001), nightmares (p<0.001), need for daytime sleep (p<0.05), and stimulant pill intake (p<0.05) were found significantly more frequent in females. Then, the adolescents were classified into two subgroups. "Insomnia group" included the students who answered "often" or "always" to one of the five questions about: having trouble falling asleep, the occurrence of early awakenings, their need for daytime sleep, sleeping pill intake, and bad sleep quality. "Non insomnia group" included those who answered "never" or "sometimes" in response to the five questions. Results showed that 233 adolescents (35.7%) exhibited persistent sleep disorders, insomnia type. 40.2% of females and 31.6% of males were included in the "insomnia group" (p<0.05). Pubertal development was found significantly more advanced in the "insomnia group" adolescents, more particularly in females. In the same way, concerns about body weight (46.7% vs 38.8%; p<0.005), size and shape (15% vs 8%; p<0.01) were found significantly more frequent in the "insomnia group"; 32.3% of the "insomnia group" adolescents vs 17.9% in the "non insomnia group" reported health problems (p<0.0001). School difficulties were found significantly more frequent in the "insomnia group" compared to the "non insomnia group". The proportion of subjects who have previously repeated at least one school year (p<0.01), who reported bad school performances (p<0.01), and who reported to be "sometimes" or "often" away from school (p<0.01) was significantly higher in the "insomnia group" than in the "non insomnia group". In the same way, data showed that school investing was also significantly weaker in the "insomnia group" adolescents (p<0.01). The proportion of subjects who believed that their relations with parents (12.4% vs 6%; p<0.05), teachers (21.4% vs 10.5%; p<0.0001), or peers (10.2% vs 4.1%; p<0.002) was found significantly higher in the "insomnia group". Risk behaviours were also significantly more frequent in the "insomnia group" adolescents: suicidal ideation (25.1% vs 11.7%; p<0.001), suicide attempt (7.3% vs 3.6%; p<0.05), psychotropic drug use (11% vs 4.9%; p<0.02), regular stimulant drug use (10% vs 5.1%; p<0.005), regular tobacco use (7.8% vs 2.2%; p<0.001), regular alcohol use (13.3% vs 8.3%; p<0.05), and illicit drug use (5.2% vs 1%; p<0.001). Lastly, all the dimensions of the self-portrait were found significantly more affected or negative in the "insomnia group" than in the "non insomnia group". For example, the proportion of subjects who described themself usually sad (10.2% vs 3.4%; p<0.001), anxious (54.9% vs 40.5%; p<0.0001), downcast (41.1% vs 32.6%; p<0.05), or instable (56.9% vs 41.7%; p<0.0001) was significantly higher in the "insomnia group". CONCLUSION: In this descriptive study, if some data suggest that pubertal development may be involved in the sleep rates or troubles observed in adolescents, the results also show that persistent sleep disorders are significantly associated with physical, psychological or social difficulties. In summary, this study shows that it is essential to take an interest in the quality of sleep in adolescents, which may be a way to approach their psychosocial difficulties.

[Phosphocalcium metabolism in the elderly. Study of 101 subjects living in an institution].

Calcium and phosphorus metabolism was studied in 101 institutionalized subjects over 70 years of age (mean: 82.5 years). The study was performed in serum and urine; it included parathyroid hormone and calcidiol (250HD3) assays and radiological examination of bones with determination of Meunier's index and metacarpal cortical index. Calcidiol concentrations did not vary with age but were higher in people who left the institution and in men. In contrast, there was a significant age-group related increase of parathyroid hormone concentrations. This increase was accompanied by an increase of parathyroid hormone activity, as shown by a parallel fall in phosphorus reabsorption rate. These findings are in agreement with current pathogenetic theories on senile osteoporosis. Invalid subjects had higher urinary calcium and serum parathyroid hormone levels and a lower cortical index. Paradoxically, there was less vertebral collapse as evaluated by Meunier's index, which may suggest that very old patients develop progressive cortical bone hyperresorption entirely independent of sequelae from their former trabecular osteoporosis.

[Acquired idiopathic sideroblastic anemia. Apropos of 39 cases].

We report a series of 39 cases of acquired idiopathic sideroblastic anemia accounting for 12 p. 100 of the 330 cases of myelodysplastic syndrome diagnosed in our department in six years. Patients' mean age was high (70.8 years). Anemia was present in 92 p. 100 of the cases and was of the macrocytic type in 87 p. 100 of the patients. Only two patients (5 p. 100) had neutropenia and two (5 p. 100) had thrombocytopenia. Thrombocytosis was observed in 17 cases (43 p. 100); it was usually moderate. The mean proportion of medullary ring-sideroblasts was 35 +/- 14 p. 100. A clonal abnormality was found in 4 of the 11 karyotype examinations performed. The course of the disease was marked by recurrences of anemia which required regular blood transfusions in 82 p. 100 of the cases. In at least 30 p. 100 of the patients followed up for more than 30 months, these transfusions were responsible for secondary haemochromatosis. Conversely, in only three patients (7.5 p. 100) the disease progressed to acute leukemia. On the other hand, 4 patients (10 p. 100) developed thrombocythemia (greater than 1,000 x 10(9)/1). Median actuarial survival rate was 48 months. There were only two factors of poor prognosis: age over 70 and anemia with less than 8 g/dl haemoglobin at the time of diagnosis. Transfusion haemochromatosis is the principal risk in acquired idiopathic sideroblastic anemia: this risk can be prevented by iron chelating agents in patients with frequent blood perfusions.

Dynamic workflow model for complex activity in intensive care unit.

Co-operation is very important in Medical care, especially in the Intensive Care Unit (ICU) where the difficulties increase which is due to the urgency of the work. Workflow systems are considered as well adapted to modelize productive work in business process. We aim at introducing this approach in the Health Care domain. We have proposed a conversation-based workflow in order to modelize the therapeutics plan in the ICU [1]. But in such a complex field, the flexibility of the workflow system is essential for the system to be usable. We have concentrated on three main points usually proposed in the workflow models, suffering from a lack of dynamicity: static links between roles and actors, global notification of information changes, lack of human control on the system. In this paper, we focus on the main points used to increase the dynamicity. We report on affecting roles, highlighting information, and controlling the system. We propose some solutions and describe our prototype in the ICU.

[Insulin pharmacokinetics in insulin-dependent diabetics. Mathematical models].

The study of insulin pharmacokinetics must help for a better understanding of clinical phenomenons commonly observed among diabetic patients, when insulin therapy is administered either intra-venously (I.V.), or subcutaneously (S.C.). We realized this work in a population of insulin-dependent diabetics. None of the subjects had significant levels of anti-insulin-antibodies. We analysed the absorption profiles (SC(t] of radio-iodine labelled insulin after a subcutaneous injection, the plasmatic profiles (P(t] after an intra-venous bolus of cold insulin. We demonstrate that, using a convolution calculation, it is possible to predict the plasmatic in insulin kinetics after subcutaneous injection or during S.C. infusion, even if the infusion rates are varying. An estimation of the proportion of insulin which is degraded at the injection site can easily be done. This model may be useful for a better understanding of the kinetics of insulin among diabetics, but also to evaluate new modes of insulin administration.

[Simultaneous scintigraphy exploration using MDP - 99 Tc and 67GA in infectious osteoarticular pathology. Initial results].

A new investigation in infectious osteoarticular pathology is presented, involving the use of two tracers: one for bone metabolism (MDP - 99mTc), the other for the infectious process (67Ga). The initial results of a prospective study of 72 scintigraphic examinations are reported. Analysis of the results reveals the value in using 67Ga to determine the stage of development of the lesion and consequently to adapt the treatment. Classical bone scintigraphy, whose value remains the early detection of signs compared to radiography, does not allow us to follow the course of the lesion over time. The need to combine scintigraphic techniques has already been recognised (Lisbona, Gaucher) but there has usually been sequential meaning, apart from the risk of false negatives (early osteitis), repeated visits by the patients over a period of time. The simultaneous use of 2 tracers, technically possible, avoids these problems and produces a rapid result (generally 6 hours after the injection).
