Accessory mammary tissue associated with congenital and hereditary nephrourinary malformations.

BACKGROUND AND OBJECTIVES: The association between polythelia (supernumerary nipple) and kidney and urinary tract malformations (KUTM) is controversial. Some authors reported this association in newborns and infants. Case-control studies dealing with adult subjects are not found in the literature. The purpose of this study is to determine the frequency of the association between accessory mammary tissue (AMT) and congenital and hereditary nephrourinary defects in an adult population compared to a control group. METHODS: The study was performed in 146 white patients (123 men, 23 women) with AMT out of 2645 subjects consecutively referred to us for physical examination. The following investigations were undertaken: ultrasonographic examination of the abdomen and the kidneys, ECG, echocardiogram, roentgenogram of the vertebral column, urinalysis, and other laboratory tests. A sex- and age-matched control group without any evidence of AMT or lateral displacement of the nipples underwent the same examinations. RESULTS: Kidney and urinary tract malformations were detected in 11 patients with AMT (nine men, two women) and in one control. These data indicate a significantly higher frequency of KUTM in the AMT-affected patients compared to controls (7.53% vs. 0.68%, P < 0.001). A broad spectrum of KUTM was discovered in association with AMT: adult dominant polycystic kidney disease, unilateral renal agenesis, cystic renal dysplasia, familial renal cysts, and congenital stenosis of the pyeloureteral joint. CONCLUSION: Accessory mammary tissue offers an important clue for congenital and hereditary anomalies of the kidneys and urinary collecting systems. Patients with AMT should, therefore, be extensively examined for the presence of occult nephrouropathies.

Unusual features of primary dermatofibrosarcoma protuberans and its myxoid recurrence.

A case of Dermatofibrosarcoma Protuberans (DFSP) with unusual clinical and histological features is presented. The lesion recurred one year after surgical removal and showed myxoid degeneration of the entire lesion. Only a few cases with myxoid degeneration of the entire lesion are described in the literature. We found CD 34 stain, in combination with a panel of antibodies including fXIIIa, S100 and actin, useful in the diagnosis of this type of tumor in order to differentiate it from other neural, fibrous or muscular neoplasms.

Basal cell carcinomas of the auricular region: a study of 23 cases.

Basal cell carcinomas (BCCs) of the auricular region are not frequently reported, especially in the Japanese literature. Predisposing conditions such as sun exposure or frostbite are possibly involved in their development. In this study, we report our cases of auricular BCCs, discussing the obtained results, the possible significance of predisposing conditions, and the correlation with the histologic subtypes involved. Among 1094 patients with BCCs, auricular tumors were observed in 23 patients (2.1%), 4 women and 19 men. All of them were present on the external sun-exposed auricular side. Histologic patterns were nodular and micronodular (13 cases) and infiltrative (10 cases). No differences were observed between sexes. The relative degree of elastosis was higher in men than in women. Frostbite was recorded in 4 cases. The ear is an anatomical region that is heavily exposed to sun-light and equally prone to frostbite. Our data showed that all the lesions were located on the auricular region more or less exposed to sunlight. There was a recorded previous history of predisposing factors in most cases, and the high degree of elastosis suggests the involvement of these predisposing factors. Moreover, the high prevalence of infiltrative subtypes observed in our survey suggests a possible correlation between some histologic subtypes, sun-exposure, and frostbite. The differences between the relatively high number of auricular BCCs reported in the literature in contrast with the Japanese observations suggest the involvement of social or local conditions.

Age and site prevalence of histologic subtypes of basal cell carcinomas.

BACKGROUND: Basal cell carcinomas (BCC) are the most frequently diagnosed skin cancers. The relationships between some clinical parameters, such as sex, age, and location of the tumor, and the histologic patterns play an important role as factors predictive of recurrence and treatment implications. METHODS: A total of 693 patients with BCC, divided according to sex, age, and location of the tumor were studied. The histologic patterns considered were nodular, superficial, and morpheiform BCC and fibroepithelioma. The correlation between clinical and histologic parameters was analyzed. RESULTS: All histologic patterns considered were most commonly represented in the 7th and 8th decades. There were no sex differences. A prevalence of a superficial pattern for sun-unexposed areas and of a nodular pattern for sun-exposed areas was observed. The morpheiform pattern showed no correlation with any body site. The fibroepithelioma showed an absolute prevalence for the lumbosacral region. CONCLUSIONS: Age does not seem to be a discriminant or permissive factor for a particular histologic pattern of BCC. The prevalence of some subtypes for a particular body area may be explained on the basis of local predisposing conditions affecting the histologic development of the tumor.

Successful cryotherapic treatment and overview of multiple clear cell acanthomas.

BACKGROUND: Clear cell acanthoma (CCA) is a usually solitary benign epidermal-tumor. Multiple lesions are infrequently reported. OBJECTIVE: A case of multiple eruptive CCA is reported in which a successful cryotherapic treatment was performed. METHODS: A 59-year-old man with varicose veins and dry skin presented with 17 eruptive asymptomatic papulo-nodular lesions on both legs. RESULTS: Two lesions were excised with a histologically confirmed diagnosis of CCA. Other lesions were treated with liquid nitrogen. All the lesions resolved with minimal residual scarring after three to four treatments. CONCLUSIONS: Multiple CCA is a rare condition; fewer than 20 cases having been described. The presence of associated conditions like dry skin and varicose veins are discussed. Because of the tendency of persistence of the lesions, cryotherapic treatment may be a useful method of therapy.

Familial aberrant mammary tissue: a clinicoepidemiological survey of 18 cases.

BACKGROUND: Aberrant mammary tissue (AMT) is a common minor cutaneous anomaly located along the embryonic mammary line. It may be sporadic, familial or associated with nephrourinary malformations. OBJECTIVE: To investigate the frequency of family history of AMT and its possible inheritance. METHODS: We investigated a population of 156 adult Caucasian subjects affected with AMT. The familial background was evaluated primarily by using a questionnaire and by direct study of family members when available. Renal ultrasound examination was also performed to disclose the presence of kidney and urinary tract malformations. RESULTS: Eighteen patients (11.5%), 12 males and 6 females, had one relative with AMT (parents: 8, sons: 5, brothers/sisters: 3, other: 2). A male-to-male transmission was found in 9 cases (75%), female-to-female and female-to-male ones in 2 cases and a male-to-female one in 1 case. No patient had nephrourinary defects. CONCLUSIONS: The frequency of familial AMT in our study is very high and reflects both racial factors and the large number of the subjects surveyed. There is no distinctive clinical pattern for familial AMT, although pseudomamma (13 cases, 72%) and the left-sided location of the defect (66%) may suggest a 'hereditary predisposition'. Inheritance is consistent with an autosomal dominant transmission, although an X-linked dominant transmission is also possible. Finally familial AMT does not seem to be related to a higher association with nephrourinary malformations.

[Symmetrical bilateral apocrine hidrocystomas of the eyelids].

A case of a 37-year-old woman with symmetrical bilateral apocrine hydrocystomas at the lateral margins of the eyelids is presented. The symmetrical, simultaneous and bilateral appearance of the lesions, the lack of association with other abnormalities and the absence of the lesion in the family history make this case remarkable.

Extensive unilateral speckled lentiginous nevus.

We report a case of speckled lentiginous nevus with extensive unilateral distribution in a 53-year-old man. The lesions involved the left back, extending from the midline anteriorly to the thorax, arm, and forearm. The background pigmentary patch covered only the back and upper arm. Histologic examination revealed a lentiginous pattern as well as a congenital nevocytic pattern, even in the areas where the background was nontanned skin. This combined clinical pattern in the same patient fits the descriptions for both the usual and unusual speckled lentiginous nevus. Cases with variants of extensive unilateral distribution are infrequent and pathologists should pay particular attention to them because of the possibility of their malignant transformation.

Pre-existing cutaneous changes in basal cell carcinomas of the lower extremities.

Basal cell carcinomas (BCCs) of the lower extremities are not frequently reported. Some pre-existing or predisposing conditions are thought to be implicated in their development. In this study, we investigated the possible relationships between pre-existing cutaneous changes and the development of BCCs of the lower extremities, possible sex and age differences, and the histologic patterns involved. Among 1053 patients with BSSc, BCCs of the lower extremities were observed in 48 (4.5%): 25 men and 23 women. Lesions appeared on the thighs (16 cases), the lower legs (27 cases), or the feet (5 cases). Histologic patterns were nodular and micronodular (18 cases), superficial (27 cases), or infiltrative (3 cases). No differences were observed between sexes. The relative degree of elastosis was higher in the lower limbs of women than of men (2.25 vs 1.53). Pre-existing cutaneous changes were observed in 7 cases. The frequency of these BCCs of the lower limbs and the higher degree of elastosis observed in the women seem to be related to sun exposure. The histologic pattern seems not to be affected by environmental factors but rather characteristic of body site. Pre-existent cutaneous changes are not thought to be possible etiologic factors.
