Mutagens in larger fungi. II. The mutagenicity of commercial pickled Lactarius necator in the Salmonella assay.
In the course of an ongoing screening of larger mushrooms for the occurrence of chemical mutagens, 33 out of 48 species tested exhibited a significant direct mutagenic activity in the Salmonella/microsome assay (Sterner et al., 1982). (No mutagens requiring metabolic activation were indicated.) These findings are of some concern, since mushrooms are used extensively as food in many areas, and there are strong indications that carcinogens in food are of considerable importance in cancer aetiology (Sugimura, 1979). A recent communication by Knuutinen and von Wright (1982) on the mutagenicity of 4 Lactarius species collected in Finland prompts us to report our own results from mutagenicity tests with commercially preserved (pickled) Lactarius necator.
Recovery of cyclic antidepressants with gastric lavage.
The role of lavage fluid volume in recovery of cyclic antidepressant with gastric lavage was determined in 13 patients treated for moderate to severe cyclic antidepressant intoxication. An orogastric tube was placed, gastric contents aspirated, and gastric lavage performed with 60 to 180 mL aliquots of tap water or 0.9% NaCl for irrigation. A volume of 12.8 +/- 4.4 liters of lavage fluid was instilled, and 97.6 +/- 6.9% of this volume was recovered. The recovered aspirate and lavage fluid were inspected for the presence of particulate matter and saved in 4 to 5 liter collections. Determination of cyclic antidepressant and metabolite concentrations was performed using gas chromatography-mass spectrometry, and the amount of cyclic antidepressant recovered was calculated. The mean total amount recovered was 110 mg and ranged from 2.4 mg to 342 mg. Of the total amount recovered, 88% +/- 13% was recovered in the first 4 to 5 liters of lavage fluid. An estimate of the dose ingested was available in 7 patients, with 8.7% (0.4% to 21.7%) of the estimated dose recovered by lavage. There were no cases of significant drug recovery in clear lavage fluid. We conclude that if gastric lavage is undertaken in cyclic antidepressant intoxications, it should utilize 5 liters initially and then continue only if particulate matter is seen.
Dystrophic epidermolysis bullosa associated with eosinophilic infiltrate and elevated serum IgE.
An infiltrate of eosinophils is rarely seen in epidermolysis bullosa. We describe a child with dystrophic epidermolysis bullosa associated with a marked eosinophilic tissue infiltrate and elevated serum IgE.
Successful thalidomide treatment of severe infantile Behçet disease.
We describe an 11-month-old child with giant ulcers of the buccal mucosa, necrosis of the tongue, abdominal tenderness, and severe diarrhea due to Behçet disease. Treatment with thalidomide resulted in prompt recovery of the mucocutaneous lesions and gastrointestinal manifestations.
Post-operative computed tomography in two dogs with cerebral meningioma.
Post-operative computed tomography (CT) has been described as a technique for diagnosing incomplete resection or recurrence of cerebral neoplasms in humans. The characteristics of immediate postoperative CT images in dogs with intracranial pathology are unknown. This report describes findings from preoperative, immediate post-operative, and 4 week to 9 month follow-up CT examinations in two dogs with histologically-confirmed cerebral meningiomas. In images of one dog after surgery there was mild contrast enhancement of the tissue surrounding the surgical site. This enhancement had resolved in later images and was probably the result of surgically induced trauma. In post operative images of the other dog there was significant hyperattenuation of the tissues around the surgical site. In post contrast images there was increased enhancement that was evident in later images. These findings, although not supported by necropsy, probably indicate incomplete excision of the tumor.
The therapeutic use of dolls.
TOPIC: Using dolls as therapy in a psychogeriatric unit. PURPOSE: To acquaint nurses with this therapeutic modality. SOURCE: The Dolls Project. CONCLUSION: Dolls can be used therapeutically to awaken pleasurable affective responses in the older psychiatric client.
Gemcitabine-induced radiation recall dermatitis: case report.
A 65-year-old male with lung adenocarcinoma received radiotherapy to the mediastinum and right upper lobe, followed by chemotherapy with gemcitabine. Radiation recall dermatitis developed in the area corresponding to the radiotherapy portal. This is one of just a few cases reported recently concerning radiation recall dermatitis stemming from gemcitabine.
Israeli quality survey: research 'spin-offs'.
Quality assurance surveys in nursing are worthless unless the results can be translated into practical action. The author demonstrates how one such survey in Israel inspired Masters' students at Tel Aviv University to base their projects and theses on the survey findings, and produced intervention models and tools for improving care in several important areas of nursing. Encouraging the students to conduct these studies as 'spin-offs' from a major piece of national research has fostered a much broader understanding of quality assurance which has had a direct impact on patient care, and provides an example which nurses, educators and students in other countries can emulate.

Academia--a stimulus for change.
When the Department of Nursing at Tel Aviv University celebrated its 20th anniversary in 1988, interest was generated on this programme's impact on its graduates and on their contribution to nursing in Israel. Based on a previous study, a questionnaire was sent to 709 graduates, of which 46.8% responded, and a telephone survey was made of 20% of nonrespondents. Below, the study's revelations.
Essential mixed cryoglobulinemia type II.
We report a rare case of essential mixed cryoglobulinemia type II with membrano-proliferative glomerulonephritis (MPGN) type I in which HCV was not found. Long-term history of palindromic rheumatism, skin leukocytoclastic vasculitis attacks and micro-normocytic anemia preceded the appearance of cryoglobulinemia. Cryoprecipitate consisted of monoclonal IgMk-RF and polyclonal IgG (essential mixed type II). The newly appreciated cryoglobulinemia was associated with Coombs positive hemolytic anemia. The MPGN in this case had a benign course and responded to complex simple therapies including prevention of exposure to cold, low antigen content diet, treatment of provoking factors such as UTI, and maximal dose of ACE inhibitor. Responsiveness of skin vasculitis to colchicine therapy was restored after a two-month colchicine withdrawal period and therefore corticosteroid and immunosuppressive therapy was postponed.
Omissions in nursing research: another look.
Nursing research findings, even when relevant to solving a problem, are useless if not implemented. Who's to blame? And how can the value of such research be used to the fullest? In a previous article (INR, March-April 1984), Professor Bergman pointed the accusing finger at some of the nondoings and omissions in nursing research, which included inadequate communications between the researcher and practitioner, nurses' lack of motivation to study the findings or even to research themselves when possessing the skills. Below, she takes an expanded view of these omissions and poses some provocative questions for both researchers and practitioners.
