A cross-national comparison of the institutionalized elderly in the cities of New York and London.

Two random samples, each constituting 0.5% of the institutionalized elderly (over 65) in the cities of New York and London have been compared. This preliminary report of a collaborative study demonstrates the similarity of the 2 samples. Institutions in the 2 cities are shown to differ in size, type of ownership and the frequency that skilled nursing is provided. Some implications of these 2 findings are raised now, and will be discussed later when the full data for the study are presented.

Sign language aphasia in a non-deaf-mute.

A 19-year-old left-handed man, who was raised by deaf-mute parents and learned sign language concurrently with normal speech, sustained a traumatic cerebral contusion. He subsequently had no evidence of apraxic, visual-spatial, or sensorimotor deficits of the left limbs with which he was accustomed to use signs. Globally aphasic with a dense right hemiparesis, he initially recovered sign language to a greater degree than spoken language with a reversal on follow-up observations. Receptive skills improved to a greater degree than expressive skills with no marked difference between verbal and sign language, but with natural signs better preserved than finger spelling.

Second-year alcoholism treatment outcome evaluation with a focus on Mexican-American patients.

This is an evaluation of an Alcohol Treatment Unit with a focus on a comparison of Mexican-American and Anglo-American outcomes. Social adjustment on a target cohort of 75 was measured on a standard rating instrument (SAI) 2 years after discharge from the hospital. The overall successfulness of the program was 39%; the validity of the Treatment Difficulty Scale (a composite index of anticipated treatment difficulty) was demonstrated for both ethnic groups and both first and second year outcome results; and the average stability of first year findings of SAI components as compared to second year results was shown to be 79%. Use of a standardized index of pretreatment patient characteristics was recommended to facilitate comparative program evaluation research.

The blood supply of colorectal liver metastases.

Post-mortem studies suggest that liver metastases obtain the majority of their nutrition from the hepatic artery; however, cytotoxic arterial perfusion with or without hepatic-artery ligation has not proved entirely successful as a therapeutic regime. In this study we have measured blood flow into colorectal liver metastases using xenon-133 (133Xe) clearance in patients undergoing surgery for colorectal cancer. Pre-operative measurements after direct parenchymal injection gave a mean flow of 41.5 +/- 22.5 ml/min/100 g which after hepatic arterial occlusion perfusion, was reduced to a mean of 5% of the pre-occlusion value. Dynamic blood-flow studies using the gamma camera were performed in the post-operative period by administration of 133Xe into both hepatic arterial and portal venous catheters. The initial distribution images indicated a predominant arterial perfusion to the metastases, but after hepatic-artery ligation, portal-vein perfusion to the metastases was statistically significantly increased. Hence, a compensatory haemodynamic mechanism exists which may account for the poor results of hepatic-artery ligation and perfusion alone.
