[Reticulovascular radiologic pattern in primary arterial pulmonary hypertension].

We tested the value of the radiographic indexes in 16 patients with the diagnosis of pulmonary hypertension of unknown etiology. The index PL/T (lung-lobe-thoracic) was abnormal in 62.5%, the Osawa-Kanemoto index (DPA(T/2] was also abnormal in 62.5% of the cases, the r/h index (which measures the ratio right pulmonary artery thickness/right lung field) was abnormal in 81% of the cases. There was no relationship between the above mention indexes and the pulmonary artery pressure. Seven of the 16 patients displayed a new radiographic image that we have named "reticulo-vascular pattern", this radiographic sign may be important in the diagnosis of primary pulmonary hypertension.

[Respiratory function in extrinsic allergic alveolitis caused by pigeons. Clinical, pathological and functional correlation].

Lung function tests (LFT) were performed in 71 patients with proven extrinsic allergic alveolitis due to pigeons (EAA-P), and they were correlated with lung biopsy (LB) findings. Lung function studies were analyzed to evaluate the clinical course of these patients treated with corticosteroids. Restrictive pulmonary function impairment was found in all cases (vital capacity 38 +/- 4%), residual volume was increased in 9 out of 14 patients (64%) (RV = 0.51 +/- 0.06 L.) and bronchial obstruction was a feature in 11/14 patients (78%) (MMEF = 53 +/- 4%). In all cases a low Pa02 was observed (44 +/- 2 mmHg) and in six an increase in PaC02 was detected. The vital capacity did not correlate with the degree of inflammation or fibrosis observed by LB. A significant negative correlation was found between Pa02 and the degree of inflammation (r = 0.68, p X 0.05) as well as with fibrosis + inflammation degrees (r = -0.63, p less than 0.05). In general, initial LFT and clinical improvement occur simultaneously. Lung function tests support the diagnosis of EAA-P, but are not capable of separating inflammation from lung fibrosis.

[Microscopic pulmonary arteriovenous fistulas].

A case of pulmonary arterio-venous microfistula in a fifteen year old male is reported. He referred dyspnea and cyanosis for 3 and 2 years respectively. Physical examination was normal except for finger clubbing. Chest radiograph and EKG were also normal. Lung function tests showed hypoxemia due to an increase in veno-arterial shunt (Qs/Qt). Its localization was not apparent by routine studies such as phonocardiogram, echocardiogram, right heart catheterization and pulmonary angiogram. However, a contrast echocardiogram and selective occlusion of the right and left pulmonary arteries performed with a Dotter-Lucas catheter were suggestive of an intrapulmonary localization of the shunt. A lung biopsy confirmed the diagnosis.

Local inflammatory response and vector spread after direct intraprostatic injection of a recombinant adenovirus containing the herpes simplex virus thymidine kinase gene and ganciclovir therapy in mice.

We have evaluated the safety and potential toxicity of an adenoviral vector containing the herpes simplex virus thymidine kinase gene (adenovirus/Rous sarcoma virus thymidine kinase in a preclinical model for prostate cancer. Clinical grade vector prepared for human trials was injected directly into the dorsolateral prostate of C57Bl/6 mice in a volume of 5 microL at doses of 2.5 x 10(6), 2.5 x 10(7), or 2.5 x 10(8). The mice received intraperitoneal injections of either ganciclovir or saline twice daily for 6 days, beginning 12 hours after vector injection. Representative tissues and fluids were collected for evaluation the day after the final dose. Microscopic pathologic evaluation revealed inflammatory infiltration without necrosis within the dorsolateral and ventral prostate, but no necrosis or leukocyte infiltration was observed in sample tissues from lung, liver, large intestine, bladder, seminal vesicle, testis, or epididymis. DNA was extracted from the above tissues as well as pelvic lymph nodes, blood, seminal fluid, urine, and sperm and analyzed by polymerase chain reaction for the presence of vector sequences. The vector was readily detected in the dorsolateral prostate, the site of injection. The amount of vector detected was reduced in some samples from ganciclovir-treated animals. At the highest dose, vector spread was observed in the ventral prostate, seminal vesicle, testis, pelvic lymph nodes, gut, and liver. Spread to the testis was observed in only one animal. Vector DNA was not detected in urine, seminal fluid, or sperm but was detected in the blood of one animal. This adenoviral vector, therefore, appears to have minimal spread to sites distant from the site of injection and no detectable pathological sequelae within this dose range in this preclinical model for prostate cancer, which may be generalizable to other solid tumors.
