Splenic abscess: the rationale for selective non-surgical treatment.

Abscess of the spleen is a potentially lethal condition, with an associated mortality rate of 60-100% in untreated patients. Splenectomy, or in selected cases, surgical or percutaneous drainage of the abscess is currently considered the standard of care. So far, conservative treatment with antibiotics has been successfully employed in four patients, including the two reported herein. The importance of a careful clinical and ultrasonographic monitoring for best therapeutic decision-making in patients with splenic abscess, is stressed.

Interactions between muzolimine, PgE2 and the renin-angiotensin-aldosterone system.

UNLABELLED: The aim of our study was to determine the possible interrelation between muzolimine and the renal PG and renin-angiotensin systems by direct assay of these renal hormones. 4 groups of five rats were subdivided as follows: Group 1: controls; Group 2: rats treated with muzolimine (100 mg/kg); Group 3: rats treated with indomethacin (4 mg/kg); Group 4: rats treated with both muzolimine and indomethacin. After 3 days, the rats were transferred to metabolic cages, urine and plasma samples were taken on the 4th day and the following parameters assayed: Urine: creatinine, Na+, PGE2. Plasma: PRA and aldosterone. RESULTS AND CONCLUSIONS: Muzolimine: 1) increase PRA (11.8 +/- 1.06 vs. 8.66 +/- 1.56 AI ng/ml/h - p less than 0.01) and 2) increases PGE2 release (7.08 +/- 3.22 vs. 0.6 +/- 0.19 micrograms/mg urine creatinine - p less than 0.005). PG synthesis-inhibition (indomethacin) lowers (0.118 +/- 0.056 mmol Na+/mg urine creatinine of group 2 vs 0.043 +/- 0.019 mmol Na+/mg urine creatinine of group 4 - p less than 0.025) although not completely abolishes (0.043 +/- 0.019 nmol Na+/mg urine creatinine of group 4 vs. 0.019 +/- 0.006 mmol Na+/mg of group 3 - p less than 0.05) the natriuretic effect of muzolimine. This suggests that the natriuretic activity of the drug, at least in part, depends on PGE2 production.

[Immunostimulation with levamisol in inoperable cancer of the esophagus].

In the light of previous personal research indicative of immunodepression in patients with cancer of the oesophagus, the effect of aspecific immunostimulation with Levamisol was assessed on 70 patients with inoperable forms. All patients were treated by positioning of a Celestin endoesophageal prosthesis for the palliation of dysphagia, and submitted to a randomised protocol involving aspecific immunostimulation with 150 mg Levamisol per os twice a week on nonconsecutive days for an indefinite period. The mean survival of the treated group was not significantly better than that of the controls, nor was any real advantage detected on dividing the series in terms of age, tumour histology and degree of differentiation, extension, or the clinical stage.

[Castleman's benign lymphoma].

The Authors, when presenting a personal case of CBL, make a critical revision of the literature, summing up the evolution of knowledge of this morbid picture. They analyse and discuss the aetiological theories suggested by other Authors on the basis of clinical and anatomopathological criteria. Lastly they evidence the difficulty of diagnosis both for the clinical physician and for the anatomopathologist, since there are numerous pathological pictures which may simulate CBL in the symptoms when present, the x-ray aspects and even the anatomohistological examination.

[Cavernous hemangioma of the mediastinum].

The authors present a rare case of cavernous hemangioma of the mediastinum. After a critical review of existing literature on the subject, they discuss the symptoms of this vascular malformation with special emphasis on the difficulties of differential diagnosis versus other space-occupying lesions of the mediastinum. They further discuss the characteristic histopathologic features of cavernous hemangioma, and state that ablative surgery is the only possible therapeutic measure.
