Maternal morbidity in the Department of Obstetric and Gynaecology, Jinnah Postgraduate Medical Centre Karachi.

Maternal morbidity was observed in 377 obstetric cases at Jinnah Postgraduate Medical Centre (JPMC) during a period of one year. Morbidity rate was 4.4% of total deliveries. Three hundred and twenty-four (86%) mothers had no proper antenatal supervision. Infection was more common after abdominal delivery and injury to genital tract after vaginal delivery. The most serious injury was rupture of gravid uterus. Thirty-seven cases of ruptured uterus were admitted in one year and 5 cases occurred within the hospital (1 in 1225 deliveries).

Cross-Sectional Studies

Giardia lamblia in patients undergoing upper G.I. endoscopy.

To determine the frequency of giardiasis in patients undergoing upper G.I. endoscopy for dyspepsia and other upper G.I. disorders, duodenal aspirates were collected in 200 patients and simultaneous duodenal biopsies in 163 patients. Nine percent aspirates and 1.8% duodenal biopsies showed Giardia lamblia trophozoites. Giardia as a cause of dyspepsia should be considered in patients with negative endoscopy and in those who remain symptomatic inspite of adequate treatment for known upper G.I. disorders.

Bacteriological and protozoal study on domestic water supply in Karachi.

Four hundred and fifty eight water samples collected from domestic tanks and taps and community taps were analysed for bacterial contamination. Faecal pollution was more in domestic tanks and taps in Mehmoodabad, Korangi and least in water collected from community taps of Liaquatabad, Korangi and Mehmoodabad. Bacteria isolated were mostly E. coli, Enterobacter sp, Klebsiella sp. and A. faecalis, while other organisms (except Pseudomonas) were found in lower numbers. Salmonella para typhi and para typhi B, Shigella dysenteriae and Aeremonas sp. were found in Clifton area. Parasite (Fluke) was isolated from one water sample only.

Bacteriological study of genital tract infection in puerperium.

Of 185 vaginal and 13 wound swabs obtained from febrile females after delivery and abortions, 140 (75.6%) vaginal and 10 (76.9%) wound swabs were positive for bacteria. Infection caused by a single bacterium was found in 100 (71.4%) and mixed infection in 40 (28.5%). E. coli was mainly isolated followed by Staph. aureus and Klebsiella. Fosfomycin was the most effective antibiotic against E. coli and Gentamycin against Staph. aureus.
