Use of serum theophylline level as a guide to optimum therapy in patients with chronic obstructive lung disease.

Blood and urine samples were collected simultaneously with measurements of pulmonary function at 2-hour intervals for 8 hours after oral administration of short-acting (SAT) and long-acting theophylline (LAT) preparations in 15 patients with stable chronic obstructive lung disease (COLD) on long-term maintenance theophylline therapy. The relationship between pulmonary function tests, serum theophylline level, plasma and urinary adenosine 3'5' cyclic monophosphate (cAMP) was examined. The highest forced expiratory volume in one second FEV1 was obtained with STL of 12.8 micrograms/ml +/- 5.21 SD and 9.14 micrograms/ml +/- 6.15 (P less than .05) after administration of SAT and LAT, respectively. A further increase in serum theophylline level (STL) offered no therapeutic benefit, and, in fact, was associated with a fall in FEV1 in many instances. Plasma or urinary cAMP measurements did not correlate with STL. STL at which the best pulmonary function is achieved is quite variable in patients with COLD on maintenance theophylline therapy, is frequently less than 10 micrograms/ml, and can be determined only by repeated measurements of pulmonary function at different STL. Therefore measurements of STL are of limited value in guiding treatment with theophylline.

Value of analysis of the evolution of the pattern of the ST segment in exercise electrocardiograms.

We analysed retrospectively the exercise electrocardiogram records of all patients with positive exercise tests which, on subsequent coronary angiography, proved to be "false positives". The exercise records of 10 "false positive" patients were compared with the records of 10 patients with true positive tests in the setting of single vessel disease and 20 patients with true positive tests and multivessel disease. Patients proving to be false positives did not differ from those with true positive tests in respect to either the slope of the ST segment, the number and site of leads showing shift of the ST segment, or the peak exercise double product. The total duration of exercise, however, was significantly longer in patients having false positive tests (7.93 minutes SD 1.34) as compared to true positive results (5.95 minutes SD 1.9 in single vessel disease, and 4.4 minutes SD 2.2 in multivessel disease, P = less than 0.05). Eight of 10 patients having a false positive test showed an atypical evolution of the pattern of the ST segment. Seven had rapid normalization of depression of the ST segment immediately after cessation of exercise, while one showed shift of the ST segment only during the period of recovery. In contrast, 28 of 30 patients with true positive tests showed gradual normalization of the depressed ST segment during the period of recovery. We conclude that shifts in the ST segment that normalize rapidly on cessation of exercise are frequently a false positive finding.

Unmasking of sinus node dysfunction by ajmaline.

A 75-year-old male with bifascicular block presented with recurrent giddiness and presyncope. Prolonged monitoring of the cardiac rhythm did not reveal any arrhythmia. Electrophysiological evaluation, including an ajmaline stress test, was performed. Ajmaline had an unusual effect. It reproducibly induced sinus arrest and thus unmasked a latent sick sinus syndrome.

Doppler echocardiographic evaluation of normal and thrombosed Björk-Shiley mitral prosthetic valves.

Doppler echocardiographic characteristics of 75 normally functioning Björk-Shiley mitral prostheses were studied in patients whose valvar function was considered normal by clinical and fluoroscopic evaluation. The mean Doppler peak and end-diastolic gradients were 9.01 +/- 3.23 mm Hg, and 2.36 +/- 1.0 mm Hg, respectively. The mean area of the mitral valve calculated by the half-time method was 2.58 +/- 0.38 cm2. No significant difference between the various Doppler parameters was found for the two different valve sizes (25 and 27 mm) studied. Trivial mitral regurgitation was detected in 21 (28%) cases. Doppler echocardiography was performed in six patients with suspected malfunction of the Björk-Shiley mitral prosthesis subsequently confirmed at operation. The end-diastolic gradients in the six patients were 16, 20, 10, 14, 16, and 24 mm Hg, respectively. The calculated areas of the mitral valve were 1.57, 1.37, 1.3, 1.5, 1.46, and 1.3 cm2, respectively. The values of the gradient and calculated areas in malfunctioning valves were very different from those found in normally functioning Björk-Shiley mitral prostheses. It is concluded that Doppler echocardiography is a very useful noninvasive technique for the study of the function of the Björk-Shiley mitral prosthesis and provides quantitative information regarding pressure gradients and valvar area.

Keratomycosis in corneal sepsis.

510 cases of corneal ulceration were studied for the presence of fungus as a causative organism. Fungus was found in 87 (17.5%) most common fungus found was aspergillus. Mucor was found in 16 cases (18.1%) which is higher than earlier reports. History of trauma specially with vegetative matter and the application of steriods for one purpose or the other is a factor of importance as noted in this study.

Anterior tibial artery pseudoaneurysm.

Traumatic pseudoaneurysms are rare in civilian trauma, more so in childhood. A case of anterior tibial artery pseudoaneurysm is described which was successfully treated non surgically by percutaneous coil embolisation, thus questioning the need for surgery in peripheral pseudoaneurysms.

3,3',5'-Tri-O-methylpiceatannol and 4,3',5'-tri-O-methylpiceatannol: improvements over piceatannol in bioactivity.

Piceatannol (3,4,3',5'-tetrahydroxy-trans-stilbene) (NSC 365798) has recently been isolated and was subsequently synthesized for NCI tumor panel testing as a new antileukemic natural product from the seeds of Euphorbia lagascae. During the synthesis, a bioactive reaction mixture of several partially O-methylated piceatannol analogues was obtained. This mixture has now been maximized and subjected to bioactivity-directed fractionation, using brine shrimp lethality, to yield 3,3',5'-tri-O-methylpiceatannol (NSC 381281); this new compound has improved stability and better bioactivity in several systems than piceatannol itself. To confirm the structure, 5 was synthesized from vanillin. In addition, the isovanillin analogue, 4,3',5'-tri-O-methylpiceatannol (NSC 381864), another new compound, was synthesized and found to be bioactive.

Mitral valve repair: techniques and results.

From January 1986 to December 1989, seventy patients underwent mitral valve repair. Sixty-four patients had severe mitral stenosis (MS) and mitral regurgitation (MR), while six patients had severe mitral regurgitation (MR) only. The technique used was a combination of posterior semicircular annuloplasty, mitral commissurotomy and chordal shortening. There were two operative deaths. All except three of the surviving patients are asymptomatic. There has been no episode of thromboembolism. One patient has required valve replacement and two others have had a revision of the repair in the follow up period. Predischarge and late (3 months to 3 years) echocardiography suggests that the repair is satisfactory.
